The annual compliance monitoring checklist for Child Care Centers was used to note requirements monitored today. The last annual compliance was conducted on June 16, 2025. Upon my arrival, Ms. Sheilah Rowell, Administrator, was in space #1 providing care to three one year old children. You currently operate with a 4-star rated license issued October 23, 2017. License restrictions include: 1st shift, meets enhanced space, children under 2 1/2 years old in rooms with direct exits only. The NC Secretary of State website was reviewed prior to today's visit and the childcare facility was listed as current and active. I was unaccompanied as I completed a general walk-through of the facility. A total of nine children were present during today’s visit. I observed the children participating in free choice play, completing routine care tasks and having lunch. Today’s lunch was consisted of hamburger on bun, broccoli, fruit cup and milk REQUIRED INSPECTIONS/DRILLS: The most recent annual fire inspection was completed on February 18, 2026. The most recent annual sanitation inspection was completed November 7, 2025, and received a Superior classification with 13 demerits. The Emergency Drill Log was reviewed and the monthly fire drills and shelter-in-place/lockdown drills are current. All staff files were reviewed. I completed the staff and training worksheet during today's visit. Three children's files were reviewed. I completed the Children's Records form during today's visit. Overall enrollment and attendance information were collected. The attached Space Capacity and Enrollment Worksheet reflect this information, along with the names of staff present and actively caring for children during this visit. The following items of non-compliance of childcare requirements were observed and documented during today’s visit. Violation Number Comment Rule 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. The cribs in space #1 were not individually assigned and identified. 15A NCAC 18A .2821(b) & (c) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The substitute caregiver's first aid certification expired May 2026. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The substitute caregiver's CPR certification expired May 2026. .1102(d) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The administrator and substitute caregiver were not listed in the ABCMS Provider Portal. G.S. 110-90.2 & .2703(r) The violations must be corrected immediately. In your response to the violations, a compliance letter must be received by June 17, 2026 that explains in detail how each violation was corrected and how compliance will be maintained. You can send the compliance letter to me at Miriam.Byrd@dhhs.nc.gov. Failure to submit your compliance letter by the stated deadline or failure to contact me regarding a revised timeline may result in a follow-up visit, and violations may be cited. If you are unable to meet the required deadline of June 17, 2026, you must contact me prior to that date with a proposed timeline for completing the required corrections. In some cases, this time frame may be extended. If your compliance letter states that corrections have been made and it is later determined they were not, this will be considered falsification of information, and an administrative action may be taken against your license. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during this visit, may negatively affect your compliance score. As of June 1, 2026, your compliance history is 99%. Thank you for your continued commitment to maintaining compliance. Reminder: While reviewing children's records, I noticed the Transportation Permission form was present for children that are under the age of 3. Since children under the age of three can not be transported, you do not need to include this form in the enrollment packet. Child Care rule .1005(b)(1) -children under the age of three can not participate in off-premise activities that involve being transported in a motor vehicle. During the TA visit conducted on May 13, 2026, we thoroughly reviewed the Pathways to the Stars. You stated at that time you were interested in Pathway #2, Classroom and Instructional Quality. Adequate handwashing was observed. A sanitation inspection was complected on April 1, 2025, with a Superior rating. A fire inspection was completed on February 14, 2025. The Annual Compliance Monitoring Checklist for Child care Centers was used today to note requirements that were monitored. The program operates with a 4-star rated license issued October 23, 2017, earning 7 points for education standards, 2 points for program standards, and one quality point for complying with a programmatic option. License restrictions include: 1st shift, meets enhanced space, children under 2 1/2 years old in rooms with direct exits only. Today’s visit was conducted with the Owner/Administrator, Sheila Rowell. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed Preschool-School-Age children playing in various activity centers, teacher lead activities, and outdoor play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. Three infants were observed during today's visit. They were participating in daily routines and free play. A sanitation inspection was completed on November 16, 2023 with 6 demerits and a Superior rating. A fire inspection was completed on January 30, 2024. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheet was obtained. A review of the information was completed. Two staff files were reviewed during today’s visit. During today’s visit, three children records were reviewed. The children’s records form was completed during the visit. Two violations of child care requirements was observed during today's visit. Computer-generated report was completed at the conclusion of today's visit and a copy was left with Ms. Rowell. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The following was observed on the outdoor play area: 1) A vine was running alongside the building near the exit door, accessible to the children; 2) Weeds are growing in the two fall zone areas. 10A NCAC 09 .0601(a) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One child did not have a signed discipline policy on file. .1804(c) Child care programs must be in compliance with applicable child care requirements at all times. The violations must be corrected immediately. Please stated in your compliance letter how the violations were corrected. The compliance letter must be received by August 31, 2024. Health and Safety training requirements were monitored. I verified via Clean Classrooms for Kids website the Lead in water test has been completed and results are on file. The Lead-Based Paint and Asbestos testing is waiting survey review by RTI. During today's visit, Ms. Rowell had a vehicle on site that she is considering purchasing for transportation purposes. The vehicle was monitored. Vehicle- 2017 Dodge Grand Caravan Dealer tags- 136155 Inspection: 10/24 Transportation requirements were reviewed with Ms. Rowell. Emergency information for each child transported with identification is on file. Insurance is current. Based on the observation of the vehicle, only 5 children can be transported in the vehicle. No one can occupy the passenger side front seat due to the airbag. Reminders: 1. Ms. L Williams needs to upload a copy of the NCECC test out certificate to her WORKS account and request equivalency for Lead Teacher Rated License: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. During today's visit, Ms. Rowell stated she would like to proceed with the Environment Rating Scales. The rated License Assessment request review was completed during the visit. I also shared the NRLAP website (www.ncrlap.org) with her during the visit. The Annual Compliance Monitoring Checklist for Child care Centers was used today to note requirements that were monitored. The program operates with a 4-star rated license issued October 23, 2017, earning 7 points for education standards, 2 points for program standards, and one quality point for complying with a programmatic option. License restrictions include: 1st shift, meets enhanced space, children under 2 1/2 years old in rooms with direct exits only. Today’s visit was conducted with the Owner/Administrator, Sheila Rowell. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed Preschool-School-Age children playing in various activity centers, teacher lead activities, and outdoor play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. Three infants were observed during today's visit. They were participating in daily routines and free play. A sanitation inspection was completed on November 16, 2023 with 6 demerits and a Superior rating. A fire inspection was completed on January 30, 2024. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheet was obtained. A review of the information was completed. Two staff files were reviewed during today’s visit. During today’s visit, three children records were reviewed. The children’s records form was completed during the visit. Two violations of child care requirements was observed during today's visit. Computer-generated report was completed at the conclusion of today's visit and a copy was left with Ms. Rowell. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The following was observed on the outdoor play area: 1) A vine was running alongside the building near the exit door, accessible to the children; 2) Weeds are growing in the two fall zone areas. 10A NCAC 09 .0601(a) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One child did not have a signed discipline policy on file. .1804(c) Child care programs must be in compliance with applicable child care requirements at all times. The violations must be corrected immediately. Please stated in your compliance letter how the violations were corrected. The compliance letter must be received by August 31, 2024. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Health and Safety training requirements were monitored. I verified via Clean Classrooms for Kids website the Lead in water test has been completed and results are on file. The Lead-Based Paint and Asbestos testing is waiting survey review by RTI. During today's visit, Ms. Rowell had a vehicle on site that she is considering purchasing for transportation purposes. The vehicle was monitored. Vehicle- 2017 Dodge Grand Caravan Dealer tags- 136155 Inspection: 10/24 Transportation requirements were reviewed with Ms. Rowell. Emergency information for each child transported with identification is on file. Insurance is current. Based on the observation of the vehicle, only 5 children can be transported in the vehicle. No one can occupy the passenger side front seat due to the airbag. Reminders: 1. Ms. L Williams needs to upload a copy of the NCECC test out certificate to her WORKS account and request equivalency for Lead Teacher Rated License: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. During today's visit, Ms. Rowell stated she would like to proceed with the Environment Rating Scales.
