Name of Operation: Efland Cheeks Elementary YMCA After School Facility ID: 68000592 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 1/6/2026 Number Present: 40 Completed Date: 1/6/2026 Age: From 5 To 12 Total Minutes: 135 Time In: 01:30 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit has been to complete the second temporary time period visit for this childcare center. When I arrived, I was greeted by the Site Director, Ms. Cassandra Davis. I explained the purpose of my visit. Currently this center operates with a Temporary License, issued on September 2, 2025. The restrictions include daytime care only; School age children only. There are 43 children enrolled and 40 children were present when attendance was taken. I observed the children arriving in the gym where attendance was taken. After attendance was taken, children transitioned to the bathroom to wash hands and then to the cafeteria for snack. I reviewed all program records requirements. The last fire drill was conducted on 12/9/25. Records: The staff and training worksheet was completed prior to today’s visit and I reviewed staff files. In addition, I reviewed 10% of the children’s records based on the number of children present in care and there were no concerns observed. The following violation was cited. Violation Number Comment Rule 1042 Accurate records were not maintained for all staff. Staff onsite did not have in their file a completed Health Questionnaire, Emergency Information and all required training forms for review. Files maintained online need to be accessible by staff, or a hard copy should be kept in the staff members files. G.S. 110-91(9) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by January 20, 2026. You can email the letter to marcia.humphrey@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM January 20, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: A violation was cited regarding staff records. It is important that all records for staff are kept onsite or made easily accessible for review. Each staff member should have a current health questionnaire, emergency information and required trainings on file/accessible. Please refer to the What’s New Section of the DCDEE website for the new rules and other updates. Ms. Davis and I talked briefly about the QRIS, Pathways to the Stars. She noted that the YMCA would work with her to choose a pathway that best fits the program needs. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. All staff are required to complete the training within 90 days of hire. The training may be accessed via the DCDEE website at www.ncchildcare.nc.gov. Recognizing and Responding to Suspicions of Child Maltreatment is also available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. I discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff and prior to enrolling children to ensure accuracy. Ensure that all new staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time period.
