Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. I monitored three (3) staff files during the visit. Your staff and training worksheets are due with your compliance letter. The sanitation inspection was completed on 12/15/25 with a “Superior” classification. The last fire inspection was conducted 4/21/26 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of 5/4/26. The NC Secretary of State website was reviewed on 5/4/26 and Alannah’s Playhouse LLC was listed as Admin. Dissolved. I observed children in the indoor learning environment. Children were participating in group time, indoor and outdoor play, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. The caregivers were interacting and meeting the developmental needs for each of the children. A selection of staff and children’s files were reviewed. Program records, including fire drills and monthly outdoor inspections were completed and current. I observed children in the indoor learning environment. Children were participating in group time, indoor and outdoor play, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. The caregivers were interacting and meeting the developmental needs for each of the children. A selection of staff and children’s files were reviewed. Program records, including fire drills and monthly outdoor inspections were completed and current. The NC Secretary of State website was reviewed on September 3, 2025 and Alannah’s Playhouse, LLC was listed as current- active. A staff member hired on 8/22/25 did not have a test result indicating the individual is free of active tuberculosis on file. You indicated that it was the first thing you received from the individual prior to hire and were unsure of why it was not in the file. The individual left the facility to retrieve a copy from the medical facility where the TB skin test was completed. The negative TB results indicated the skin test was read on 8/8/25. We discussed using the current Staff file checklist to ensure that all documents are received within required timeframes. ITS-SIDS – Early childhood teachers touch the lives of young children and their families in many important ways. One of the most important ways to reduce infant deaths in child care settings is for teachers to implement (and model for parents/guardians and families) safe sleep practices in their programs. Safe sleep practices include placing infants on their backs to sleep, using safe cribs and equipment for sleeping, prohibiting smoking, and training teachers on appropriate safe sleep practices. Assigned staff who work alone in the infant space must have a current ITS-SIDS training certificate. The individual working in space 1 designated for the care of infants and one year old children does not have a current ITS-SIDS certificate on file. You indicated the individual has not completed the training. You added a staff person with ITS-SIDS training to the space. Plastic Bags – Plastic bags have been recognized for many years as a suffocation hazard. The U.S. Consumer Product Safety Commission (CPSC) has received average annual reports of twenty-five deaths per year to children due to suffocation from plastic bags. In child care facilities, plastic bags must be made inaccessible to children less than three years of age. In Space 3 (designated for children two-three years-of-age) a plastic grocery bag used to store a child’s change of clothing was stored in a cubby. When brought to your attention, you removed the plastic bag. If the teachers are going to use the plastic bags, then these bags must be stored out of reach of the children, which is defined as five feet or higher from the finished floor. Electrical Outlets – Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or a tamper-resistant electrical outlet. An open electrical outlet was observed near the window in space 3. Taping a reminder on the wall over each socket and power stick can serve as a reminder to add the safety cover when not in use or have the wall plate on the socket replaced with a tamper resistant electrical outlet. Menus - Planning menus in advance helps to ensure that food will be on hand. Posting menus in a prominent area and distributing them to parents/guardians helps to inform parents/ guardians about proper nutrition. Parents/guardians need to be informed about food served in the facility to know how to complement it with the food they serve at home. If a child has difficulty with any food served at the facility, parents/guardians can address this issue with appropriate staff members. Substitutions must be documented on the menu prior to the meal being served. Children's Records - Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. One out of two records monitored today did not have a medical and immunization record within 30 days of enrollment. You indicated the child was enrolled in your other facility and transferred to your new location. You thought the file could be transferred along with the child. Although you own both facilities, they have two different facility ID numbers. When the child transferred to the new location, a new application was to be completed along with all requirements on the children’s file checklist. Nutrition - Children learn through simply watching other people which means that our own behavior can easily be picked up and copied by children. In relation to eating, this means that children are likely to imitate the eating behaviors of people they spend time with and see around them. This means that role models can play a really important part in shaping children's eating habits. To be good role models for children food and beverages must meet the nutritional requirements as outlined in the Meal Pattern for Children in Child Care Programs when they are in the presence of children. A clear cup of lemonade was stored on a low-lying shelf in space 3. We reviewed the requirement of only consuming nutritional beverages and food in the presence of children. Preservice Requirements – Preservice requirements for the lead teacher and teachers’ position are in place to ensure children are being cared for by qualified individuals. In licensed childcare centers a lead teacher or a teacher must be 18 years of age and have a high school diploma or its equivalent. An individual identified as the lead teacher today in Space 1, does not have a high school diploma. You indicated the individual is currently enrolled in school to obtain a diploma and that a lead teacher will be positioned in that space once the enrollment exceeds five children. You, R. Moore, Administrator, assisted me with the visit. This facility was issued a temporary license on June 6, 2025 with a restriction of daytime care only and children under 2 ½ years old in rooms with direct exits only. A sampling of classrooms, the outdoor learning environment, sampling of staff and child files, and other required program records were monitored. Lunch consisted of chicken nuggets, green beans, mandarin oranges, and milk. The NC Secretary of State website was reviewed on July 7, 2025 and Alannah’s Playhouse, LLC was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed and the information is current. It is important to frequently check your email to remain informed about any new changes that have or may occur. Compliance History - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The administrator indicated she thought the child would turn two years old in September and immediately arranged for the parent to pick up the child. Remember the only exceptions are children can be grouped together for the first and last operating hour of the day, provided the staff/child ratio for the youngest child in the group is maintained; and when a physician certifies that the developmental age of the child makes this placement appropriate. Preservice Requirements – Preservice requirements for the lead teacher and teachers’ position are in place to ensure children are being cared for by qualified individuals. In licensed childcare centers a lead teacher or a teacher must be 18 years of age and have a high school diploma or its equivalent. An individual identified as the lead teacher today in Space 1, does not have a high school diploma. You indicated the individual is currently enrolled in school to obtain a diploma and that a lead teacher will be positioned in that space once the enrollment exceeds five children. Currently the individual does not meet the requirements for the lead teacher or teacher position. You must place a qualified individual in the space to meet the requirements. T. Simpson, Lead Teacher at your other childcare center came to the facility with her employee file and assumed the lead role during the visit. Utilizing the staff file checklist will help you ensure you have all required verifications and documents. Children’s Records – The Child’s Application captures important emergency contact and medical information. In the event of an emergency, this lets first responders know where additional medical information may be obtained. It also provides verification that a parent has authorized you to seek emergency medical care for their child. First Responders will check for this authorization before providing medical care if the parent is not present. A parent signature was not on one child’s application. The parent signed the application today during drop-off. Review this form carefully before you sign off to ensure that all items are completed. Additional Information: Rated License - We discussed the rated license process. You completed the Application for Assessment for a Two Component Star Rated License and the ERS Request Form during today’s visit and requested the Environment Rating Scales (ERS). I explained the process to register and submit information in the DCDEE WORKS System for all staff. Only the information reflected in WORKS will be used to determine education. For continued eligibility in the subsidized child care program, you must achieve a three star or higher rated license at the end of the temporary licensed period.
