Jennifer Bradshaw Garrett, Child Care Consultant accompanied me on today's visit. K. Senn, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance, issued October 1, 1998. The last annual compliance visit was conducted on March 12, 2025. The sanitation inspection was completed June 27, 2025, with a “Superior” classification. The last fire inspection was conducted on December 9, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two percent as of February 12, 2026. Church, Emmanuel Baptist is listed as the current owner of this program. Prior to making any changes to the ownership statues, contact me to discuss change of ownership procedures to ensure requirements are met. You visited the indoor and outdoor spaces used by children. Children were engaged in free choice play using age-appropriate materials and toys. Children completed morning routine reviews which included hand washing and toileting. Proper hand-washing techniques were observed. Lunch was observed. Lunch consisted of meals prepared and brough from home. Nutrition opt out forms were on file for children. The following violations were documented: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Departure times were not documented for ten or more children between the dates of January 5, 2026, through February 10, 2026. 10A NCAC 09 .0302(d)(4) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence). An infant feeding plan was not on file for a fourteen month old child enrolled. 10A NCAC 09 .0902(a) 807 A safe indoor and outdoor environment was not provided for the children. The barrier enclosing the equipment on the preschool playground was bent creating sharp edges in space accessible to children. A compartment attached to a playhouse on the toddler playground was cracked exposing sharp edges. 10A NCAC 09 .0601(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 26, 2026,, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. TECHNICAL ASSISTANCE & GUIDANCE: CHILDREN & STAFF FILE CHECKLIST: The updated children file checklist was not being used to manage children records. I advised you to utilize the most recent version of the children file checklist to ensure all required documentation is retrieved and placed on file. A copy of the checklist was given to you during the visit., SAFE INDOOR/OUTDOOR ENVIRONMENT: We discussed the potentially hazardous areas identified on the playgrounds. You stated the cracked items would be fixed immediately as well as removal of the foundation built up around the bottom of the fence that has decreased height. I recommended periodically checking the height of the fence as part of the outdoor plays inspections to ensure height requirements are maintained. Also, when found broken and damaged materials must be removed to avoid potential injuries and prevent children’s access. Be reminded electrical outlets in surge protectors should be cover. You inserted safety plugs in the outlets uncovered during today’s visit. ARRIVAL/DEPARTURE TIMES: Departure times were not documented for ten or more children between the dates of January 5, 2026, through February 10, 2026. You reported reminders have been given to families about the importance of signing children out. We discussed having staff to document time of departures when parents forget to do so and add their initials beside it reflecting who recorded the information. RECOGNIZING & RESPONDING TO SUSPICIONS OF CHILD MALTREAATMENT: The recognizing and responding to suspicions of child maltreatment training certificate for one employee hired on September 3, 2025, was dated July 20, 2023, which exceeds the twelve months. I informed you that previous recognizing and responding to suspicions of child maltreatment training can be transitioned; however, it must be within twelve months prior to the employment date. You stated you would ask the employee to complete the training as required to meet requirements. ABCMS: The program's roster was not included in the ANBCMS system. I verified completion of the training from the certificate on file. When asked, you stated a code was not provided when the training was completed. I advised you to contact the Division's Criminal Record Check unit to request the code. Once the code is retrieved, it should be given to the staff to input in their criminal background check portal. After the code has been included in staff portals, you must return and add their hire dates. This will link the staff to your program. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. The barrier enclosing the equipment on the preschool playground was bent creating sharp edges in space accessible to children. TECHNICAL ASSISTANCE & GUIDANCE: CHILDREN & STAFF FILE CHECKLIST: The updated children file checklist was not being used to manage children records. I advised you to utilize the most recent version of the children file checklist to ensure all required documentation is retrieved and placed on file. A copy of the checklist was given to you during the visit., SAFE INDOOR/OUTDOOR ENVIRONMENT: We discussed the potentially hazardous areas identified on the playgrounds. You stated the cracked items would be fixed immediately as well as removal of the foundation built up around the bottom of the fence that has decreased height. I recommended periodically checking the height of the fence as part of the outdoor plays inspections to ensure height requirements are maintained. Also, when found broken and damaged materials must be removed to avoid potential injuries and prevent children’s access. Be reminded electrical outlets in surge protectors should be cover. You inserted safety plugs in the outlets uncovered during today’s visit. ARRIVAL/DEPARTURE TIMES: Departure times were not documented for ten or more children between the dates of January 5, 2026, through February 10, 2026. You reported reminders have been given to families about the importance of signing children out. We discussed having staff to document time of departures when parents forget to do so and add their initials beside it reflecting who recorded the information. RECOGNIZING & RESPONDING TO SUSPICIONS OF CHILD MALTREAATMENT: The recognizing and responding to suspicions of child maltreatment training certificate for one employee hired on September 3, 2025, was dated July 20, 2023, which exceeds the twelve months. I informed you that previous recognizing and responding to suspicions of child maltreatment training can be transitioned; however, it must be within twelve months prior to the employment date. You stated you would ask the employee to complete the training as required to meet requirements. ABCMS: The program's roster was not included in the ANBCMS system. I verified completion of the training from the certificate on file. When asked, you stated a code was not provided when the training was completed. I advised you to contact the Division's Criminal Record Check unit to request the code. Once the code is retrieved, it should be given to the staff to input in their criminal background check portal. After the code has been included in staff portals, you must return and add their hire dates. This will link the staff to your program. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. Once children have control of their head and upper body, they must begin washing their hands at the sink with soap and warm water. Hands are required to be washed upon arrival to the facility, before meals/snacks, after toileting/diapering, after messy play, when children return from outside play, and before they leave the facility to go home. In space 2, the children one (1) year of age, did not wash their hands in the sink after diapering. Hand wipes can only be used when children are outside, and soap and water are not available. At all other times, children’s hands must be washed with warm running water and soap. Placing a good, sturdy step or stool in front of the sink will make it easier for the children to use the sink. Make sure the step or stool is wide enough that, if they start dancing as they sing their hand washing song, the structure will not tip and cause them to fall. You stated that you have a step that they can use to help the children use the sink. Additional paper towels must also be added to the space, so staff do not have to go to the changing table to get what they need. Consultation: Sharing vs Taking Turns – Children do not always understand the terms we use to guide their behavior. They must be taught what the expectations are. Demonstrating what you want them to do helps them to learn good behaviors. Instead of asking children to “share” consider demonstrating to them how to “take turns”. If a child has a toy that another child wants, tell them to wait their turn and explain that when the child puts the toy down and chooses anther toy, then it is your turn. This gives the child a visual cue as to when they can then take the toy to play with. Another example would be “we use nice hands”. What does “nice hands” mean to a young child? You can use other terms such as “gentle” and then demonstrate what that feels like by gently touching their arm or hand. This helps children understand the terms you are using and also helps them change their behavior. This included tracking permission form dates, medication expiration dates, and ensuring that out of date medications are returned to the parent within 72 hours or disposed of. In space 3, the permission form for Diazepam expired on 1/16/25. You stated that you had instructed staff to review the forms monthly. Medication violations have been cited in past visits and continue to be a concern. To prevent further violations and ensure the safety of the children, a tracking plan must be put into place to ensure medications, permission forms, and action plans are reviewed regularly and updated as needed. Documentation of the plan must be submitted as part of your compliance verification letter. You stated that you will work to create something more hands-on to track this information. Recognizing and Responding to Suspicions of Child Maltreatment – The training helps to ensure all staff can identify potential maltreatment and know what steps to take to ensure the safety of the children. The training must be completed within three (3) months of hire. Staff can complete the required Recognizing and Responding to Suspicions of Child Maltreatment at www.preventchildabusenc.org. Once staff have completed the Recognizing and Responding to Suspicions of Child Maltreatment on the Prevent Child Abuse NC website, any approved training related to Child Maltreatment can be used to meet this topic area for the five-year cycle. One staff member, hired on 8/15/24, did not complete the training until 1/15/25. Consider making completion of the training part of your orientation process. This helps to ensure the staff member is well trained before serving children. Children's Records – Each form or document required to be reviewed with parents and present in the child’ file helps to ensure parents understand how your facility operates and what is required per NC Child Care Rules and laws. One (1) child’s file, out of five (5) files reviewed, did not have documentation that the Discipline Policy had been reviewed with the parent. Two (2) children’s files, out of five files reviewed, did not have documentation that the parent received a copy of the Summary of Child Care Law. You stated that you would review all file to ensure they have the required information. Additional Information: NCID Password Change – In the Raise NC email blast sent out on 2/21/25, announced changes to the password rules. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from 8 to 14 characters. The change will apply to all providers. NCID passwords must be updated on a regular basis to keep them active. The next time you update your current password; you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deepsleep). Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The sanitation inspection was completed 9/11/24. with a “Superior” classification. The last fire inspection was conducted 12/16/24. and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 93% as of 3/10/25. Facility Information: We reviewed your facility information and per your statements, the information is current and accurate. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time and free play. The children continue to bring all meals and snacks from home. Nutrition Opt-out forms were on file. There continues to be two (2) fenced playground spaces with age-appropriate materials such as play houses, portable climbing structures, trucks, and sand toys. Once staff have completed the Recognizing and Responding to Suspicions of Child Maltreatment on the Prevent Child Abuse NC website, any approved training related to Child Maltreatment can be used to meet this topic area for the five-year cycle. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from 8 to 14 characters. The change will apply to all providers. NCID passwords must be updated on a regular basis to keep them active. The next time you update your current password; you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deepsleep). As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from 8 to 14 characters. The change will apply to all providers. NCID passwords must be updated on a regular basis to keep them active. You will need to work with parents and staff to ensure that, when clothing is brought to the facility, they remove the clothing from the plastic bag to place it in the child’s cubie. They must dispose of or store the plastic bag five (5) feet above the floor or in locked storage. This was corrected during the visit. Reminders: Medication Administration – Technical assistance was given on tracking medications in the facility. This includes tracking permission form dates, medication expiration dates, and ensuring that out of date medications are returned to the parent within 72 hours or disposed of. We reviewed the need to cross check that the medication, the pharmacy box, and all paperwork match so that the right medication is being administered to the right child. Remember, if the date of expiration on the pharmacy label is earlier than the date on the medication, you always go by the date on the pharmacy box. You stated that providers review their medication every three (3) months but you do not provide them with a checklist to use during the review. You stated that using the information shared today, you will create a checklist. Emergency Preparedness and Response Plan – Emergency situations can occur at any time. The event could include loss of internet and loss of phone connectivity. This is why having a hard copy of a map of the area is so important. The maps in the current EPR plan did not distinguish local road systems that could be used to evacuate if needed. In the event of an emergency things like traffic jams or washed-out roads could prevent you from using the usual route you would take to safety. Having a hard copy of a map can help you maneuver around these obstacles and reach a safe place. Each form must be carefully examined to ensure all the required information is present before accepting the medication into your facility. If there is not a statement giving permission to administer the medication included on the medical action plan, you must have the parent complete a separate Permission to Administer Medication form. You stated that you thought the parent had signed the form and will have them complete a permission form when they pick up the child at the end of the day. These incidents must be recorded on the incident report form supplied by the Division. The report must include all the required information. An incident report, completed on 3/19/24 for AH, did not have documentation of the date and time the parent was contacted and did not include the name of the provider who contacted the parent. You must review the form with staff to ensure they understand the importance of completing all the information on the form. This information provides transparency with the parents and helps to protect the staff. Provider Access to ABCMS - Beginning February 1, child care administrators can access the Automated Background Check Management System (ABCMS) This access will allow providers to: •See the real-time background check status of staff members. •Run a printable report of the staff roster to assist with compliance visits. •See new background check applicants and add them to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. All staff must connect their CBC with your license number and the date this was completed listed on your Staff and Training Worksheet. Required Water, Lead Paint, and Asbestos Testing - New child care rules require child care facilities to test for lead in drinking water, lead paint, and asbestos. Testing is required by 1/1/2025. Check out the FAQ in the updated Clean Classrooms for Carolina Kids. Currently there is funding to pay for the testing and to mitigate lead paint or asbestos hazards. Depending on the age of your home or building and the information you provide in the enrollment process, testing for lead paint and asbestos may not be required. K. Senn, Administrator, assisted me with today’s visit. The last annual compliance visit was conducted 5/11/23. The sanitation inspection was completed 4/19/23 with a “Superior” classification. The last fire inspection was conducted 12/7/22 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was 88% as of 10/25/23. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children from spaces 1, 3, and 4 were having lunch in the social hall of the church. All the children had brought their lunches from home. Each item in their lunch bags was labeled with their name and today’s date. After lunch, the children toileted and washed their hands in preparation for nap. The children in space 2 were already on their cots. Each child had a linen covered cot placed 18 inches apart from the other children. There continues to be fenced playground areas located behind the church. The materials included a stationary slide located over sufficient mulch surfacing, portable climbing structures, play kitchens, and sand toys. The first aid kit located in space 4, contained packets of Triple Antibiotic Ointment and Antiseptic Wipes. 15A NCAC 18A .2820(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the bathroom used by children 2 years of age, there was a plastic trash bag filled with children’s shoes located in the cabinet under the sink. In the teacher’s desk drawer in space 1, there was a plastic zip lock bag with card board shapes. Written permission must be obtained from the parent before applying any medication to a child. Per our discussion, when purchasing a new first aid kit, make sure staff go through the contents and remove all medications. Plastic Bags - Plastic bags have been recognized for many years as a suffocation hazard. In child care facilities, plastic bags must be made inaccessible to children less than three years of age. In the bathroom used by children 2 years of age, there was a plastic trash bag with children’s shoes located in the cabinet under the sink. Additional Information: Child Care Immunization Report 2023-2024 - This year's Child Care Immunization Reports are due by midnight on Wednesday, November 1, 2023, and will not be accepted after that date. Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county. You must submit a report even if no children are attending your child care center or your child care center has closed. This will ensure your center meets the reporting requirement and is not considered delinquent.
