K. Picart, Operator, was present. This facility was issued a temporary license effective November 20, 2025 – May 20, 2026, providing first and second shift care. The facility was assessed for supervision, staff/child ratio, and health and safety requirements. Children’s records, operator records and program records were monitored. Jennifer Bradshaw-Garrett, Child Care Consultant, accompanied me during the visit. Corporation Status: Per your statement and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact your child care consultant prior to any changes regarding the ownership of this facility. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and the information is current. It is important to frequently check your email to remain informed about any new changes that have or may occur. During this visit, we walked through the family child care home observing the indoor and outdoor environments. The operators’ two (2) preschool-age children were present. The children were building a farm using wood blocks as indicated on the activity plan. The operator asked the children, “if you had a farm, what animals would you have?” The children answered and discussed a time when they visited a petting zoo and some of the animals they saw. A farm theme related book was read; “Old McDonald had a Farm and BINGO was sung. The children completed handwashing and transitioned to their low-lying table near the kitchen. Ingredients and items were gathered to make dirt cups for an Earth Day activity. As the children were completing the activity, the operator told the children that taking care of the earth is everyone’s responsibility. Lunch included turkey and cheese sandwiches, green beans, apple sauce and milk. Compliance History - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The compliance history for this facility prior to today’s visit is 97%. The violation(s) documented may impact the compliance history score and must be corrected immediately. The following violation was documented and must be corrected immediately. On or before 5/7/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violation was corrected. Deep, puncture wounds from rusty metal (like nails or playground equipment) can lead to serious, life-threatening infection if a child is not vaccinated, causing severe muscle spasms. These items must be removed from the fenced area until they can be repaired. Conducting a daily inspection will help to identify safety concerns and remove them. Rated License – You submitted the Rated License Application for Assessment for a Rated License. You have chosen to meet the requirements for licensure in Pathway 2- Classroom and Instruction Quality. You and I reviewed the application and all supporting documentation. You are implementing the Gee Whiz curriculum. You are applying for a 2-Star Rated License to allow time for you to complete some coursework. We reviewed your WORKS evaluation letter and you meet the 2-Star Education Level. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Maintaining complete and accurate records for the program and children is a vital part of being an operator. During today’s observation of children’s records, neither of the operator’s own preschool age children had an application for enrollment on file. Additionally, the children did have documentation of the receipt of shaken baby and abusive head trauma policy, summary of childcare law, or the discipline policy. One child’s file did not have a medical report on file. You indicated you had an enrollment application on file for one and took it. You also stated you thought you had an enrollment application in for the other child. Setting aside regularly scheduled times each week to review records can assist you in keeping up with the task. A tracking and reminder systems are also essential to keep track of tasks, due dates, and expiration dates for both staff and children. Per our discussion, you must set aside time to review files to ensure forms are complete. Checklists for children’s files can be found under provider documents on the Division’s website. Placing a copy in each file will help you to see that you have all the required information on file. Being proactive with an organized approach to record-keeping helps you ensure easy access to information when needed, especially in emergency situations. Reminders: Unannounced Visits - I will conduct at least one additional temporary time period visit. Ensure you document updates on your verification of required information for operator form for future monitoring. We reviewed the 3-Star Education Standards for Family Child Care Home operators. You indicated you do not meet any of the options in the 3-Star Leve. You also indicated you will begin two (2) eight (8) week EDU courses on March 11, 2026, totaling 6 credit hours. The courses will end on May 12, 2026.
