Name of Operation: ELIZABETHTOWN HEAD START Facility ID: 09000154 Consultant: MIRIAM BYRD Operation Type: Center Case Number: Visit Date: 9/20/2023 Number Present: 14 Completed Date: 9/20/2023 Age: From 2 To 4 Total Minutes: 120 Time In: 09:45 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor your program for compliance with applicable child care requirements during a routine unannounced visit. Annie Graham, Administrator, was present during today's visit. The program currently operates with a Three-star rated license issued July 30, 2021 with 3 points earned in Staff Education, 3 points in Program Standards and 1 point in Quality by meeting a programmatic option. Restrictions: Daytime care only, meets enhanced ratio and enhanced space. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were observed eating breakfast, that consisted of cereal or oatmeal, pineapples or applesauce. 1% unflavored milk was served. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. A sanitation inspection was completed on May 21, 2022. A sanitation inspection has not been completed since. Ms. Graham has been in contact with the Environmental Health Specialist in regards to the delinquent inspection. A fire inspection was completed on May 2, 2023. One violation of child care requirements was documented during today's visit. Computer-generated report was printed at the conclusion of today's visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. A sanitation inspection has not been completed since May 18, 2022. 10A NCAC 09 .0304(b) The violation must be corrected immediately. You are required to submit a signed and dated compliance letter that explains how the violation was corrected. The compliance must be received by October 4, 2023. You can mail or email the letter to: Miriam Byrd, Child Care Consultant, PO Box 1731, Clinton, NC 28329 Miriam.Byrd@dhhs.nc.gov If you state in your letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your current Compliance History score is 100%. During today's visit we discussed the upcoming Rated License Requirements. Rated License: The Division of Child Development and Early Education is starting the Rated License Assessment back to get programs current with their Rated License. You are in Cohort 2. Therefore, your Prep year will July 1, 2024 and will end June 30, 2025. During this time, you need to make sure all full-time staff have completed the NCECC and their information in WORKS is updated. This means submitting any certificates or an original transcript showing EDU coursework completed is in WORKS. If you are wanting the Environment Rating scales completed to earn more points in Program Standards, please take advantage of all the trainings they have on their website: ncrlap.org. Your Assessment year will begin July 1, 2025, and you must complete the Rated License Assessment by June 30, 2026. During today's visit I assisted Ms. Graham with training opportunities that are available through CCR&R Region 4. I also advised Ms. Graham to contact NCRLAP regarding the Prep Year Assessments. A copy of the NCRLAP Prep Year ERS Assessment flier and the Prep Year Activities and Ideas for Cohorts 1, 2 and 3 were given to Ms. Graham. Your Annual Compliance visit is due prior to January 31, 2024. Make sure to have the staff and training worksheet completed and submitted to me by December 1, 2023.
