Your program currently operate with a 3-star license, effective December 6, 2025, earning 4 points in the Education Standards component, 2 points in the Program Standards component and 1 point in Quality for restricting infant capacity to no more than two children under one year of age. License restrictions include 1st, 2nd, and 3rd shift, children must play in fences area when outside & will not serve no more than 2 infants under 1 year of age. Jamie Tate, home operator, was present with seven children, ages 1-4 years. I explained to Ms. Tate that this is a violation of G.S. 110-91)(7)-For FCCHs, capacity rules define how many children an operator can care for simultaneously, including the operator's own preschool-age children: Option 1: up to 8 children total(max of 5 children ages 0-5, plus 3 school-age children up to age 13) Option 2: Up to 9 children total (max of 3 children 0-24 months, 3 children ages 2 to 5, and 3 school-age children up to age 13) Option 3: Up to 10 children total, provided all children are older than 24 months. I explained to Ms. Tate that preschool-aged children are children ages 0-5 years. In order to be determined a school-age, the child must have attended the first day of Kindergarten. I observed the children participating in teacher-directed activities and preparing for lunch. Adequate handwashing was completed. Today's lunch consisted of chicken with rice, applesauce, bread and water. I found interactions and supervision to be in compliance. During today’s visit I monitored the following: program space, program records, interactions, supervision, equipment and furnishings, medication administration, indoor and outdoor learning environments. Ms. Tate stated she does not provide transportation. I monitored Ms. Tate's file and all children records. Two violations of child care requirements were observed. Computer-generated report was reviewed with Ms. Tate and a copy was printed at the conclusion of today’s visit. Violation Number Comment Rule 101 Number of children exceeded number allowed. Seven (7) children were present with two children that were one year of age. Only six (6) preschool children can be present when a 1 year old is present. This program currently operates with a temporary license effective June 5, 2025 to December 5, 2025. Applicable child care requirements were discussed with you, Jamie Tate, Operator. All indoor and outdoor areas approved for child care were monitored during today’s visit. Three (3) children between the ages of 1 and 4 years old were present. The children were observed eating breakfast, which consisted of waffles, diced pineapples, blueberries, and milk. The children and operator were observed washing their hands after eating. Children were observed during free play activities. The children were adequately supervised, and appropriate interactions between the operator and children were observed during the visit. The following were also monitored during the visit: supervision; capacity; CPR/FA, ITS SIDS; medicine and hazardous product storage; adequate/approved space; staff records; program records; children records; posted license; and permit restrictions. During today’s visit you requested to increase your capacity to ten (10) children. During the visit I verified that you met all requirements to increase your capacity to ten (10) children. I received your approved zoning letter dated October 6, 2025, during the visit. I discussed the following capacity requirements with you: A maximum of three children from birth to 24 months of age, plus three children from 2 to 5 years of age and three school-age children up to 13 years of age, for a total of nine children. Note: Mixed age groups consisting of children aged zero-twenty four months, children aged 2-5 years, and school-aged children will be monitored as follows: A maximum of nine children, with no more than three children aged zero to twenty-four months and of the six additional children, three must be school age. A maximum of 10 children if all children are older than 24 months of age. I recommend that you make a copy of the above capacity requirements and post as a reference to ensure that the capacity requirements are met at all times. This was corrected during the visit when the plastic bags were stored so they were not accessible to the children. .1719(a)(18) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. This was not on file for one child enrolled. .1726(b)&(c) The five (5) violation/s documented above must be corrected immediately.
