Upon arrival A. L. Whidbee, teacher assistant, assisted with the visit. C. Jones, Administrator, arrived shortly after and assisted. This center currently operates with a two-star license, earning two points in education standards, seven points in program standards and one quality point for having reduced staff/child ratios by one in each group from the seven-point level as required in .2818(c). The last annual compliance visit was completed on November 7, 2024. The last sanitation inspection was completed on May 6, 2025, earning a “Superior” classification. The last fire inspection was completed on September 2, 2025, and the center was approved for day and nighttime care. Twenty-six (26) children ranging from ages three (3) to four (4) years old were observed in care today. Children were engaged in morning routine/activities, free-choice play, outdoor play, completing hand washing and toileting routines, eating breakfast, and interacting with teachers. The following violation was documented today. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Departure times were not recorded for twelve children enrolled between the dates of September 4, 2025, through October 2, 2025. The arrival time was not recorded for one child on October 3, 2025. 10A NCAC 09 .0302(d)(4) The violations documented must be corrected immediately. SAFE ARRIVAL/DEPARTURE RECORDS: Based on my review of arrival and departure records for the months of September and October 2025, departure times were not recorded for twelve children between the dates of September 4, 2025, through October 3, 2025. Additionally, one arrival time was not recorded for one child on October 3, 2025. Please be reminded that arrival and departure times must be recorded when each child arrives and leaves for the day. It was recommended having staff periodically review the arrival/departure logs to ensure parents are signing children in and out. FIRE INSPECTION REPORT: A copy of the updated fire inspection report was given to me during today's visit. ANNUAL IMMUNIZATION REPORT: The annual immunization report will be due soon. The administrator from Chowan Head Start was working as a substitute in a classroom and assisted me with the visit. Your program currently operates with a two-star license, issued 4/7/20 earning 1 points in the education component, 3 points in the program standards component (meeting enhanced ratios minus 1 and enhanced space) and 1 quality point for: Has reduced staff/child ratios by one in each group from the seven point level as required in .2818(c). Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. North Carolina Rated License Assessment Project - NCRLAP’s mission is to promote the quality of child care by consistently and reliably assessing environments for the North Carolina Star Rated License. They collaborate with professionals in the early childhood education field to foster the development and learning of young children. Their website, ncrlap.org, offers resources, trainings, videos, worksheets, and help to get ready for the new ITERS-3, ECERS-3, and FCCRS-3 assessment tools that will be used in the new QRIS system. The center's compliance history was reviewed with the operator. The program’s compliance history was 92 % as of 5/5/25. Corporation Status: This facility is currently owned by the Economic Improvement Council, Inc. According to the NC Secretary of State's website, this corporation is current and active. Please contact me prior to any information changing regarding the ownership of this facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The children sang songs in the hallway as they waited their turn to toilet and wash hands before going outside. There continues to be a fenced playground behind the building. The children had access to stationary climbing structures, tricycles, and dramatic play areas. The caregivers were interacting and meeting the developmental needs for each of the children. Lunch was observed and consisted of chicken nuggets, carrots, peaches, roll, and milk. The Albemarle Alliance for Children and Families / Child Care Resource & Referral is able to provide you with resources and information, training opportunities, and technical assistance on child care issues and the Environment Rating Scales. The following violation(s) were documented: Violation Number Comment Rule 1008 Administrator did not have NC Early Childhood Administration Credential or equivalent or was not enrolled as specified in GS 110-91(8). The person listed as an administrator, effective 8/26/24, had not submitted information to WORKS for evaluation and no documentation was on file to verify qualification as an administrator. GS 110-91(8); .0704(a) Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. In November 2024, a request was received to add CJ as the administrator with a start date of 8/26/24 and the person was instructed to submit their education to WORKS for evaluation. This requirement has not been completed, and no documentation was on file to verify the qualifications for an administrator. The facility is required to have a qualified administrator on site to oversee the program. If the person does not meet the requirements, they may share the position with another individual who is qualified as an administrator until the requirements can be met. Reminders: Orientation – Documentation on file for one (1) provider stated that some items on the required orientation form were completed in Moodle. Orientation should be tailored to the specific building/facility where the provider will be working. This ensures that new staff know the organization’s specific policies and procedures. They should also be trained in information that is specific to each building. This would include the location of the Emergency Preparedness and Response Plan, individuals that are first responders listed on the Emergency Medical Care Plan, or how medications are stored and administered at the specific location. Each orientation should allow time on site for the person to become familiar with the space, the location of evacuations for fire drills, and other information they will need in the event of an emergency. If staff are moved to a different location, they must be trained for each facility to ensure the safety of the children. Additional Information: NCID Password Change – In the Raise NC email blast sent out on 2/21/25, announced changes to the password rules. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from 8 to 14 characters. The change will apply to all providers. NCID passwords must be updated on a regular basis to keep them active. The next time you update your current password; you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deepsleep). Strong, unique passwords, along with practicing mindful cybersecurity practices, are key components to the Division protecting your devices and personal information Required Water, Lead Paint, and Asbestos Testing - New child care rules require child care facilities to test for lead in drinking water, lead paint, and asbestos. Testing is required by 1/1/2025. Check out the FAQ in the updated Clean Classrooms for Carolina Kids. Currently there is funding to pay for the testing and to mitigate lead paint or asbestos hazards. Depending on the age of your home or building and the information you provide in the enrollment process, testing for lead paint and asbestos may not be required. The administrator from Chowan Head Start was working as a substitute in a classroom and assisted me with the visit. Your program currently operates with a two-star license, issued 4/7/20 earning 1 points in the education component, 3 points in the program standards component (meeting enhanced ratios minus 1 and enhanced space) and 1 quality point for: Has reduced staff/child ratios by one in each group from the seven point level as required in .2818(c). Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. North Carolina Rated License Assessment Project - NCRLAP’s mission is to promote the quality of child care by consistently and reliably assessing environments for the North Carolina Star Rated License. They collaborate with professionals in the early childhood education field to foster the development and learning of young children. The center's compliance history was reviewed with the operator. The program’s compliance history was 92 % as of 5/5/25. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. The children sang songs in the hallway as they waited their turn to toilet and wash hands before going outside. There continues to be a fenced playground behind the building. The children had access to stationary climbing structures, tricycles, and dramatic play areas. The caregivers were interacting and meeting the developmental needs for each of the children. Lunch was observed and consisted of chicken nuggets, carrots, peaches, roll, and milk. This would include the location of the Emergency Preparedness and Response Plan, individuals that are first responders listed on the Emergency Medical Care Plan, or how medications are stored and administered at the specific location. Each orientation should allow time on site for the person to become familiar with the space, the location of evacuations for fire drills, and other information they will need in the event of an emergency. If staff are moved to a different location, they must be trained for each facility to ensure the safety of the children. Additional Information: NCID Password Change – In the Raise NC email blast sent out on 2/21/25, announced changes to the password rules. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from 8 to 14 characters. The change will apply to all providers. NCID passwords must be updated on a regular basis to keep them active. The next time you update your current password; you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deepsleep). Warmth and affection can be expressed in so many ways including smiles, laughter, voice tone, words of endearment, encouragement, and many types of appropriate physical contact. Staff should always use a pleasant calm voice, simple language and positive responses when speaking with children. Per our discussion, the Behavior Specialist at Albemarle Alliance for Children and Families can provide technical assistance to help manage these difficult behaviors. They can be contacted at 252-333-1233. Another resource would be the Healthy Social Behaviors (HSB) Project. This project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. The team is made up of specialists with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. You also stated that you will be the teacher in space 1 moving forward. Fire and Sanitation Inspections – Fire and sanitation inspections protect children from hazards in the building and provides a safe place for the children to learn and grow. Ensure both Sanitation and Fire Inspections are conducted at least annually. A courtesy call should be conducted to the Inspectors at least two (2) months before the next inspections are due to ensure they are completed within the required time frame. You are a new administrator and will need to complete the EPR Training by 12/26/24. Contact Albemarle Alliance for Children and Families at 252-333-1233 for the dates of the next training available. Once completed, you will need to log into the EPR Plan completed by the last administrator, review it and update it, as needed. Make sure you click on “publish” to save your work. Submission of Education to WORKS – You have initiated a WORKS account but have not yet submitted your education for evaluation. You must submit official transcripts to WORKS to verify that you are qualified as an Administrator. Once the transcripts have been sent, you must log into WORKS and request to be evaluated as an Administrator and a Lead Teacher. You can access the WORKS portal on the Division’s website at ncchildcare.dhhs.nc.gov. Under “Providers” you will see a tab for “WORKS”. There will be a training module and helpful information in the blue box on the right for submitting your transcripts. There is also a help number listed if you need further assistance. Your program currently operates with a two-star license, issued 4/7/20 earning 1 points in the education component, 3 points in the program standards component (meeting enhanced minus one ratios and enhanced space) and 1 quality point for: Has reduced staff/child ratios by 1 from the 7 point level. Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. In space 2, the children were making trees with paint on their arms and hands. The children were asked what type of tree they wanted to paint. In space 1, the children sang songs in circle time and learned the sound of the letter G. Lunch was observed and consisted of cheese pizza, broccoli, pineapple tidbits, and milk. Providing positive guidance through nurturing and responsive relationships and supportive environments helps children to feel safe. A child in space 1, who did not want to use the bathroom before going outside, was picked up by their arms and told to put their feet down as the assistant teacher moved the child to the bathroom. When the child came out of the bathroom, the they went under a table. The lead teacher took the child by one (1) arm and pulled the child out from under the table. Children should never be picked up or pulled by their arm as this could result in injury to the joints and/or cause nursemaid elbow. When the children transitioned to the playground, the assistant teacher from space 2 was observed yelling “no”, “stop that”, and “don’t do that” at the children. When thinking about how to respond to a child's behavior, consider the child's development and the behaviors that are typical for children at that stage of development. Warmth and affection can be expressed in so many ways including smiles, laughter, voice tone, words of endearment, encouragement, and many types of appropriate physical contact. Staff should always use a pleasant calm voice, simple language and positive responses when speaking with children. Per our discussion, the Behavior Specialist at Albemarle Alliance for Children and Families can provide technical assistance to help manage these difficult behaviors. They can be contacted at 252-333-1233. Another resource would be the Healthy Social Behaviors (HSB) Project. This project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. The team is made up of specialists with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. To speak to a Behavior Support Advisor, contact 1-888-600-1685 option 1. You stated that you have been working with the staff to help them address the difficult behaviors in their classes this year. You also stated that you will be the teacher in space 1 moving forward. Fire and Sanitation Inspections – Fire and sanitation inspections protect children from hazards in the building and provides a safe place for the children to learn and grow. Ensure both Sanitation and Fire Inspections are conducted at least annually. A courtesy call should be conducted to the Inspectors at least two (2) months before the next inspections are due to ensure they are completed within the required time frame. A fire inspection was due by 8/25/24 but was not completed until 9/4/24. A sanitation inspection was due by 9/28/24 but was not completed until 10/1/24. Playground Inspections: Monthly playground inspections must be conducted by an individual who has completed the Playground Safety Training. Copies of the August 2024 playground inspection documented that the inspection was completed by the Family Advocate. The Family Advocate did not have documentation of Playground Safety Training on file. A trained administrator or staff person must make a record of each inspection using a playground inspection checklist. The checklist must be signed by the person who conducts the inspection and must be maintained for 12 months in the center’s files for review by a representative of the Division. You stated that you are trained and will complete the inspections moving forward. Per our conversation, consider having all staff complete the training. This can count towards their on-going training hours and help ensure there is always a qualified person to complete the inspection. New Administrator Information: Emergency Preparedness and Response Training - Each facility is required to have at least one person on-site who has completed the full Emergency Preparedness and Response training. You are a new administrator and will need to complete the EPR Training by 12/26/24. Contact Albemarle Alliance for Children and Families at 252-333-1233 for the dates of the next training available. Once completed, you will need to log into the EPR Plan completed by the last administrator, review it and update it, as needed. Make sure you click on “publish” to save your work. Submission of Education to WORKS – You have initiated a WORKS account but have not yet submitted your education for evaluation. You must submit official transcripts to WORKS to verify that you are qualified as an Administrator. Once the transcripts have been sent, you must log into WORKS and request to be evaluated as an Administrator and a Lead Teacher. Using a video, danced and then practiced counting various items on cards. The provider explained what each item was, such as a light bulb or a lime, and asked the children questions about them. Later, the children went outside. There continues to be a fenced playground with a climbing structure over an approved surface. The children also played with tricycles, portable play houses, balls, and blocks. Lunch was observed and consisted of hamburger on a bun, peas, sweet potato puffs, and milk. You stated that the first aid kit had just been given to you and you had placed it in the backpack without looking at the contents. You locked the kit in a file cabinet until you can properly dispose of the medications. Hand Washing – Washing hands is one of the best ways to prevent the spread of disease and keep children and staff healthy. Hands are required to be washed upon arrival to the facility, before and after meals/snacks, after toileting, after messy play, when children return from outside play, and before they leave the facility to go home. In space 1, the children washed their hands when they returned inside and were then instructed to sit on the rug in the center time space. The provider then began setting out the lunches and calling individual children to the table. Children’s hands were not washed again after touching the rug and being seated to eat. The children also did not wash their hands after lunch and before resting on their cots. Per our discussion, if the children go to the center time space after washing hands, a staff member must be with then to do stand-up activities that keep their hands from touching anything before eating. You could also consider calling them from the center, as you set out the food, have them wash their hands and then be seated for lunch. When they finish, they must wash their hands again. Consider having them wash hands and then return to the center time space for a story or activity while the other provider puts cots down. Incident Reports and Log - You must now document every injury incurred while a child is in your care, including cuts, scrapes, bumps, and bitemarks. These incidents must be recorded on the incident report form supplied by the Division. The report must include all the required information and be signed by the parent. The incident must then be logged on to the facility incident report log and the completed report placed in the child’s file. An incident report, completed for MV, was not placed in the child’s file.
