The annual compliance visit was conducted on October 7, 2025. This facility currently operates with one NC Pre-K classroom, with a 5-star rated license issued October 1, 2025. An ECERS-R was last completed on March 12, 2019. The facility operates a minimum of 6.5 hours a day, attendance is taken, USDA approved meals are served, and menus are planned at least one week in advance. The center uses Creative Curriculum and Teaching Strategies. Today Ms. Maultsy, Lead Teacher and Ms. Johnson, Teacher Assistant, were present and interacting with the children as they participated in circle time activities and teacher-directed play. Adequate handwashing was observed prior to the children leaving for lunch. Staff/child ratio, group size, capacity, use of space, license restrictions and supervision were observed. The license and emergency medical care plan were posted. REQUIRED INSPECTIONS/DRILLS: -A sanitation inspection was completed on October 1, 2025, with 9 demerits and a Superior classification. -A fire inspection was completed on September 17, 2025. -The Emergency Drill log was monitored. A fire drill was conducted on February 13,2026 at 9:42 am with 16 children and 2 adults participating in the drill. A bell was used to signal the drill. The last documented lockdown drill was recorded on October 1, 2025. Shelter-in-place/lockdown drills must occur every three months. *This is a violation of child care requirements. -The last outdoor inspection was on February 2, 2026 by Ms. Maultsby. She completed Playground Safety training on October 31, 2019. -The school’s lead water test was on December 3, 2023 and will expire on December 3, 2026. There was one violation of child care requirements observed and documented during today's visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented lockdown drill occurred on October 1, 2025. There was no documentation of a shelter-in-place/lockdown drill occurring every three months. .0604(u);.0302(d)(8) The violation must be corrected immediately. In response to the violation, you must submit a signed compliance letter that explains how the violation was corrected. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history from 8/2/2024 to 2/1/2026 is 98%. During today’s visit, Ms. Maultsby was given the opportunity to ask questions regarding the Outreach Assessment facility summary report scoring. Technical Assistance- 1. Was provided regarding the low score received on Item 6, Indicator 3.1 of the ECERS-3 Outreach Assessment, which addressed the provision of adequate time for gross motor play. During the assessment, it was noted that outdoor gross motor play was scheduled for later in the day; however, it did not occur during the observation period. Discussion included the importance of ensuring that outdoor play is scheduled at a time that will reliably occur during the typical observation timeframe. I reviewed the posted NC Pre-K schedule during today's visit and it was noted that outside time occurred between 12:00 PM-1:05 PM. I encouraged Ms. Maultsby to schedule outdoor gross motor play earlier in the day, when possible, to ensure it occurs during the observation; such as ideally before lunch, as this increases the likelihood that it will occur consistently and during the ECERS-3 observation window. Morning outdoor play also supports improved focus, behavior regulation, and engagement during later instructional periods. Children often have higher energy levels earlier in the day, making this an optimal time for active large-motor experiences. 2. A violation was cited for failure to document a lockdown/shelter-in-place drill at least once every three months as required by regulation. While drills may have been conducted, documentation was not available to verify compliance within the required timeframe. I explained that emergency preparedness requirements include conducting and documenting lockdown or shelter-in-place drills quarterly. Proper documentation not only demonstrates compliance but also supports the safety of children and staff. Ways to comply with this requirement were shared-establish a quarterly drill schedule in advance (e.g., September, December, March), and review documentation monthly to ensure all required drills are completed within the required time-frame. QRIS Pathways to the Stars- On February 20, 2026, I received the following forms for applying for Assessment for a Rated License for Centers: • Completed and signed Application • Family and Community Engagement Standards for Child Care Center with 4 additional options selected • Completed and signed Facility Continuous Quality Improvement (CQI) Plan • Completed and signed Self-Study Verification Form • Environment Rating Scale (ERS) Assessment Request • Completed Staff Information and Education Worksheet I verified the following during today’s visit: • The program is meeting enhanced staff/child ratios • The program is using an approved curriculum-Creative Curriculum/Teaching Strategies The program completed an ECERS-3 Outreach Assessment on 2/6/2026. The ECERS-3 for licensure was submitted upon the completion of today’s visit. I verified the following education status prior to today’s visit: Lead Teacher- O. Maultsy- meets 5-star education standard Other Educators- Administrator- J. Spencer- meets 5-star education standard Teacher- B. Johnson- stated she is currently enrolled in the credential coursework and will finish in May. I informed her that as soon as she can get an Original transcript showing that completion she must send the original transcript to DCDEE WORKS. She also must electronically apply for position within her DCDEE WORKS account. The DCDEE WORKS Return letter, dated 11-6-2025, was left with Ms. Johnson. Upon the completion of the ECERS-3 for licensure and the education evaluation of Ms. Johnson, you will be informed of the star rating obtained. The program operates with a 5-star rated license issued April 4, 20219, earning 7 points for education standards, 7 points for program standards, and one quality point for complying with a programmatic option. License restrictions include 1st shift, meets enhanced ratios and enhanced space and meets reduced ratios minus 1. Today’s visit was conducted with Ms. Joyce Spencer, Principal and NC Pre-K staff. The program currently has one NC Pre-K room on site. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, and outdoor play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A substitute teacher was present with Ms. Maltsby. The teacher assistant, Rose Burgess, arrived during the visit. A sanitation inspection was completed on September 24, 2023. A current inspection has not been completed at this time. A fire inspection was completed on September 9, 2024. During today's visit all staff files were reviewed. The staff and training worksheet was completed on site. During today’s visit, three children records were reviewed. The children’s records form was completed on site. Three violations of child care requirements were observed and documented. Due to printer difficulties, the computer-generated report was emailed to Ms. Spencer during today's visit, along with a request to print the visit summary. The Annual Compliance Monitoring Checklist for Child care Centers was used today to note requirements that were monitored. The program operates with a 5-star rated license issued April 4, 20219, earning 7 points for education standards, 7 points for program standards, and one quality point for complying with a programmatic option. License restrictions include 1st shift, meets enhanced ratios and enhanced space and meets reduced ratios minus 1. Today’s visit was conducted with Ms. Joyce Spencer, Principal and NC Pre-K staff. The program currently has one NC Pre-K room on site. Staff/child ratios, group size, capacity, use of space, and supervision were observed. I observed children playing in various activity centers, teacher lead activities, and outdoor play. Developmentally age-appropriate materials were used. Positive and nurturing communication occurred as teachers interacted with children. A substitute teacher was present with Ms. Maltsby. The teacher assistant, Rose Burgess, arrived during the visit. A sanitation inspection was completed on September 24, 2023. A current inspection has not been completed at this time. A fire inspection was completed on September 9, 2024. During today's visit all staff files were reviewed. The staff and training worksheet was completed on site. During today’s visit, three children records were reviewed. The children’s records form was completed on site. Three violations of child care requirements were observed and documented. Due to printer difficulties, the computer-generated report was emailed to Ms. Spencer during today's visit, along with a request to print the visit summary. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last sanitation inspection was completed on September 25, 2023. A current sanitation inspection has not been completed. 10A NCAC 09 .0304(b) 1043 All staff records, except financial records, were not made available for review. A file for the substitute provider was not made available for review. G.S. 110-91( 9) 1757 A valid qualification letter was not on file and available to review at the facility. The substitute teacher did not have a valid qualification letter on file. The qualification letter expired May 15, 2024. Due to computer issues, today's computer-generated report will be emailed to the facility. This program currently operates with a five-star license, issued April 4, 2019, earning seven (7) points in the education component, five (5) points in the program standards component, and one (1) point in Quality. Restrictions are: Daytime care only, meets enhanced ratios minus 1; meets enhanced ratios and enhanced space. The last annual compliance visit was conducted on October 15, 2023. A sanitation inspection was completed on September 25, 2023 with 2 Demerits and a Superior rating. A fire inspection was completed on September 25, 2023. The emergency drill log was reviewed during today's visit. I observed the last documented emergency drill was a lockdown conducted in November 2023. The next drill should have been completed in February 2024. Pre-K staff stated a lockdown drill was conducted on April 22, 2024. This was not documented on the drill log report. This was corrected during today's visit when the drill log report was updated. One NC Pre-K classroom was observed. Fourteen (14) children were present with two teachers. The license was posted, and the restrictions were in compliance. All indoor and outdoor areas were monitored. I observed children in outdoor learning environments and found supervision and staff/child ratios in compliance. The teachers are currently involved with the Ball study available through the electronic @teachingstrategies.com platform. They reported that parents will often send in items suggested to support the current study because they send a parent letter home that gives suggestions to bridge learning from home to school. The teachers were interacting and meeting the developmental needs for each of the children. One violation of child care requirements was observed and documented; however, it was corrected during the visit therefore a compliance letter is not required. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented drill was a lockdown conducted in November 2023. The next drill should have been completed in February 2024. Pre-K staff stated a lockdown drill was conducted on April 22, 2024. This was corrected during the visit when the drill log report was updated. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance with outdoor learning environments was offered. Children were observed participating in the following activities: art/creative play, science/nature, sand/water play, and gross motor play. Staff were engaged in and actively participated with the children. The children had planted sunflower seeds and I observed children discussing the planting process. In addition to planting flowers, I suggest some ands-on gardening with the children that may include fresh fruit and vegetables so the children can see the benefits of enjoyment of healthy eating early in life. The sensory impact of gardening creates a positive understanding of how seeds turn into plants helped by soil, sun, water—and human care. Young children learn essential gardening skills for healthy living: planting, caring, harvesting, and ultimately preparing tasty eating experiences. Designated production gardens in childcare center outdoor learning environments can inspire lifelong healthy eating habits and provide replicable models for the families they serve. Reminders: 1. NC 10A NCAC 41C .1001-1007 requires lead-based paint and asbestos inspections at all licensed childcare facilities in NC. You can learn more at Clean Classrooms for Carolina Kids. Rated License: The Division of Child Development and Early Education is starting the Rated License Assessment back to get programs current with their Rated License. You are in Cohort 1. Therefore, your Prep year began July 1, 2023 and will end June 30, 2024. During this time, you need to make sure all teaching staff and Principal have a current Status letter in WORKS. This means submitting any certificates or an original transcript showing any EDU coursework completed after 2019 is updated in WORKS. If you are wanting the Environment Rating scales completed to earn more points in Program Standards, please take advantage of all the trainings they have on their website: ncrlap.org.
