Lynch, Principal, and J. Privott, Legal Designee/Lead teacher, assisted me with the visit. Your program currently operates with a five-star license, issued June 28, 2022, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced ratio minus one and enhanced space) and 1 quality point for the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You stated the program continues to use Creative Curriculum with Teaching Strategies Gold. Activities were documented on the creative curriculum lesson plans. Additionally, tools and resources for creative curriculum was observed. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two percent as of March 26, 2026. Chowan County School is listed as the current owner of this program. Prior to making changes to the ownership status, contact me to discuss changes of ownership procedures. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; licensed indoor and outdoor areas used by children were monitored. I observed children actively engaged in play in the indoor learning environments. Compliance with supervision and staff/child ratios requirements were met. Children were participating in free choice with age-appropriate materials, transitions and personal care routines. Proper hand-washing procedures were observed. Breakfast was observed which consisted of maple flavored pancakes, pineapples, mixed fruit and milk. The teachers were interacting and meeting the developmental needs for each of the children. You reported two new staff have been hired since the last visit. New staff files were monitored today. A sanitation inspection was completed on March 9, 2026. A fire has not occurred since the last visit and remains current. One violation was documented today. M. Lynch, Principal, N. Tyrell, Assistant Principal, L. Heginbotham, NC Pre-K Coordinator and J. Privott, lead teacher assisted with the visit. This center currently operates with a five-star license, earning seven points in the education component, seven points in the program standard component and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. The center’s last annual compliance visit was completed on November 18, 2024. The sanitation inspection was completed on February 12, 2025, earning a “Superior” classification. The fire inspection was completed on October 6, 2025, and the center was approved for daytime care only. The center continues to meet enhanced ratios minus one. Ninety (90) children were observed in care today. Children were engaged in morning activities, free choice play with developmentally age-appropriate materials in various activity areas, completing toileting and hand washing routines, interacting with teachers, and napping. Breakfast and lunch were also observed. Breakfast consisted of blueberry muffins, pineapples, string cheese and milk. Lunch consisted of hamburger on bun, French fries, baked beans, peaches and milk. The center’s compliance history score was reviewed with the operator. The program’s compliance history was 94% as of October 28, 2025. The following violations were documented. Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Screen time was not documented on the screen time log or activity plans reviewed. .0510(d)(2)(A-C) 807 A safe indoor and outdoor environment was not provided for the children. NC PRE-K REQUIREMENTS: NC Pre-K requirements were monitored during today’s visit. Based on my observation and reviewing all requirements in section .3000 for NC PreK were met. This program currently operates six licensed preschool classrooms with five of them serving NC Pre-K children with a total enrollment of ninety (90) children. Activity plans for the months of September and October of 2025 were reviewed. Developmental domains were listed and identified on the activity plans. Children were engaged in activities listed in the activity plans. It was reported the program continues to use the Creative Curriculum with Teaching Strategies Gold. Off-site forms were provided for staff records and transportation. The site monitoring tool was not available for review. Staff observed working were the same staff listed in the NC Pre-K plan. The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. TECHNICAL ASSISTANCE: SCREEN TIME: We discussed ensuring screen time is recorded for all children using the screen time log or included in the activity plan. I explained that screen time is any digital device used by children including tablets, iPad, smart boards, computer, etc. You stated moving forward the screen time for children will be included in the activity plan. I advise you to ensure staff can communicate where the screen time is documented upon request. NEW OFFICE LOCATION: The North Carolina Department of Health & Human Services, including the Division of Child Development and Early Education, has moved to a new location. The DCDEE mailing address will remain at 2201 Mail Service Center, Raleigh, NC 27699-2200. If you are visiting the new building, the paid parking deck for our headquarters is immediately beside the building at 1910 Human Services Lane. All guests will check in at the front desk and be escorted by a DHHS staff member to the appropriate floor for their meeting. PATHWAYS TO THE STARS: The discussion regarding your program’s pathway to the stars was not conducted today with the Nc Pre-K Director. We talked about the requirements to meet pathway 1 & 2. I also shared documents and information regarding the family community engagement standards, the continuous quality improvement plan, the application process, rated license process and self-study. You stated Pathway #1 is the choice at this time; however, if there are changes you would notify me. We set a tentative timeframe for requesting the scale in early spring around March 2025. I suggest having staff and the administrator to start completing the self-study and continuous quality improvement plan. The previous environment rating scale assessment score should be used for completing self-study. I explained that staff education needs to be in DCDEE WORK and evaluated at the time of processing the rated license. I recommended visiting the NCRLAP website to review the webinars and information. Technical assistance, training and support can be requested from your local Smart Start/Partnership for Children. Contact me with any questions. FIRE INSPECTION REPORT: You gave me a copy of the fire inspection report dated October 6, 2025. It was the report completed for the entire school. I asked that you request the fire inspector complete the fire inspection report for child care moving forward. A copy of the form was given to you. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. NC Pre-K requirements in the NC Child Care Rules Section .3000 which are applicable to Rooms B102, B104, B106, B107, and B108 were monitored during the Annual Compliance visit on 11/18/24. Last Annual Compliance visit – 11/18/24 18-month compliance history from 10/8/23– 4/7/25 = 93% Last Sanitation Inspection – 2/12/25 - Superior Last Fire Inspection – 9/30/24 – Satisfactory *Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. Send fire inspections to me within seven (7) days of the inspection. Required Water, Lead Paint, and Asbestos Testing - The Clean Classrooms for Carolina Kids website reflects the following information about your center’s required three-year water testing and one-time lead paint testing, and asbestos testing: Three-year Water Testing – 8/15/24; due again 8/15/27 Lead Paint Testing – enrollment started; move forward to complete this process Asbestos Testing – enrollment started; move forward to complete this process Maintain any paperwork received by mail or email in your Program Records file. *Because the program is part of a larger DPI school, water testing for the school may count as testing for the licensed Pre-K; however, the Pre-K program should consider enrolling separately for the lead paint testing and asbestos testing to take advantage of the 100% reimbursement for mitigation if lead paint or asbestos is found. *For more information check out the FAQ page at https://www.cleanwaterforuskids.org/en/carolina/ M. Lynch, Administrator, was present, she and the Pre-K staff assisted me with the visit. The facility currently operates with a Five-Star Center License, issued 6/28/2022, earning points in the following components: Program (7 PTS) + Education (7 PTS) + Quality Point (1 PT) = 15 Points = 5 Stars The program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum 5th edition) as required for all four- and five-star facilities where four-year old children are enrolled. The next rated license assessment is due by 6/28/25, but legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. At this time, you may choose to complete a rated license assessment using the current rated license process or you may wait until the new QRIS is implemented. The proposed QRIS Modernization rules have been published and are pending adoption. Note that the current program assessment tool is changing from the Early Childhood Environment Rating Scale – Revised (ECERS-R) to the Early Childhood Environment Rating Scale – 3 (ECERS-3). Information and training on the new program assessment tool is available on the NC Rating License Assessment Project website at https://ncrlap.org/. This facility is owned by Edenton Chowan Schools. S. Evans is no longer the Authorized Individual for the school system or the administrator of the NCPre-K program. M. Lynch, School Principal, informed me that the current superintendent, T. Ward, will be listed as the Authorized Individual for the school system. I gave her a blank Appendix G (Government) to have Ms. Ward complete, sign and return to me. M. Lynch will be listed as the new administrator. I provided her with an Administrator Preservice form to complete, sign, and have signed by Ms. Ward. Please return that also. I made a few updates in Regulatory to reflect new email and contact information. Please contact me prior to any information change regarding the ownership of this facility. License information was current. Contact me to make any changes to address, phone, or email or to request changes to the license. The NCPre-K program requested to increase their classroom staff/child ratios and capacity from 1:9 to 1:10 with a maximum capacity of 20. A new building inspection is not required, but you need to obtain approval from the Building Inspector to change the classroom capacities. An email from the building inspector stating that you may increase the capacity of each classroom from 18 to 20 children will be sufficient. You currently have eight (8) rooms approved for use: B102, B104, B106, B107, B108, B109, B110, B11. You are not currently using B109 and B111 but get them approved for 20 children also so that they can be used at that capacity in the future, if needed. This will increase your overall capacity to 160 children. When I receive the forwarded email from the building inspector, I will approve twenty in each classroom, and you can increase the numbers as needed. I will update the permit to reflect the capacity increase. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 3/2024. I monitored five staff records and all program records. The facility meets DPI requirements for transportation. Center Observations: Eighty (80) children were present in five classrooms with two teachers in each classroom. Children participated in a variety of activities including group time with stories and music and movement, small group time where they worked on specific tasks related to the weekly theme, and free play where they chose their centers and played independently and with friends. Teachers provided supervision while interacting with children to encourage conversation and language development. Challenging behaviors were handled with redirection and substitution. Children played on the fenced playground which has been recently updated to include synthetic surfacing, outdoor storage, and portable gross motor materials. Children ate at the child-sized tables and chairs in their classrooms. Lunch was grilled cheese sandwiches, French fries, sweet potatoes, mixed fruit, and milk. Children rested on linen-covered cots. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A staff person hired on August 14, 2024 had started an application for a criminal background check but had not completed it. Medication was stored in red ready-to-go bags with medical action plans and administration logs. Medication permission forms are kept on file with the nurse. Emergency medications (asthma inhalers, Epi-pens, anti-seizure medication) can be stored out of reach of children. The next rated license assessment is due by 6/28/25, but legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. At this time, you may choose to complete a rated license assessment using the current rated license process or you may wait until the new QRIS is implemented. The proposed QRIS Modernization rules have been published and are pending adoption. Note that the current program assessment tool is changing from the Early Childhood Environment Rating Scale – Revised (ECERS-R) to the Early Childhood Environment Rating Scale – 3 (ECERS-3). I made a few updates in Regulatory to reflect new email and contact information. Please contact me prior to any information change regarding the ownership of this facility. License information was current. Contact me to make any changes to address, phone, or email or to request changes to the license. The NCPre-K program requested to increase their classroom staff/child ratios and capacity from 1:9 to 1:10 with a maximum capacity of 20. A new building inspection is not required, but you need to obtain approval from the Building Inspector to change the classroom capacities. An email from the building inspector stating that you may increase the capacity of each classroom from 18 to 20 children will be sufficient. You currently have eight (8) rooms approved for use: B102, B104, B106, B107, B108, B109, B110, B11. You are not currently using B109 and B111 but get them approved for 20 children also so that they can be used at that capacity in the future, if needed. This will increase your overall capacity to 160 children. When I receive the forwarded email from the building inspector, I will approve twenty in each classroom, and you can increase the numbers as needed. I will update the permit to reflect the capacity increase. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 3/2024. I monitored five staff records and all program records. The facility meets DPI requirements for transportation. Center Observations: Eighty (80) children were present in five classrooms with two teachers in each classroom. Children participated in a variety of activities including group time with stories and music and movement, small group time where they worked on specific tasks related to the weekly theme, and free play where they chose their centers and played independently and with friends. Teachers provided supervision while interacting with children to encourage conversation and language development. Challenging behaviors were handled with redirection and substitution. Children played on the fenced playground which has been recently updated to include synthetic surfacing, outdoor storage, and portable gross motor materials. Children ate at the child-sized tables and chairs in their classrooms. Lunch was grilled cheese sandwiches, French fries, sweet potatoes, mixed fruit, and milk. Children rested on linen-covered cots. NC Pre-K requirements in the NC Child Care Rules Section .3000 which are applicable to Rooms B102, B104, B106, B107, and B108 were monitored during the Annual Compliance visit on 11/18/24. Last Annual Compliance visit – 11/18/24 18-month compliance history from 10/8/23– 4/7/25 = 93% Last Sanitation Inspection – 2/12/25 - Superior Last Fire Inspection – 9/30/24 – Satisfactory *Sanitation and fire inspections are required at least annually by the end of the month of the previous inspection. The facility currently operates with a Five-Star Center License, issued 6/28/2022, earning points in the following components: Program (7 PTS) + Education (7 PTS) + Quality Point (1 PT) = 15 Points = 5 Stars The program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum 5th edition) as required for all four- and five-star facilities where four-year old children are enrolled. The next rated license assessment is due by 6/28/25, but legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. At this time, you may choose to complete a rated license assessment using the current rated license process or you may wait until the new QRIS is implemented. The proposed QRIS Modernization rules have been published and are pending adoption. Note that the current program assessment tool is changing from the Early Childhood Environment Rating Scale – Revised (ECERS-R) to the Early Childhood Environment Rating Scale – 3 (ECERS-3). Ward, will be listed as the Authorized Individual for the school system. I gave her a blank Appendix G (Government) to have Ms. Ward complete, sign and return to me. M. Lynch will be listed as the new administrator. I provided her with an Administrator Preservice form to complete, sign, and have signed by Ms. Ward. Please return that also. I made a few updates in Regulatory to reflect new email and contact information. Please contact me prior to any information change regarding the ownership of this facility. License information was current. Contact me to make any changes to address, phone, or email or to request changes to the license. The NCPre-K program requested to increase their classroom staff/child ratios and capacity from 1:9 to 1:10 with a maximum capacity of 20. A new building inspection is not required, but you need to obtain approval from the Building Inspector to change the classroom capacities. An email from the building inspector stating that you may increase the capacity of each classroom from 18 to 20 children will be sufficient. You currently have eight (8) rooms approved for use: B102, B104, B106, B107, B108, B109, B110, B11. You are not currently using B109 and B111 but get them approved for 20 children also so that they can be used at that capacity in the future, if needed. This will increase your overall capacity to 160 children. When I receive the forwarded email from the building inspector, I will approve twenty in each classroom, and you can increase the numbers as needed. I will update the permit to reflect the capacity increase. I monitored the classrooms and spaces used for child care and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 3/2024. I monitored five staff records and all program records. The facility meets DPI requirements for transportation. Center Observations: Eighty (80) children were present in five classrooms with two teachers in each classroom. Children participated in a variety of activities including group time with stories and music and movement, small group time where they worked on specific tasks related to the weekly theme, and free play where they chose their centers and played independently and with friends. Teachers provided supervision while interacting with children to encourage conversation and language development. Challenging behaviors were handled with redirection and substitution. Children played on the fenced playground which has been recently updated to include synthetic surfacing, outdoor storage, and portable gross motor materials. Children ate at the child-sized tables and chairs in their classrooms. Lunch was grilled cheese sandwiches, French fries, sweet potatoes, mixed fruit, and milk. Children rested on linen-covered cots. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A staff person hired on August 14, 2024 had started an application for a criminal background check but had not completed it. G.S. 110-90.2(b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for a staff member expired on December 20, 2024 and was not renewed. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. Two staff members did not have a current qualification letter. G.S. 110-90.2(b) & (d) & .2703(e) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Documentation of completion of the ABCMS Provider Portal training was not available. G.S. 110-90.2 & .2703(r) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4)(d) to achieve and maintain an 18-month compliance history score of at least 75%. Her Qualifying Letter expired on 12/20/2024 and she has not started the process to renew her five-year CBC. You stated that in the transition of staff and administration. Ms. Perry needs to begin the process immediately so that it is completed, and she receives a new Qualifying Letter by 4/24/2025. If not completed and a copy of the Qualifying Letter is not emailed to me by that date, she may not work in the licensed Pre-K program until I receive a copy of her Qualifying Letter. Chasity Strawser was hired 8/14/2024 and is a NCPre-K Teacher Assistant on Room 102. Ms. Strawser has not completed a Criminal Background Check and does not have a Qualifying Letter. Information on the ABCMS Portal reflects that she started the process but did not complete it. The portal states “A New Application Must Be Submitted.” You stated that you thought it had been completed. Because Ms. Strawser has never completed a CBC nor received a CBC Qualification Letter, she may not work in the licensed Pre-K classroom until she is qualified and has provided you with a Qualification Letter. She may work in other parts of the elementary school. You will need to use a qualified substitute in Room 102 until Ms. Strawser provides you with a Qualifying Letter. All owners of licensed child care facilities and employees working in a licensed child care center are required to submit a Criminal Background Check (CBC) and receive their Qualifying Letter prior to their first day of employment and complete a five-year requalification (on-line application and fingerprints) so that the new Qualifying Letter is on file before the expiration date listed on their current Qualifying Letter. Multiple instances of failing to complete Criminal Background Checks may result in an Administrative Action. Your program currently operates with a five-star license, issued May 23, 2022, earning 7 points in the education component, 7 points in the program standards component (meeting enhanced requirements and reduced ratios) and 1 quality point. Your program was also monitored for compliance with implementing an approved curriculum Creative Curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted December 14, 2023. The sanitation inspection was completed on October 3, 2024, with a “Superior” classification. The last fire inspection was conducted on December 15, 2023, and your facility was approved for daytime care only. The center’s contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed. All information is current. It is important that you check your email at least once a week. If any of your contact information changes, contact me. Upon arrival children throughout the center were eating breakfast. Breakfast consisted of Marshmallow mates cereal, applesauce, and milk. After breakfast, children transitioned to personal care routines and Indoor freeplay. Indoor freeplay, large group time, Outdoor freeplay, and proper handwashing was observed. Developmentally appropriate toys were available on low-lying open shelves. The fenced outdoor area includes stationary and portable equipment. Additionally, a covered shelter provides a shaded space for the children. The lunch menu reflected breaded drumstick, yams, mixed fruit, roll and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files was monitored. The facility uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 7:50 am to 2:50 pm. Parent conferences are held three times per year. You stated that you use the Class DoJo app, newsletters, and face to face opportunities to communicate with parents. The following violations were documented. In space B107 an electrical power strip beside the refrigerator was missing three(3) outlet covers, the outlet located under the television was missing and outlet cover, and the outlet located near the cots was missing an outlet cover. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space B108 an aerosol can of Lysol disinfectant spray, 2 cans of Barbasol shaving cream, a box of 20 Mule borax, and 2 containers of Clorox wipes were stored in the unlocked cabinet above the sink. In space B107 a packet of Vita Globe Super Probiotics Dietary Supplements was stored on the side of a lunch bag that was sitting on top of the mini refrigerator. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space B107 a tube of Orajel was in a lunch bag on the shelf beside the Cozy Corner. 15A NCAC 18A .2820(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented were corrected immediately during the visit. Technical assistance: Storage of Hazardous Items-All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. All aerosols must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and key must not be stored in the lock. In space B108 an aerosol can of Lysol disinfectant spray, 2 cans of Barbasol shaving cream, a box of 20 Mule borax, and 2 containers of Clorox wipes were stored in the unlocked cabinet above the sink. In space B107 a packet of Vita Globe Super Probiotics Dietary Supplements was stored on the side of a lunch bag that was sitting on top of the mini refrigerator. Daily checks should be performed to make sure these items are behind locked storage. This was corrected during the visit. Electrical Outlets - Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets (2,3). Most these injuries involve children under the age of six. In space B107 an electrical power strip beside the refrigerator was missing three(3) outlet covers, the outlet located under the television was missing and outlet cover, and the outlet located near the cots was missing an outlet cover. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips and outlets over counters in the kitchen. This was corrected during the visit. Storage of Medications - Items that might be poisonous or hazardous to children must be kept in locked storage to prevent unintentional injuries. In space B107 a tube of Orajel was in a lunch bag on the shelf beside the Cozy Corner. Prevention is the key to prevent poisonings by making sure medications are inaccessible to children. This item was corrected during the visit. Additional Information: Employee Purses/Lunch Bags- Employee purses and other personal effects must be kept out of reach of children. It is important to remember that purses/lunch bags may contain items that can be potentially hazardous to young children. Storing purses and personal effects five feet from the finished floor protects children from potential injury. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. The last annual compliance visit was conducted 1/5/2023. The sanitation inspection was completed on October 16, 2023, with a “Superior” classification.
