J. White, Administrator, assisted me with the visit. Your program operates with a five-star license, issued May 30, 2025, earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and enhanced ratios) and 1 quality point for the administrator having at least 10 years of documented child care administration work experience in a licensed program. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on April 16, 2025. The sanitation inspection was completed April 7, 2025, with a “Superior” classification. The last fire inspection was conducted on February19, 2026, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of April 14, 2026. The NC Secretary of State website was reviewed on April 13, 2026, and HESED After School Care & Summer Camp LLP was listed as current- active. I visited the indoor and outdoor spaces used by children. Children were observed completing hand-washing routines, eating snacks, completing homework and interacting with group leaders. Proper hand-washing procedures were observed. Snack consisted of a variety of chips, peaches and apple juice. The following violations were documented: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. A sanitation inspection was not completed on or before the expiration date of April 7, 2026. SAFE INDOOR/OUTDOOR ENVIRONMENT: An active ant mound was observed on the playground in space accessible to children. I showed one group leader the ant mound and discussed strategies to help remove the mound. I explained hot water and dish soap could be a temporary solution until a permanent solution could be applied. We talked about removing the ant mound in ways that are safe while children are in care. You contacted the owner and requested the mounds be addressed as soon as possible to remove the potential hazard. You covered the electrical outlets with safety caps during the visit. I suggest conducting safety checks to ensure children and staff return the safety caps when removed. The safety caps could be placed in an area close to the outlet as a visual reminder to return it after usage. SANITATION INSPECTION: A sanitation inspection has not been completed at your program since April 7, 2026. I advised you to reach out to environmental health to inquire about getting the inspection completed. You stated contact was made today and you were told a new inspector has not been hired at this time. Additionally, when transferred to the lead environmental health representative but there was no answer. You shared voice messages were left for a return call. ADMINISTRATIVE ACTION REVIEW: I informed you that modifications were required to the written policies and procedures received from you on March 31, 2026, to meet stipulation requirements. You confirmed the email was received describing the modification required. I asked you to revise the written policies and procedures and submit them to me on or before April 20, 2026. The administrative action, cover letter and corrective action plan were posted on the parent board next to the entrance door in Space #4. Implementation of the steps listed in the written policies and procedures will be monitored during future visits. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. J. White, Administrator, assisted me with the visit. Your program operates with a five-star license, issued May 30, 2025, earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and enhanced ratios) and 1 quality point for the administrator having at least 10 years of documented child care administration work experience in a licensed program. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on April 16, 2025. The sanitation inspection was completed April 7, 2025, with a “Superior” classification. The last fire inspection was conducted on February19, 2026, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of April 14, 2026. The NC Secretary of State website was reviewed on April 13, 2026, and HESED After School Care & Summer Camp LLP was listed as current- active. I visited the indoor and outdoor spaces used by children. Children were observed completing hand-washing routines, eating snacks, completing homework and interacting with group leaders. Proper hand-washing procedures were observed. Snack consisted of a variety of chips, peaches and apple juice. The following violations were documented: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. A sanitation inspection was not completed on or before the expiration date of April 7, 2026. The previous inspection report was dated April 7, 2025. 10A NCAC 09 .0304(b) 807 A safe indoor and outdoor environment was not provided for the children. An active ant mound was observed on the playground in space accessible to children. 10A NCAC 09 .0601(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Electrical outlets not in use in the wall next to the entrance and a surge protector in Space #4. 10A NCAC 09 .0604(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. SAFE INDOOR/OUTDOOR ENVIRONMENT: An active ant mound was observed on the playground in space accessible to children. I showed one group leader the ant mound and discussed strategies to help remove the mound. I explained hot water and dish soap could be a temporary solution until a permanent solution could be applied. We talked about removing the ant mound in ways that are safe while children are in care. You contacted the owner and requested the mounds be addressed as soon as possible to remove the potential hazard. You covered the electrical outlets with safety caps during the visit. I suggest conducting safety checks to ensure children and staff return the safety caps when removed. The safety caps could be placed in an area close to the outlet as a visual reminder to return it after usage. SANITATION INSPECTION: A sanitation inspection has not been completed at your program since April 7, 2026. I advised you to reach out to environmental health to inquire about getting the inspection completed. You stated contact was made today and you were told a new inspector has not been hired at this time. Additionally, when transferred to the lead environmental health representative but there was no answer. You shared voice messages were left for a return call. ADMINISTRATIVE ACTION REVIEW: I informed you that modifications were required to the written policies and procedures received from you on March 31, 2026, to meet stipulation requirements. You confirmed the email was received describing the modification required. I asked you to revise the written policies and procedures and submit them to me on or before April 20, 2026. The administrative action, cover letter and corrective action plan were posted on the parent board next to the entrance door in Space #4. Implementation of the steps listed in the written policies and procedures will be monitored during future visits. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. Today upon arrival while parking, I observed a vehicle pull up and park near the entrance gate of (HESED Afterschool Care & Summer Camp) the child care facility. As I was preparing to exit my vehicle, the school bus arrived to drop off children at the entrance; however, no one was present to retrieve them. As I continued to observe, the school bus proceeded to the front parking area and dropped children off at the licensed child care facility next door (Out of the Box Childcare Center). After the school bus departed, I entered the other facility to observe and speak with the administrator. At that time, four school-age children were standing in Space #1a and were grouped with preschool children. The group leader arrived shortly after being called to retrieve the children and then transitioned with them to the assigned child care building. During our conversation, you stated children were not supposed to be dropped off at that location. You explained that during the las visit, staff were instructed not to receive school-age children at that child care facility and the bus driver was also asked to drop the children off at their designated location. Additionally, you shared, the caregiver was asked not to take the children when the bus arrived to release them. You reported the group leader was supposed to be at the designated drop-off location waiting for the children; however, you were not sure what occurred. Later in the visit, you indicated the group leader reported she was in the bathroom at the time the school bus arrived. I reminded you that the children were not permitted to be cared for or to visit another licensed child care program in which they are not enrolled at any given time. I informed you the because the children were brought into the other licensed program and signed in at that location again, a violation would be documented for children being cared for in unapproved space. Once inside HESED After School Care & Summer Camp, the children washed hands, were served snacks and completed activities such as homework review and completing art activity of creating vehicles. The plan should include guidance for staff regarding arrival and departure procedure for school-age children. I encouraged you and the owner to contact the bus driver or school system to discuss procedures for ensuring that school-age children are dropped off at their designated location and establish steps that should be taken if no one is present to receive the children. Best practice would also be to discuss plans with parents, so they are aware of any procedures implemented that relates to their child/children. You, Joan White, Administrator, assisted me with the visit. Your program currently operates with a five-star license, issued May 30, 2025, earning 7 points in the education component, 6 points in the program standards component (meeting enhanced ratios and enhanced space) and 1 quality point for the administrator having at least 10 years of documented child care administrative experience. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of December 16, 2025. The NC Secretary of State website was reviewed on December 16, 2025, and Hesed After School Care & Summer Camp LLP was listed as current- active and also in process. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children arrived at the program on the school bus. They were checked in and transitioned to Space #3 for care. Hand washing routines were completed and snack served. Snack consisted of peanut butter crackers, choice of apple sauce or peaches, and fruit juice. The group leader and administrator were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Fire and sanitation inspections have not occurred since the last visit and remain current. Two violations were observed today. The violations are as follows: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). I advise you to develop a system for tracking the expiration dates for criminal background checks to ensure the renewal process can be completed in a timely manner. This process will build over time, using established goals to support both you in achieving and maintaining long term quality care within your program. All plans should identify a goal, what is needed to accomplish the goal, why the goal is needed, and an annual review of progress toward the goal. It is possible that a goal is achieved, and a new goal is set during your annual review. I advised you to start conducting research on the school-age curriculums to determine which best fits your program. EDUCATION STANDARDS: We discussed the requirements in the education standard for you and your staff. I request that you complete the education worksheet included in the application that will be emailed to you and return it to me. This information will be used to determine the star level for staff member’s education. You should also check to ensure staff education is included in the DCDEE WORKS system. I shared if you have an employee that choose to meet education using the competency evaluations, to notify me so we can talk to ensure the process is completed with all components included. J. White, administrator was on-site and assisted me when needed. Your program currently operates with a five-star license, issued 8/1/2022. The last annual compliance visit was conducted 6/5/23. The sanitation inspection was completed 1/6/2023 with a “Superior” classification. The last fire inspection was conducted 10/25/2022 and your facility was approved for daytime care only. According to the NC Secretary of State's website, this facility is currently owned by Hesed After School Care & Summer Camp LLP. Please contact me prior to any changes regarding the ownership of this facility. The program’s compliance history was ninety-one percent as of 11/17/2023. A walk-through of the facility was completed today, and indoor areas were monitored. I observed children in the indoor learning environment and found children throughout the facility were participating in free play and small group time. Snack was served and consisted of Cheez it crackers, mixed fruit, and milk. The following violation was documented.
