This visit was conducted with Anahi Nixon, lead teacher. Your program currently operates with a FIVE (5) Star license effective 10/31/18. Restrictions include 1st shift daytime care; meets enhanced ratios; meets enhanced space; reduced ratios minus one. School age playground cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center play, outdoor gross motor play, and preparing for lunch. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold. The last annual compliance visit was conducted on 3/11/25. A sanitation inspection was completed on 9/9/25 with a Superior classification and four (4) demerits. The last fire inspection was conducted 10/29/25 by Nikki Hamby. Program records and required postings were monitored. A fire drill was conducted on 11/19/25 at 2:00 pm with one (1) minute fifty-nine (59) seconds to evacuate sixteen (16) children and two (2) adults by T. Stone. A lockdown emergency drill was documented on 9/11/25 at 1:15 pm with one (1) minute thirty (30) seconds to lockdown by T. Stone. An outdoor inspection was documented on 1/5/26 by A. Nixon. No new staff, one staff file, one substitute file and three (3) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. Today I observed Benadryl, and a Neffy nasal spray for a cinnamon allergy and a liquid Methylphenidate for ADHD. The program does provide transportation through the Wilkes County school system buses. Today I observed the Off-Site Transportation Records Verification signed by Dr. Westley Wood on 8/9/25 and the Transportation Policies for Parents signed by the parent of one (1) child who rides the Wilkes County school bus. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 8/30/24. A fire drill was recorded 8/28/24 at 2 pm with one (1) minute eighteen (18) seconds to evacuate five (5) children and two (2) adults by T. Stone. New lead teacher Anahi Nixon assisted me with the visit. Your program operates in CB Eller Elementary School and currently operates with a FIVE (5) Star license effective 10/31/18. Restrictions include 1st shift daytime care; meets enhanced ratios; meets enhanced space; reduced ratios minus one. School age playground cannot be used by preschoolers. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, circle time, and outside time. One (1) new staff were reported at this program, Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 8/30/24. A fire drill was recorded 8/28/24 at 2 pm with one (1) minute eighteen (18) seconds to evacuate five (5) children and two (2) adults by T. Stone. An emergency lockdown drill for the facility was last recorded 8/28/24 at 1:30 pm with fifty-eight (58) seconds to lockdown by T. Stone. The most recent fire inspection was on not available for review today. I observed a sanitation inspection was last conducted on 1/29/24 earning a Superior classification with fourteen (14) demerits. You stated that you do provide transportation for NC Pre-K children. Today I observed the School Bus Driver Responsibilities and Procedures orientation form. I was not able to observe the Public-School Off-Site Transportation Requirements form signed by Eric Barker, Director of Transportation for Wilkes County Schools. The following violations were cited during today’s visit: Violation Number Comment Rule 115 A summary of the NC Child Care Law was not posted in a prominent place in the center. I obserrved there was no Summary of the Law posted in this classroom today. This violation was corrected during the visit as a Summary of the Law was printed for Ms. Nixon today for her to post in the classroom. G.S. 110-102 832 There was no written emergency medical care (EMC) plan. I observed there was no emergency medical care plan posted in this classroom today. Ms. Nixon stated that she was meeting with Allison Wingerson with Wilkes County Schools this afternoon to complete the form and post in the classroom upon completion. 10A NCAC 09 .0802(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Storage and administration of medication were monitored. It was reported to me by J. McGrady that one child in the NC Pre-K classroom, is a diabetic and the diabetic medication paperwork is kept in the office with the school secretary. The diabetic medication is kept in the classroom in a pouch in a locked closet. Medication authorization was monitored in the office today. You stated that you do provide transportation am and pm for Pre-K children. During today’s visit, I observed the Public Schools Off-site Records Verification - Staff Records, signed by Dr. Westley Wood, Assistant Superintendent of Personnel and Human Relations, on 8/23/23. I observed the Public-School Off-Site Records Verification -Transportation Requirements signed by Eric Barker Director of Transportation signed on 8/23/23. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are long-erm substitute lead teacher, J. McGrady and teacher, K. Ponder. Ms. Ponder was absent today helping at another NC Pre-K location in the county. Ms. McGrady has her B-K degree with twenty-eight (28) years’ experience as a retired NC Pre-K teacher. Ms. Ponder has an AA degree in Early Childhood A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. The children are learning about Spring and Bugs. You completed the Dial-4 screenings for February. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/23. The classroom operates from 7:30 a.m. to 2:45 p.m. Parent conferences are held two (2) times per year. You stated that you use Class Dojo to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by inviting parents into the classroom for special activities and allowing parents to send materials and snacks into the classroom for special projects. The environment rating scale score received on 10/5/2018 was 6.28. The following violations were cited at today’s visit. A lockdown drill was documented on 2/21/24 at 9 am by T. Stone. An outdoor inspection was documented on 3/1/24 at 10 am by J. McGrady with no issues noted. Two (2) staff files and two (2) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. It was reported to me by J. McGrady that one child in the NC Pre-K classroom, is a diabetic and the diabetic medication paperwork is kept in the office with the school secretary. The diabetic medication is kept in the classroom in a pouch in a locked closet. Medication authorization was monitored in the office today. You stated that you do provide transportation am and pm for Pre-K children. During today’s visit, I observed the Public Schools Off-site Records Verification - Staff Records, signed by Dr. Westley Wood, Assistant Superintendent of Personnel and Human Relations, on 8/23/23. I observed the Public-School Off-Site Records Verification -Transportation Requirements signed by Eric Barker Director of Transportation signed on 8/23/23. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are long-erm substitute lead teacher, J. McGrady and teacher, K. Ponder. Ms. Ponder was absent today helping at another NC Pre-K location in the county. Ms. McGrady has her B-K degree with twenty-eight (28) years’ experience as a retired NC Pre-K teacher. Ms. Ponder has an AA degree in Early Childhood A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. The children are learning about Spring and Bugs. You completed the Dial-4 screenings for February. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/23. The classroom operates from 7:30 a.m. to 2:45 p.m. Parent conferences are held two (2) times per year. You stated that you use Class Dojo to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by inviting parents into the classroom for special activities and allowing parents to send materials and snacks into the classroom for special projects. The environment rating scale score received on 10/5/2018 was 6.28. The following violations were cited at today’s visit. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. I observed there was no documentation that the EMC plan was reviewed annually with the substitute teacher. 10A NCAC 09 .0802(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. I observed the record was incomplete for emergency shelter-in-place and lockdown drills as the time it took to reach a designated location was not documented. .0604(u);.0302(d)(8) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. I observed there was no documentation that the EPR Plan was reviewed annually for substitute in teacher position. .0607(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Monday, April 1, 2024. Refer to Child Care rule 10 A NCAC .09 .0607(e). • There was no documentation of review of the Emergency Medical Care for substitute or for long tern substitute for the lead teacher. Refer to Child Care Rule 10 A NCAC .09 .0802(a). Additional Information/Consultation: • Remember standing water is to be emptied daily to prevent the spread and breeding of mosquitos. Ms. J. McGrady stated she will have janitor drill a hole in the bottom of the large metal pot used as a musical instrument on the fence. • Substitute L. Williams FA/CPR has expired and needs to sign up for the training. • You must notify your consultant 30 days prior to changing the ownership status of your facility. • You are in Cohort 1 which means your prep year is going on now, 7/1/23-6/30/24 and your license reassessment year will be 7/1/24-6/30/25. • I have sent you tasks and activities to complete during your prep year. If you need me to resend these tasks and activities, please let me know. • In the meantime, be sure all NCPK staff have as much education as they can receive. You must send the official transcripts into DCDEE WORKS for evaluation prior to your license reassessment period. • Contact your Wilkes County Partnership for children to come out and conduct mock assessments in preparation for your license reassessment. Jessica Sale, new lead teacher, and teacher assistant, Katherine Ponder, assisted me with the visit. Your program currently operates with a FIVE (5) Star license effective 10/13/18. Restrictions include 1st shift day time care; meets enhanced ratios; meets enhanced space; reduced ratios minus one; school age playground equipment cannot be used by preschoolers. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having freee choice center time, circle time, and outside time. Two new staff were reported at this program, one new lead teacher, Jessica Sales, and one new principal, Ms. Teresa Stone. Forms for Pre-Service for Administrator will be sent to Ms. Sales and Ms. Stone to complete and scan back to me. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Ms. Sale has not completed the Shaken Baby Abusive Head Trauma training which is required prior to caring for children. There was no medical statement available for review, in her file. It was observed that there was no documentation of the TB screening or test available for review in her file. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 8/25/23. A fire drill was recorded 8/30/23. A shelter in place drill for the facility was last recorded 3/9/23. There was no Shelter-in-place in June as this public-school program closes in May. There has been no emergency drill for shelter-in-place or lockdown since school stated back at this NC Pre-K program. The most recent fire inspection was on 9/6/22. I observed a sanitation inspection was last conducted on 1/27/23 earning a Superior classification with six (6) demerits. You stated that you do provide transportation in the am & pm for NC Pre-K children. Ms. Sale stated that all emergency forms for children who are transported on the bus are located on the bus the child rides. It was observed that the Public School Off-Site Records Verification form was completed on 8/22/23 by Westley Wood of Wilkes County Schools. The following violations were cited during today’s visit: Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months.
