Upon arrival D. Knotts, Cook assisted with the visit. You, L. Knotts, Administrator, arrived shortly after and assisted me with the visit. Your program currently operates with a Notice of Compliance effective September 13, 2022. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of February 25, 2026. The NC Secretary of State website was reviewed on February 25, 2026, and First Assembly of God of the City of Edenton was listed as current- active. The Notice of Compliance was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. You also allowed me to observe interactions in classrooms, from the video footage in your office. I observed children engaged in activities in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas, transitions and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. Lunch was observed and consisted of ham and cheese sandwiches, cucumbers, strawberries and milk. Fire and sanitation inspections have not occurred since the last visit. The sanitation inspection remains current; however, the fire inspection expired on October 29, 2025. Five violations were observed today. Two were corrected during the visit and the other must be corrected immediately. The violations are as follows: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center's fire inspection expired on October 29, 2025. 10A NCAC 09 .0304(a) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. One child's hands were not washed after a diaper change in Space #2. 15A NCAC 18A .2803(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two cans of Lysol spray in aerosol dispensed container were observed in an unlocked cabinet beneath the diaper changing table. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A bottle of Mylicon gas drops were not return to the parent or discarded after the authorization and medication had expired. .0803(12) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A caregiver was observed drinking a Welches fruit drink in the presence of children in Space #2. .0901(i) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. SAFE INDOOR ENVIRONMENT: During my observation, while checking cabinets to ensure they were secured properly, I observed two cans of Lysol spray in aerosol dispensed containers and cleaning wipes stored under an unlocked diaper changing table in Space #2. One of the staff members indicated she was planning to re-lock the cabinet, as items had been previously removed. I informed you that any item in an aerosol dispensing container must remain locked in a space with an approved locking mechanism when not in use. I encouraged you to remind staff to ensure all cabinets and closets are properly secured when items are not actively accessed. Additionally, I notified you that an approved locking mechanism must be installed on the cabinet located in the bathroom of Space #1, as supplies and aerosol cans are stored in that cabinet. You stated a lock has already been order for that particular cabinet and would be added when received. MODELING APPROPRIATE EATING BEHAVIORS: One caregiver was observed drinking Welch’s fruit juice in the presence of children. I notified you that child care requirements state staff must model appropriate eating behavior while in the presences of children which includes drinks. Therefore, only foods and beverages that meet USDA nutritional guidelines may be consumed in front of children. I explained that staff can be permitted to have personal beverages; however, this should be done outside of the classroom or in an area where children are not able to visually observe. Best practice would be to consume food and drinks that are nutritional and possibly using those items to implement a lesson on healthy lifestyles with children. HANDWASHING: One child’s hands were not washed after a diaper change. The child was placed on the floor and allowed to resume playing with materials prior to hand washing. After you were notified, you went into the classroom, spoke with the teacher and ensured the child’s hands were properly washed to meet requirements. Please be reminded that hand washing is essential in maintaining a safe and healthy environment and plays a critical role in reducing the spread of germs. QRIS PATHWAY TO THE STARS: I shared the Division has changed the quality rating improvement system. I explained that the transition for licensed programs to the new QRIS system will be completed on or before December 2026. Upon arrival, M. Pate, Teacher, assisted with the visit. You arrived shortly after and assisted. Your program currently operates with a Notice of Compliance, issued September 13, 2022. The last annual compliance visit was conducted on November 14, 2024. The sanitation inspection was completed on July 17, 2024, with a “Superior” classification. The last fire inspection was completed on October 29, 2024, and your facility was approved for daytime care only. During my walk-through, twenty-eight children were observed in care today. Children were engaged in free-choice play, eating breakfast, completing toilet and hand washing routines, napping and with teachers. Breakfast consisted of toast, scrambled eggs, sliced apples, and milk. The following violations were documented: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Cement footing anchoring three wooden poles on the shade port were protruding above ground posing tripping hazards. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A can of Scentsy air spray in an aerosol dispensed container was on the cubby above the diaper changing table in space #1. A bottle of MiraLAX was observed in an unlocked file cabinet in Space #4. Lysol spray, Clorox wipes and other items in aerosol dispensed containers were stored in a cabinet with a child protective lock. .2820(b) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Sunscreen in Space 2, for one preschool child enrolled did not have a permission form to accompany it. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. A tube of Aquaphor diaper cream in Space 2 for one preschool child expired in August 2025. 10A NCAC 09 .0803(1)(d) 871 Center staff did not comply with the safe sleep policy. An infant was observed asleep in a rocking swing in Space #1 upon arrival into the classroom. 10A NCAC 09 .0606(a) The violations documented must be corrected immediately. According to information verified on the North Carolina Secretary of State’s website, First Assembly of God of the City of Edenton remains Current and Active. SAFE INDOOR ENVIRONMENT: We discussed the potentially hazardous items observed during the walk-through. I recommend having caregivers conduct safety checks of their classrooms to ensure any potential hazards are removed and made inaccessible to children. Be reminded that any item in an aerosol dispensed container must be stored in a locked cabinet or closet. Additionally, child safety locks are not considered approved locks according to the child care requirements. Approved locks are a combination, electronic, or magnetic device, key, or equivalent locking device. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. It was also suggested you encourage the staff to reference the storage of hazardous items flyers posted on the walls in their classrooms to assist with determining how items should be stored. You acknowledged awareness of the cement footings protruding above ground on the preschool playground and stated that the process of having them removed has already been initiated. I advised you to develop and implement a safety plan to ensure children do not have access to the cement footing to avoid potential tripping while playing. I also observed that the presence of the cement footing was noted on the playground inspection forms for the months of August and September. You should ensure that any safety concerns identified are addressed and corrected immediately to prevent children from being exposed to potential safety hazards. We also discussed ensuring electrical outlets not in use always be covered with safety caps. Safety caps could be placed in visual spaces as a reminder to reinsert once the outlet is no longer used. MEDICATION REQUIREMENTS: A tube of Aquaphor diaper cream expired in August 2025. In addition, a container of sunscreen did not have the required permission to administer medication form. I advised you to have staff periodically check all medications and topical ointments for expiration dates and to ensure the required permission forms are current and on file prior to administering he medication to children. ANNUAL IMMUNIZATION REPORT: The annual immunization report will be due soon. North Carolina immunization law requires all schools providing Pre-K and all licensed childcare facilities (as defined in G.S. 110-86(3) to file an annual immunization report on children aged newborn through preschool (G.S. 130A155(c). The last fire inspection was completed on October 29, 2024, and your facility was approved for daytime care only. During my walk-through, twenty-eight children were observed in care today. Children were engaged in free-choice play, eating breakfast, completing toilet and hand washing routines, napping and with teachers. Breakfast consisted of toast, scrambled eggs, sliced apples, and milk. The last annual compliance visit was conducted on November 14, 2024. The sanitation inspection was completed on July 17, 2024, with a “Superior” classification. The last fire inspection was completed on October 29, 2024, and your facility was approved for daytime care only. During my walk-through, twenty-eight children were observed in care today. Children were engaged in free-choice play, eating breakfast, completing toilet and hand washing routines, napping and with teachers. Breakfast consisted of toast, scrambled eggs, sliced apples, and milk. The following violations were documented: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Be reminded that any item in an aerosol dispensed container must be stored in a locked cabinet or closet. Additionally, child safety locks are not considered approved locks according to the child care requirements. Approved locks are a combination, electronic, or magnetic device, key, or equivalent locking device. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. It was also suggested you encourage the staff to reference the storage of hazardous items flyers posted on the walls in their classrooms to assist with determining how items should be stored. You acknowledged awareness of the cement footings protruding above ground on the preschool playground and stated that the process of having them removed has already been initiated. I advised you to develop and implement a safety plan to ensure children do not have access to the cement footing to avoid potential tripping while playing. I also observed that the presence of the cement footing was noted on the playground inspection forms for the months of August and September. You should ensure that any safety concerns identified are addressed and corrected immediately to prevent children from being exposed to potential safety hazards. We also discussed ensuring electrical outlets not in use always be covered with safety caps. Safety caps could be placed in visual spaces as a reminder to reinsert once the outlet is no longer used. MEDICATION REQUIREMENTS: A tube of Aquaphor diaper cream expired in August 2025. In addition, a container of sunscreen did not have the required permission to administer medication form. I advised you to have staff periodically check all medications and topical ointments for expiration dates and to ensure the required permission forms are current and on file prior to administering he medication to children. Please contact me prior to any information change regarding the ownership of this facility. License and contact information were current, but we added the administrator’s cell phone. Contact me to make any changes to address, phone, or email or to request changes to the license. Today, we updated the Legal Designee Information form to reflect the administrator and assistant administrator positions as legal designees. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored seven staff records and all program records. The facility does not offer transportation. Center Observations: Children in all classrooms were engaged with teachers in developmentally appropriate free play and teacher directed play. Infants ate, played and slept on their own schedules. Documentation of safe sleep checks and feeding plans were available. Bottles were dated and labeled and stored in the refrigerator. Staff provided time for tummy time on the floor. The toddlers were using paint dabbers to decorate their frog head bands. The children, two years of age, were busy with free play with materials accessible on low shelves. The children, three years of age, in Space 4 were building with magnetic blocks at the table and playing make believe “phone”. In the large space in the back, the preschool-age children were decorating lady bug head bands on the right while the school-age children were working on arts and crafts in their assigned space on the left. Lunch was ham and cheese wrap on a tortilla, cucumbers, strawberries, and milk. The cook cut up the wrap into small pieces for the younger children. After lunch, the children rested on linen-covered cots. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An outlet behind a crib in Space 1 was not in use or protected with an outlet cover. 10A NCAC 09 .0604(c) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Three staff members did not complete Recognizing and Responding to Suspicions of Child Maltreatment or documentation of completion was not available. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored seven staff records and all program records. The facility does not offer transportation. Center Observations: Children in all classrooms were engaged with teachers in developmentally appropriate free play and teacher directed play. Infants ate, played and slept on their own schedules. Documentation of safe sleep checks and feeding plans were available. Bottles were dated and labeled and stored in the refrigerator. Staff provided time for tummy time on the floor. The toddlers were using paint dabbers to decorate their frog head bands. The children, two years of age, were busy with free play with materials accessible on low shelves. The children, three years of age, in Space 4 were building with magnetic blocks at the table and playing make believe “phone”. In the large space in the back, the preschool-age children were decorating lady bug head bands on the right while the school-age children were working on arts and crafts in their assigned space on the left. Lunch was ham and cheese wrap on a tortilla, cucumbers, strawberries, and milk. The cook cut up the wrap into small pieces for the younger children. Today, we updated the Legal Designee Information form to reflect the administrator and assistant administrator positions as legal designees. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored seven staff records and all program records. The facility does not offer transportation. Center Observations: Children in all classrooms were engaged with teachers in developmentally appropriate free play and teacher directed play. Infants ate, played and slept on their own schedules. Documentation of safe sleep checks and feeding plans were available. Bottles were dated and labeled and stored in the refrigerator. Staff provided time for tummy time on the floor. The toddlers were using paint dabbers to decorate their frog head bands. The children, two years of age, were busy with free play with materials accessible on low shelves. The children, three years of age, in Space 4 were building with magnetic blocks at the table and playing make believe “phone”. In the large space in the back, the preschool-age children were decorating lady bug head bands on the right while the school-age children were working on arts and crafts in their assigned space on the left. Lunch was ham and cheese wrap on a tortilla, cucumbers, strawberries, and milk. The cook cut up the wrap into small pieces for the younger children. After lunch, the children rested on linen-covered cots. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. L. Knott, administrator, arrived on-site and assisted me as needed with the visit. Your facility currently operates with a Notice of Compliance issued 9/13/2022. The last annual compliance visit was conducted 12/7/2023. The sanitation inspection was conducted on 7/17/24 with a “Superior” classification. The last fire inspection was conducted 10/29/24and your facility was approved for daytime care only. The program’s compliance history was ninety-four percent as of 11/8/2024. This facility is owned by First Assembly Of God Of The City Of Edenton according to the NC Secretary of State's website, this non-profit corporation is current and active as of 11/8/2024. Please contact your child care consultant or DCDEE prior to any information changing regarding the ownership of this facility. A walk-through of the facility was completed today. I observed children during nap time resting on linen-covered cots. After nap time children transitioned to personal care routines and were offered snack. Snack consisted of strawberry and banana flavored yogurt, crackers, and water. The two fenced outdoor play areas consisted of a variety of push and pull toys, assortment of balls, benches, picnic tables, and areas for shade. There following violations were documented. In one out of seven files reviewed a statement acknowledging receipt was not available for review. .0608(b)(1-6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. L. Knotts, Administrator, assisted me as needed with the visit until she had to leave. Your facility currently operates with a Notice of Compliance issued 9/13/2022. The last annual compliance visit was conducted 1/4/2023. The sanitation inspection received – 9/1/2023 with a “Superior” classification. The last fire inspection was conducted 7/14/2023 and your facility was approved for daytime care only. The program’s compliance history was ninety-one percent as of 12/1/2023. This facility is owned by First Assembly Of God Of The City Of Edenton according to the NC Secretary of State's website, this non-profit corporation is current and active as of 12/1/2023. Please contact your child care consultant or DCDEE prior to any information changing regarding the ownership of this facility. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found children throughout the facility were participating in free play, transitions and personal care routines. The two fenced outdoor play areas consisted of a variety of push and pull toys, assortment of balls, benches, picnic tables, and areas for shade. Lunch was served and consisted of peanut butter and jelly sandwiches, mandarin oranges, potatoes, and milk. The following violations were documented. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. A child enrolled on 7/31/23 did not have documentation available for review. GS 110-102 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An outlet in the hallway did not have safety cover. Two outlets in the school-age space did not have safety covers. 10A NCAC 09 .0604(c) 893 A copy of the safe sleep policy was not given to and/or explained to the parent of each child on or before the first day the infant attended the center. A child enrolled on 7/31/23 did not have documentation available for review. 10A NCAC 09 .0606(c) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child enrolled on 7/31/23 did not have a medical exam on file until 9/13/23. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. A child enrolled on 7/31/23 did not have an immunization record available for review today. 10A NCAC 09 .0302(d)(2) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. A child enrolled on 7/31/23 did not have documentation available for review today. .1804(c) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. A child enrolled on 7/31/23 did not have documentation available for review today. .0604(j) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. In space #2 medical authorization for two(2) Ventolin HFA inhalers was expired. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. A child enrolled on 7/31/23 did not have documentation available for review today. .0608(b)(1-6) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before 12/21/2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical assistance: Summary of NC Child Care Law - Operators of child care centers must provide a copy of a summary of the child care law to each child’s parent, guardian, and full-time custodian before the child is enrolled in the center. Each parent, guardian, or full-time custodian must sign a statement acknowledging that he or she received a copy of the summary before the child’s enrollment. A child enrolled on 7/31/2023 did not have documentation available for review. A signature form could simplify your enrollment process for parents and at the same time, ensure you have on file what is required. Electrical Outlets - Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. Two electrical outlets located near the television in the school-age space did not have safety covers. All electrical outlets not in use that are in areas accessible to children must be protected with an outlet cover. This includes unused outlets on power strips. Daily checks should be done to replace outlet safety covers. Safe Sleep- Place babies, 12 months of age or younger, to sleep on their back. A waiver may apply under certain circumstances. Childcare providers must have a written safe sleep policy and must explain it to parents before the baby is enrolled. Today a child enrolled on 7/31/23 did not have documentation available for review. Permission to Administer Medication - Any medication remaining after the course of treatment is completed, after authorization had expired, or the child withdraws from care, must be returned to the child's parent. Today, the permission to administer medication forms for a Ventolin HFA inhaler in space #2 expired on November 8, 2023 and a Ventolin HFA inhaler in space #2 expired on 7/19/23. Teachers should be reminded to check their medications each month to ensure that all permission forms are complete and current. If the medication is no longer needed, it must be returned to the parent within 72 hours or discarded. You will need to create a procedure to track that your staff are monitoring medications at least monthly and include this tracking plan in your letter of compliance. Children's Records(Medical and Immunization) -Children are required to have a medical exam by a doctor and a copy of their immunization record no later than 30 days from their first day of attendance. Immunization records are required for all children; however, a medical exam is required for all children from birth to five, that have not started Kindergarten. A medical exam can be accepted if completed within the last 12 months prior to enrollment. A child enrolled on 7/31/23 did not have a medical report on file until 9/13/23, immunization documentation was not available for review. Review children’s records periodically to ensure that the required documentation is on file by the intended due date. Discipline Policy - Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. A discipline policy must be reviewed with each parent prior to enrollment and they must sign the form showing this was completed. Smoking and Tobacco Policy- Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. Notification of you smoking and tobacco restriction must be in writing and given to each parent prior to enrollment. A child enrolled on 7/31/2023 did not have documentation available for review. Review your children’s files on a regular basis to ensure all information is on file, as required. Shaken Baby Syndrome and Abusive Head Trauma: Educating parents is a key part of being a provider. Reviewing the policy helps parents to better understand the dangers of Shaken Baby Syndrome and the effects of head trauma. Per our discussion, you use a policy signature page when reviewing policies with your parents. Signed acknowledgements must be placed in each child’s file. Additional Information: Record keeping is one of the most time-consuming tasks associated with managing a child care business. Maintaining complete and accurate records for the program, staff and children is a vital part of being a licensed child care provider. Setting aside regularly scheduled times each week to review records can assist you in maintaining accurate licensing records. A tracking and reminder system is also essential to keep track of tasks, due dates and expiration dates for maintaining compliance with child care requirements. Being proactive with an organized approach for record-keeping helps you ensure easy access to information when needed, especially in emergency situations. New Criminal Background Check Changes – Legislative changes approved on 7/11/2022 made two important changes to Criminal Background Checks required for child care professionals: 1. The period for background check qualification renewals was changed from every 3 years to every 5 years 2. The provisional qualification period was reduced to 45 days The Division intends to implement these changes as soon as possible. An individual’s current Qualification Letter (QL) will be extended 2 years for a total of 5 years. Updated letters will not be generated. Individuals with a Provisional Qualification Letter due to an out-of-state check will received a qualifying letter on the 45th day. Letters are generated in the CBC Portal. Public Health Toolkit Updates - The ChildCareStrongNC Public Health Toolkit for Child Care (updated 7/2022) available on the DHHS website, and sent to your facility contact email, provides guidance on requirements and recommendations to reduce the spread of COVID-19 in child care facilities. If you have questions about COVID-19, an exposure, or exclusion policies, contact the Child Care Health Consultant assigned to your county and/or your local health department. COVID Signage – On August 15, 2022, the governor lifted the “State of Emergency” status for North Carolina. As a result, you are no longer required to post the CORONA Virus Information Sheet on your main entrance door or the Health and Safety Certificate which verified that you agreed to meet the additional health and safety requirements during the pandemic.
