This program is currently operating under a Temporary License effective May 28, 2025 through November 28, 2025. Applicable childcare requirements were discussed with you, Angela Yankasky, Operator. During today’s visit, I conducted a walkthrough of the premises to monitor compliance in all indoor and outdoor areas approved for child care. During the visit children were observed actively engaged and participating in various free-choice centers, including fine motor activities, books, blocks, and dramatic play. Positive and developmentally appropriate interactions between the operator and children were noted throughout the visit. The temporary license was posted and in compliance with all restrictions noted on the license. The operator’s file was reviewed for compliance. In addition, six (6) children’s files and all required program records were reviewed for compliance. It is your responsibility to regularly review all files to ensure they are complete, accurate, and compliant with applicable requirements. For breakfast the children were served sausage links, scrambled eggs, grits, toast, and milk. Overall enrollment and attendance information were collected. The attached Family Child Care Home Enrollment Worksheet reflects this information. At the conclusion of the visit, the Visit Summary was reviewed with you, and a computerized copy of the Visit Summary was provided for your records. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. There was an aerosol dispenser accessible to children. This was corrected during the visit when it was placed in locked storage. .1719 (a)(7) 716 Electrical outlets not in use were not covered. Two outlets were not covered on a power strip. This was corrected when the operator covered the two outlets. 10A NCAC .1719(a)(27) 2033 Signed discipline statement did not include the required information, as outlined in rule. The date of signature was not included on the discipline statement for two children enrolled. .1727(c )(1-3) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation. This information not on file for one child enrolled. 10A NCAC 09 .1721(a)(3)(G)(H) A total of four (4) violations were documented. Two (2) violations were corrected during the visit. The two (2) violations not corrected during the visit must be corrected immediately. A signed and dated letter of compliance must be submitted to me no later than July 23, 2025, clearly detailing how each violation has been corrected. You may email your compliance letter to me at leisa.benson@dhhs.nc.gov. If you are unable to meet the July 23, 2025, deadline, you must contact me immediately with a proposed timeline for completing the corrections. In some cases, this timeframe may be extended. Failure to submit your compliance letter by the stated deadline or failure to contact me regarding a revised timeline may result in a follow-up visit, and violations may be cited. Important Notice: If your compliance letter states that corrections have been made and it is later determined they were not, this will be considered falsification of information, and an administrative action may be taken against your license. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 85%. Your compliance history was reviewed with you, and you were also given a copy. Rated License Information: In order to maintain or be eligible for subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. If you decide to apply for a star rated license, you will need to submit your application to me by June 2, 2025. Education Standards: I verified in DCDEE WORKS Unit that you meet the educational requirements for a FCCH by HS + 6 ECE hours. If you receive additional semester hours you should update your education information, no later than the fifth month of the temporary license period. Program Standards: You could receive two (2) points in program standards without the FCCERS-3 assessment; however, to receive more than two (2) points in program standards, you will need to have the FCCERS-3 assessment and score at least a 4.0. If you decide you want the FCCERS-3 Assessment, be sure to visit www.ncrlap.org to familiarize yourself with the assessment. I recommend that you contact the Bladen County Partnership for Children to help you prepare for the assessment. You can contact them at https://www.bladensmartstart.org. If you decide you want the assessment conducted you should contact me by July 31, 2025. Quality Bonus Point: While the facility may meet more than one possible quality point, the facility can only earn a maximum of one quality bonus point. Total points and Star Rated License Information: The total points received in both education and program standards and the quality bonus point will determine the star license issued to your FCCH. To receive a three-star license you must earn at least seven points. To receive a four-star license, you must earn at least ten points. To receive a five-star license you must earn at least thirteen points. In order to earn two or more points in Program Standards for the rated license a three-month self- study on the FCCERS-3 must be completed. If you choose to earn at least two points in Program Standards you should begin your three-month self-study no later than August 1, 2025, to ensure that it is completed by November 28, 2025. The FCCERS-3 book may be purchased through various retail outlets or some resource and referral agencies. Also, if you meet the requirements for a 4-5 star rated license and plan to provide care to 4-year old’s you will be required to have a curriculum that has been approved by DCDEE. It is recommended that you routinely inspect outlets to ensure covers are in place and undamaged. Maintaining accurate and current medical information for all children in care is essential for health and safety, legal compliance, and the delivery of high-quality, individualized care. This information ensures that caregivers are prepared to meet each child’s individual needs and respond appropriately in case of medical emergencies. It is recommended that when you receive an application from a parent, you review it carefully to ensure that all sections have been completed accurately and thoroughly. Aerosol dispensers are considered hazardous and must be stored in locked areas to ensure children’s safety. I discussed the allegations with Angela Yankasky, Owner. I completed a walk-through of the home with Ms. Yankasky, I documented children present today and additional child you have been providing care for on an enrollment worksheet, which you signed, verifying the information was accurate. Additional information is needed to determine if you are operating in violation of the law. Violation Number Comment Rule 1706 Three or more children less than 13 years old, who do not reside where the care is provided, receive care on a regular basis of at least once per week for more than four hours but less than 24 hours per day from persons other than their guardians or full-time custodians, or from persons not related to them. Based on information obtained during today's visit, you have been providing care for four children not related to you for more than four hours a day. GS 110-86(2) I reviewed North Carolina General Statute 110-86(2) & (3) which defines child care and child care facility with you. I left a copy of that section of the child care law with you today and asked you to sign the verification of receipt o, f the information. You may not care for more than two children unrelated to you for more than four hours a day. If you operate a child care facility without being licensed, you will be in violation of North Carolina General Statute 110-98 and subject to penalties of North Carolina General Statute 110-103. Providing illegal child care could result in the issuance of a warrant for your arrest and you being convicted of a felony punishable by a fine, imprisonment, or both.
