Limited monitoring was conducted today. Upon arrival, I informed the appropriate parties of the purpose of my visit. A self-report was made regarding two incidents that took place on April 15, 2026. Findings: I spoke with the Program Coordinator via telephone on April 15, 2026. I spoke with the Principal, Assistant Principal and three staff members in person today, regarding the incident. Two separate incidents occurred on this day. In one incident the student stated to another teacher that they had been hurt by the teacher. The second incident was reported by a staff member. Both incidents were captured on the school’s surveillance system. I interviewed the Principal, Assistant Principal and three staff members. One staff member stated that a child came to them and stated they had been hurt by the teacher. The teacher asked the child questions to better understand what had occurred and the child stated that they had been kicked by the teacher. Upon reviewing footage, the teacher did use her foot to move the child’s leg. In another incident the children appear to be in a bookbag train. Each child is seen holding onto the friend’s bookbag in front of them. The staff member stated that they were trying to fix the zipper on the child’s bookbag in front of them. The staff member is seen pulling the child’s bookbag back towards them. This also pulls the child in front of that child backwards as well. The parent was notified by the principal regarding the second incident via telephone. During today's visit, I observed children during free play. Based on my observations and the information gathered, the following violation was cited today. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member assigned to care for the children in Space 1 was recorded pushing a child's leg with their foot. The same staff member was later seen pulling on a child's bookbag in a manner that the child and the child in front of them was pulled back. G.S. 110-91(10) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by May 12, 2026. You can email the letter to me at turkessha.casterlow@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received at my office by 5:00 PM May 12, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: A violation was cited regarding nurture and care. Teachers should use their words to describe to the children what they are requesting them to do. Teachers should utilize different techniques to transition children from one space to another or one activity to another activity. If teachers have access to the Mighty Minute cards, I suggest that they find transitional activities that will guide the children towards a smoother transition. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Linda Jones, Lead Teacher. Your program currently operates with a Five-Star license. Restrictions include Daytime care only; Meeting all enhanced standards and enhanced ratios minus one; Meets enhanced ratios; Meet enhanced space. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 98% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children participating in free play, group activities, and playing in their outside learning environment. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on December 18, 2024. A sanitation inspection was completed September 15, 2025, with a Superior classification. The last fire inspection was conducted on July 3, 2025. Program records and required postings were monitored. A fire drill was conducted on October 14, 2025. An outdoor inspection was documented on October 1, 2025. Children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. Two children's files that I checked did not have a completed health assessment on file. This DPI program is operating with five star rated license serving NCPREK children. Upon arrival, I checked in at the front office and proceeded to the PreK classrooms. There was a Head Start classroom and a NCPREK classroom. I conducted a walkthrough of the indoor and outdoor learning environment. I observed the children having a group time activity, center time activity, cleaning up, and playing in the outdoor learning environment. NCPREK requirements were monitored. The operation completed the last sanitation inspection on 10/13/2023 with a superior rating and the fire inspection on 1/19/24 and was not approved. I observed the following violations today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. "Three fire inspections including an initial and two re-inspections were conducted by a fire marshal. I only received the third inspection report when I asked for it during today's visit. The third inspection was dated 1/19/2024". 10A NCAC 09 .0304(a) 415 A current schedule was not posted for each group of children for reference. In one room, a daily schedule was not posted. GS 110-91(12);.0508(a) 847 Parent's medication authorization did not include required information. An authorization for Epipen did not have the required information such as the length of the time the authorization is good for and when the medication is to be given. 10A NCAC 09 .0803(4)(6-9) 9995 A violation was found for which there is no item number. A lead teacher provided me with a copy of the fire inspection. According to the inspection report, the operator has not passed the fire inepection, and the re-inspection is required. This DPI program is operating with five star rated license serving NCPREK children. Upon arrival, I checked in at the front office and proceeded to the PreK classrooms. There was a Head Start classroom and a NCPREK classroom. I conducted a walkthrough of the indoor and outdoor learning environment. I observed the children having a group time activity, center time activity, cleaning up, and playing in the outdoor learning environment. NCPREK requirements were monitored. The operation completed the last sanitation inspection on 10/13/2023 with a superior rating and the fire inspection on 1/19/24 and was not approved. I observed the following violations today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. "Three fire inspections including an initial and two re-inspections were conducted by a fire marshal. I only received the third inspection report when I asked for it during today's visit.
