The NC Secretary of State website was reviewed on 4/13/26 and Kids-N-lay Center of North Carolina, LLC was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and confirmed current. It is important to frequently check your email to remain informed about any new changes that have or may occur. C. Hill, Administrator, accompanied me during the walk through of all indoor and outdoor spaces. Children were engaged in outdoor free play when we arrived. Four (4) students from Northeastern High School were also in the building completing observation for a school assignment. Lunch included broccoli and cheese, corn dogs, diced pears, a roll and milk. The fenced outdoor space located at the side of the facility include both anchored and portable play equipment as well as a shaded play space. Staff, children, and program records were monitored. Inspections - The sanitation inspection was completed on 3/19/26 with a “Superior” classification. The last fire inspection was conducted on 11/6/25 and your facility was approved for daytime care only. The building inspection was completed on 3/27/26. Zoning Requirements – You indicated the zoning permit is scheduled to be received this week. Lead and Asbestos Results - Lead in water results were completed on 3/13/26 w/ less than 10ppb indicated. Lead -based paint and asbestos are awaiting an on-site visit. You indicated the assessors came to the facility on 3/26/26. You also indicated you contacted Clean Classrooms for Carolina Kids and were informed this process could take up to 3 weeks. Please be advised that an administrative action may be recommended. As discussed throughout the 6-month temporary time period, a star-rated license cannot be issued without all results lead and asbestos results. Criminal Background Check - Criminal background checks (CBC) are completed to protect children from abuse and reduce liability risks. You must use the staff file checklist and pay close attention to the due dates of all items to avoid non-compliance with this. Activity Plans – Lesson plans lay the groundwork for a what and how children learn. They act as roadmaps, guiding learners through the various stages of academic growth. An activity plan was not posted in Space 115. You did not indicate why. You must complete weekly activity plans (dated) that include developmentally appropriate activities for enrolled children. This violation was documented during the January 8, 2026 visit. You indicated in your correction letter that a daily walk through would occur to ensure activity plans are posted. When you submit your correction letter, it is important to implement a procedure that will effectively ensure compliance is met and maintained. Emergency Medical Care Plan: Your program’s Emergency Medical Care plan was created to assist you with following a plan in the event of a medical emergency and who is responsible for each task list. You must update the plan whenever changes occur in your program. Your facility’s emergency medical care plan must be reviewed with staff annually or when changes occur. The emergency medical care plan was not updated in Space 115 to reflect current staff members. You stated the EMC plan was updated in all other spaces but was somehow overlooked in the space. Posting a reminder sign on a bright colored sheet of paper near the closet, is one measure to serve as a reminder. They act as roadmaps, guiding young learners through the various stages of early academic growth. During the walkthrough, I observed spaces with a lesson plan that were not dated. They must reflect 3 activities in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). Menu - Planning menus in advance helps to ensure that food will be on hand. Posting menus in a prominent area and distributing them to parents/guardians helps to inform parents/guardians about proper nutrition. Parents/guardians need to be informed about food served in the facility to know how to complement it with the food they serve at home. If a child has difficulty with any food served at the facility, parents/guardians can address this issue with appropriate staff members. The posted menu at the entrance of the lobby indicated cheerios, apple slices and milk were to be served for breakfast. Only cheerios and milk were served. The individual serving was flustered and forgot the apple slices. In child care programs, breakfast must include milk, a vegetable, fruit or both, and a grain. All three components must be served. Plastic Bags – Plastic bags have been recognized for many years as a suffocation hazard. The U.S. Consumer Product Safety Commission (CPSC) has received average annual reports of twenty-five deaths per year to children due to suffocation from plastic bags. In child care facilities, plastic bags must be made inaccessible to children less than three years of age. In Space 106 (designated for children one-two years-of-age) a plastic grocery store bag used to store a child’s blanket was observed inside the child’s cubby. When made aware, the teacher removed the blanket from the bag and indicated the parent had just dropped the child off. It is imperative that children’s belongings are checked for hazards prior to placing them inside the child’s cubby. Nutrition - Children learn through simply watching other people which means that our own behavior can easily be picked up and copied by children. In relation to eating, this means that children are likely to imitate the eating behaviors of people they spend time with and see around them. This means that role models can play a really important part in shaping children's eating habits. To be good role models for children food and beverages must meet the nutritional requirements as outlined in the Meal Pattern for Children in Child Care Programs when they are in the presence of children. A Bojangles cup was stored on the window seal in Space 101 and two Bojangles cups and a Bojangles food bag was stored on top of the cubby in Space 106. We reviewed the requirement of only consuming nutritional beverages and food in the presence of children. Rated License Assessment - Discussions occurred today regarding the Pathways to the Stars. We also reviewed Education Standards for each star level. By February 9, 2026, you must determine which Pathway you will pursue. WORKS - WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their workforce information to support education requirements. The WORKS portal is available online and requires the user to create and maintain an individual North Carolina Identity Management (NCID) username and password. Electronic official transcripts can be submitted to the Workforce Education Unit from a person’s college or university provided the school participates with the National Clearing House. If not, then official transcripts must be mailed to the Workforce Education Unit at DCDEE for evaluation. All staff education should be current within four months of the six month temporary time period (March 2026). This program currently operates with a Temporary License issued November 3, 2025. The temporary license is valid through May 3, 2026. This facility is owned and operated by Kids-N-Play Center of North Carolina, LLC. The NC Secretary of State website was reviewed on 1/7/26 and Kids-N-lay Center of North Carolina, LLC was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and confirmed current. It is important to frequently check your email to remain informed about any new changes that have or may occur. A sanitation, fire and building inspection must be completed and submitted within the 6-month Temporary Time Period. Additionally, lead in water, lead-based paint and asbestos testing must be completed. I am requesting the completion of these requirements on or before March 15, 2026. There are twelve (12) approved indoor spaces and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in all classrooms. All staff members were actively engaged with the children. Infants were observed playing on the floor with soft toys. The outdoor space had a variety of stationary and portable materials. Breakfast was observed during today’s visit and included cheerios and milk. The following violations were documented. We reviewed that all facilities must maintain at least a 75% compliance history score over an eighteen-month time frame to be in good standing with DCDEE. If a facility’s compliance history falls below 75% over an eighteen-month time frame, some type of administrative action may be issued. The violation(s) documented must be corrected immediately. On or before 1/22/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The classroom staff did not indicate why the door was unlocked. Prevention is the key to prevent poisoning by making sure all items required to be kept in locked storage are indeed locked up.
