Jennifer Bradshaw Garrett, Child Care Consultant, accompanied me on the visit. You, J. Watts, Administrator, assisted me with the visit. Your program currently operates with a Notice of Compliance issued June 7, 2006. The last annual compliance visit was conducted on June 6, 2025. The sanitation inspection was completed on July 17, 2025, with a “superior” classification. The last fire inspection was conducted on July 16, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-six percent (86%) as of May 22, 2026. The NC Secretary of State website was reviewed on May 22, 2026, and Countryside Church of Christ was listed as current- active. The contact information for the facility was reviewed with you and all information was verified. No changes are required. Contact me immediately if you need to request any changes to your address, telephone number, or email address or to request changes to the license. You visited each indoor and outdoor space with me. In space 1, teachers were sitting on the floor and singing with the children. In Space 2, the children played with transportation vehicles on the carpet and at the table. The teachers interacted with them as they played. In space 3, the children participated in a teacher directed worksheet activity of identifying the letters w and x and the numbers 11- 14. They reviewed letter sounds and counted. In space 4, the children were engaged in dramatic play. They wore dress-up clothes and played with playhouse and props. Breakfast was observed and consisted of oat square cereal, pears, and milk. The following nine (9) violations were documented. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In outdoor space 1, the rubber coating was peeling causing a tripping hazard and exposing rust, the basketball goal had peeling paint and exposed rust, one corner of the white playhouse was coming apart, and the blue castle had sharp edges. In space 3, there was white out in a desk drawer. 10A NCAC 09 .0601(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space 3, there was not a medication authorization form on file for Sacrosidase. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space 1, there was albuterol with a prescription label adhered to it, but it was not in the original prescription box. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 1, there were two (2) tubes of expired Butt Paste with expiration dates of January and March 2026. .0803(12) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. A teacher who was rehired on January 12, 2026, orientation was not updated. A teacher hired on June 2, 2025, orientation was not fully completed or signed by the teacher and administrator. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file A teacher’s First Aid training expired in March 2026, and a current certificate was not on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A teacher’s CPR training expired in March 2026, and a current certificate was not on file. .1102(d) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The Administrator did not notify the Division within five (5) of an employee that was hired on hired on January 12, 2026. G.S. 110-90.2 & .2703(r) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A teacher hired on February 3, 2026, documentation of Shaken Baby Head Trauma Policy was dated February 11, 2026. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 10, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Equipment, materials, furnishings, and play areas should be sturdy, safe, and in good repair. Equipment and furnishings that are not sturdy, safe, or in good repair, may cause falls, entrap a child’s head or limbs, or contribute to other injuries. Disrepair may expose objects that are hazardous to children such as sharp points or corners; splinters; Protruding nails, bolts, or other components that could entangle clothing or snag skin; and loose, rusty parts. In outdoor space 1, the rubber coating was peeling causing a tripping hazard and exposing rust, the basketball goal had peeling paint and exposed rust, one corner of the white playhouse was coming apart, and the blue castle had sharp edges. You said that you were trying to find parts online for the large climber. It was suggested making the climber inaccessible to children until it can be repaired. I will research if you can scrape the rubber coating and paint the rusted surface. Follow up will be provided. You said that you were going to cover the rust on the basketball goal with duct tape and then enclose it in a rubber pool noodle. Snap the white playhouse’s walls back together. You removed the blue castle and discarded it. In your compliance letter, state the dates each item was corrected. Over-the-Counter Medications: Expired diaper cream active ingredients can degrade, reducing its ability to heal or prevent rashes, while its base ingredients can break down to harbor harmful bacteria. In space 1, there were two (2) tubes of expired Butt Paste with expiration dates of January and March 2026. To monitor over-the-counter medications, create a list of over-the-counter medications for each classroom that include their expiration dates and up-date the list monthly. In your compliance letter state when the Butt Pastes were returned to the parents or discarded. Storage of Hazardous Products – Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. In space 3, there was white out in a desk drawer. White out must be stored out of reach of the children, which is defined as five feet or higher from the finished floor. The item was moved to a cabinet on a shelf five (5) feet from the floor. Review “Keep out of Reach” items with staff and how to store them properly. Prescription Medication – To verify that prescription medication supplied to you from parents is for the child using the medication, the parent needs to provide the medication in the original container or box with the prescription attached. The prescription box for an Albuterol inhaler used by a preschool child was not available. Ask the parents to provide you with the box with the current prescription label. For your compliance letter, state the date when the box with prescription label is available for the inhaler. Medication Permission Forms – Not having current authorization to administer medication, could create medical or liability issues if there are changes to the child’s medication, dosage, or instructions for administration. In space 3, there was not a medication authorization form on file for Sacrosidase. The medication was logged when it was administered and was in locked storage box. You said you did not realize a medication permission for was not completed. We discussed having the parent complete the authorization form for the medication and providing a copy to add to the medication. Orientation: Employee orientation is the process of introducing newly hired employees to their new workplace. It provides the basic organizational information employees need to feel prepared for their new role within the facility. A teacher who was rehired on January 12, 2026, did not have an updated orientation. A teacher hired on June 2, 2025, orientation was not fully completed or signed by the teacher and administrator. You said that it got away from you and forgot to finish it. A teacher hired on February 3, 2026, documentation of Shaken Baby Head Trauma Policy was dated February 11, 2026. You said the policy was in the packet of information given to her on her first day and you did not know why she did not sign the policy on her first day of employment. It was recommended that employees files be checked monthly to ensure they are in compliance. CPR/First Aid: A teacher’s CPR and First Aid training expired in March 2026, and a current certificate was not on file. You said she took the training with you in March. You received your certificate, but they did not send hers. I suggested that you contact the American Red Cross and request a copy of her certificate. ABCMS: The Division must be notified within five (5) days of when a new employee is hired. This process is done through the ABCMS portal. The Administrator did not notify the Division within five (5) that a new employee was hired on January 12, 2026. Add the employee to your facilities’ ABCMS portal. Reminders Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. FACILITY PROFILE INFORMATION: You verified the phone number, email address and mailing address listed on the facility profile are correct. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. Each group of children were observed having snack in the kitchen at separate times. Snack consisted of cereal, peaches, and milk. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. The fire and sanitation inspections were completed and remains current. The fire inspection was dated for July 16, 2025, and sanitation July 17, 2025. Two violations were observed today. According to the NC Secretary of State's website, this non-profit corporation is current and active as of 6/4/25. Please contact your child care consultant prior to any information change regarding the ownership of this facility. License and contact information were current. Contact your child care consultant to make any changes to address, phone, or email or to request changes to the license. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored thirteen staff records, nine children’s records and all program records. The facility does not offer transportation. Center Observations: Children and teachers were engaged in learning and play activities that included group time and individual free play. On and off rain did not permit outdoor play today. Infants were cared for on their individual schedules which include eating, napping and floor time with materials. Documentation of safe sleep checks was observed. Feeding plans were posted. Toddlers who are cared for in the same classroom had their own play space away from the infants. Morning snack was French toast sticks, banana and milk. Lunch was chicken nuggets, green beans, mashed potatoes, whole grain bread, and milk. Except for the infants, children ate in the cafeteria next to the kitchen. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff medical report was not available in the staff file for an employee hired on June 3, 2024. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A current TB test was not available in the staff file for an employee hired on June 3, 2024. The date of the TB test in the file was October 1, 2020. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). An employee who was required to renew her five-year criminal background check by December 11, 2024 did not renew it until April 3, 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member due to renew First Aid by September 30, 2024 did not renew it until April 29, 2025. According to the NC Secretary of State's website, this non-profit corporation is current and active as of 6/4/25. Contact your child care consultant to make any changes to address, phone, or email or to request changes to the license. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored thirteen staff records, nine children’s records and all program records. The facility does not offer transportation. Center Observations: Children and teachers were engaged in learning and play activities that included group time and individual free play. On and off rain did not permit outdoor play today. Infants were cared for on their individual schedules which include eating, napping and floor time with materials. Documentation of safe sleep checks was observed. Feeding plans were posted. Toddlers who are cared for in the same classroom had their own play space away from the infants. Morning snack was French toast sticks, banana and milk. Lunch was chicken nuggets, green beans, mashed potatoes, whole grain bread, and milk. Except for the infants, children ate in the cafeteria next to the kitchen. I observed and documented the following violations during today’s visit. A staff medical report was not available in the staff file for an employee hired on June 3, 2024. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A current TB test was not available in the staff file for an employee hired on June 3, 2024. The date of the TB test in the file was October 1, 2020. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements using the Child Care Center Requirements, effective 11/1/2024 and Item Numbers Listing, effective 4/2025. I monitored thirteen staff records, nine children’s records and all program records. The facility does not offer transportation. Center Observations: Children and teachers were engaged in learning and play activities that included group time and individual free play. On and off rain did not permit outdoor play today. Infants were cared for on their individual schedules which include eating, napping and floor time with materials. Documentation of safe sleep checks was observed. Feeding plans were posted. Toddlers who are cared for in the same classroom had their own play space away from the infants. Morning snack was French toast sticks, banana and milk. Lunch was chicken nuggets, green beans, mashed potatoes, whole grain bread, and milk. Except for the infants, children ate in the cafeteria next to the kitchen. I observed and documented the following violations during today’s visit. J. Watts, Administrator, assisted me with the visit. Your facility currently operates with a Notice of Compliance issued 5/24/2006. The last annual compliance visit was conducted 8/8/2022. The sanitation inspection was completed 1/9/2023 with a “Superior” classification. The last fire inspection was conducted 7/25/22 and your facility was approved for daytime care. The program’s compliance history was ninety-three percent as of 7/18/2023. This facility is owned by Countryside Church of Christ. According to the NC Secretary of State's website, this non-profit corporation is current and active as of 7/18/2023. Please contact your child care consultant or DCDEE prior to any information changing regarding the ownership of this facility. Upon entering the building, children from space 3(three) were eating lunch in the cafeteria space. Lunch consisted of chicken nuggets, corn and green beans, mashed potatoes, bread, and milk. The children in space 1(one)were resting in their cribs on linen covered mattresses. In space 2(two) the children were transitioning to personal care routines. Children in space 4 were engaged in free play. In space 5a and 5b the children were engaged in free play. Toys were accessible on low-lying open shelving. The outdoor play area consisted of jelly bean rubber mulch, a stationary climber with a slide, a stationary jeep, sand box, picnic tables, shelter, playhouses, a variety of balls, a basketball goal, and a variety of push/pull toys. The following violations were documented: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The kitchen door was left open when the cook was not in the kitchen. These spaces should be locked because they may contain hazardous materials. 10A NCAC 09 .0601(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
