The last annual compliance was conducted on February 4, 2025. Today's visit was conducted with NC Pre K Staff and Mrs. Smith, Principal. You currently operate with a 5-star license, issued August 1, 2022, earning 7 points in Staff Education, 7 points in Program Standards, and 1 point in Quality by meeting a programmatic option. Restrictions on the license: Daytime care only, meets highest voluntary enhanced requirements, meets enhanced ratios, meets enhanced space. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, and free play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A sanitation inspection was completed on October 8, 2025 with 8 demerits and a Superior rating. The last fire inspection was completed on September 20, 2025. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheet was obtained. A review of the information was completed. During today’s visit, nine children’s records were reviewed. The children’s records form was completed during the visit. Two violations of child care requirements were observed and documented. Each were thoroughly reviewed with Mrs. Smith. Today's visit summary report was printed at the conclusion of today's visit. Along with the visit summary report, the following forms were left with Mrs. Smith: Annual Compliance Monitoring Checklist, Staff and Training Worksheet, Children's Records form, and Staff/Child Ratio form. Violation Number Comment Rule 1314 Emergency information did not name childs health care professional. Two children's emergency information did not include the hospital preference. .0802(c)(2) 1757 A valid qualification letter was not on file and available to review at the facility. One caregiver's qualification letter expired November 19, 2025. A new qualification letter has not been issued. G.S. 110-90.2(b) & (d) & .2703(e) Child care programs must be in compliance with applicable child care requirements at all times. The violations must be corrected immediately. You must email or mail written verification addressing the individual violations and how they were corrected. The compliance letter must be received before or no later than January 29, 2025. You may email the compliance letter to: Miriam.Byrd@dhhs.nc.gov You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. During today's visit, I verified Ms. Lakeshia Hannah's criminal background qualification letter expired November 19, 2025. I checked the criminal background in the ABCMS Portal and Ms. Hannah completed a current check on January 6, 2026; however, it was still being processed. Ms. Hannah contacted the CBC Unit and was told that her fingerprint card was never received. Ms. Hannah then contacted the Bladen County Sheriff's office and was told they had put the wrong code on the cards and the fingerprints were being electronically submitted at that time with the correct code. DCDEE will allow Ms. Hannah fifteen (15) days to get the new criminal background check completed and a copy of qualification letter on file. The violation cited must be corrected within the fifteen (15) days and a copy of the qualification letter be mailed to me with the required compliance letter. If a current qualification letter has not been obtained within 15 days, Ms. Hannah cannot be at the facility until she receives the qualification letter. QRIS: Pathways to the Stars: On October 9, 2025, I met with Kimberly Harris, Program Specialist Accountability, NC Pre Kindergarten & Title I, regarding QRIS for the Pre-K's in Bladen County Schools. The Conversation template was completed with a plan for Elizabethtown Primary Pre-K to apply in Spring 2026. I was informed that the program will have an Outreach ECERS-3 Assessment completed between March 2-27, 2026. This assessment does not count towards the QRIS: Rated License Process. Please contact me when the program is ready to apply and I will conduct the Rated License visit. In the meantime, make sure that all teaching staff (Lead Teachers and Teacher Assistants) are in WORKS and have a current Status Letter on file showing their Education qualification status. Restrictions on the license: Daytime care only, no children under 3, meets enhanced ratios and space, meets enhanced ratios minus 1. The indoor and outdoor areas were monitored. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, free play and preparing for lunch. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A sanitation inspection was completed November 21, 2024 with 4 demerits and a superior rating. The last fire inspection was completed on September 11, 2024. The Emergency Drill log was reviewed during today’s visit. Prior to today’s visit the staff and training worksheet was obtained. A review of the information was completed. Three staff files were reviewed during today’s visit. During today’s visit, five children’s records were reviewed. The children’s records form was completed during the visit. There were no violations of child care requirements observed during today's visit. The last annual compliance visit was conducted on February 13, 2024. The program currently operates with a Five-Star license, issued August 1, 2022 with 7 points earned in Staff Education, 7 points in Program Standards, and 1 point in Quality by meeting a programmatic option. License restrictions include 1st shift, meets highest voluntary enhanced requirements. Ms. Kristy Smith, Principal, was on-site during today's visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. I observed in three of the five classrooms, the Fire Drill Log section of the Emergency Drill Log and Report form was incomplete. The time of drill and time to evacuate and signature of person conducting the drill were not documented. I observed one group of children eating lunch in their classroom. Today's lunch consisted of hot dog with bun, tator-tots, fruit cup (peaches) and milk. No violations of child care requirements were observed. Documenting fire drills is important because it provides a record of compliance with safety regulations, allows for identification of areas needing improvement in evacuation procedures, demonstrates staff competency, and can be used to track trends over time, ultimately helping to ensure a safer environment in case of a real fire emergency; essentially, it serves as proof that fire drills are being conducted properly and that necessary actions are being taken to address any issues that arise during drills. Technical Assistance was offered on promoting language during free play in the classroom used by the EC children. Language aids in a nonverbal child’s cognitive development. Conversations using descriptive and reasoning words help build a child’s vocabulary and language understanding. There are plenty of opportunities outside of a planned group time for conversations and language building activities. Children learn through play and interaction. Staff can use opportunities such as meals and snacks in addition to free play to talk to the children. The program currently operates with a Five-Star license, issued August 1, 2022 with 7 points earned in Staff Education, 7 points in Program Standards, and 1 point in Quality by meeting a programmatic option. License restrictions include 1st shift, meets highest voluntary enhanced requirements. Ms. Kristy Smith, Principal, was on-site during today's visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. I observed in three of the five classrooms, the Fire Drill Log section of the Emergency Drill Log and Report form was incomplete. The time of drill and time to evacuate and signature of person conducting the drill were not documented. I observed one group of children eating lunch in their classroom. Today's lunch consisted of hot dog with bun, tator-tots, fruit cup (peaches) and milk. No violations of child care requirements were observed. Computer-generated report was completed at the conclusion of today's visit and a copy was left with Mrs. Smith. Based on the information obtained from reviewing staff records, two violations of child care requirements were observed and documented during today's visit.
