This visit was conducted with Vanessa Dalton, Lead Teacher. Currently this center operates with a Five Star license. Restrictions include daytime care only and meets all enhanced requirements and reduced ratio. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed March 23, 2026. The facility received a superior rating with no demerits. The last fire inspection on file was completed on November 12, 2025. A copy was received today. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Ten (10) children were present during today’s visit. The children were observed playing indoors, completing routine tasks and participating in group time. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed March 27, 2026. It took the children forty-five (45) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on January 23, 2026. Emergency drills must be conducted every three (3) months. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Ms. Dalton stated that no child enrolled has emergency medication. They do not provide routine transportation. The NC Pre-K requirements were monitored today. The facility currently has eight (8) NC Pre-K children. The teacher assistant in the NC Pre-K plan was incorrect. The new teacher assistant began on March 24, 2026. She has an Associate’s Degree in Early Childhood. The following violation was observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The mesh cover for the sandbox is torn in several places. G.S. 110-91(6); .0601(b) Rated License Information: The QRIS modernization plan was discussed during a routine unannounced visit conducted in October. Today we discussed where the facility is in the process. Ms. Dalton is new to the facility. She stated that she is interested in Pathway 2 but will have to see if the administrator will complete the required trainings before she will decide. If she does pathway 1, Program Assessment, I reminded her that the self study must be completed three (3) months before the environment rating scale can be requested. She can begin the self study now and then when school resumes in the fall request the assessment. Consultation: The new teacher assistant must complete the Recognizing and Responding to Child Maltreatment within ninety (90) days of employment. She can find the training at www.preventchildabusenc.org. She already has a current CPR and First Aid training. All other health and safety trainings must be completed within her first year of employment. Compliance: You must correct the violation found during today's visit immediately. Please send me two (2) letters verifying compliance. The letters must be received by April 28, 2026. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. This visit was conducted with Pennialisha Lawson, Long-term lead teacher Sub. Currently this center operates with a Five (5) Star license. The restrictions include daytime care only and meets all enhanced requirements and reduced ratios. This facility is owned and operated by Rockingham County Schools. Ten (10) children were present during today’s visit. I observed the children playing indoors, completing routine tasks and departing. The emergency drill log was monitored during today's visit. The last fire drill recorded was completed on October 22, 2025. The last emergency drill, a lockdown drill, was conducted on September 5, 2025. There are new staff. Both teachers are long-term substitutes. One has been a substitute since August 29, 2025 and the other has been a substitute since September 5, 2025. Neither individual was in the NC Pre-K plan. The NC Pre-K contract administrator was contacted today. When I arrived at the classroom the long-term teacher assistant substitute was alone with seven (7) children. A few minutes later the long term lead teacher substitute came back to the classroom with three (3) children. I asked to see the files for both substitutes. Only the long term lead teacher sub’s file could be found. I looked up the other substitute in the ABCMS system and she has a current qualifying letter. She stated that she does not have a current CPR and First Aid training because hers has expired. The administrator, L. Neal, that is new, does not have a qualifying letter in the ABCMS system either. She cannot be left alone with any of the children or be counted in staff/child ratio until she has a qualifying letter. They do not provide routine transportation for children enrolled this school year. The following violations were cited today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The new administrator does not have a completed criminal background check. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The teacher that was present alone with the children does not have a current first aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The teacher present alone with the children does not have a current CPR training. .1102(d) Rated License: I meet with Amy Rose, Preschool coordinator, on October 6, 2025 to review the QRIS modernization pathways. I completed a template documenting the pathway you plan to pursue when , identifying needs, documenting a timeline for completion, resources, next steps, etc. Over the next few months, I’ll be checking in with you, ahead of your next visit to check on progress and determine if additional assistance is needed with WORKS, NCRLAP/ERS, accreditation, etc. Consultation: I recommend that the teachers review the Division’s website under the What’s New tab and the QRIS modernization tab. There is a rule roll out available on Moodle. Compliance: You must correct the violation found during today's visit immediately. Please send me a two (2) letters verifying compliance. The letters must be received by November 11, 2025. Please include in your letter each violation number and how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Restrictions include daytime care only and meets all enhanced requirements and reduced ratio. This facility is owned and operated by Rockingham County Schools. The sanitation inspection was completed April 10, 2024, with a "Superior" classification with no demerits. The fire inspection was completed on May 6, 2024. A copy must be emailed to me. Upon arrival staff/child ratio was observed and in compliance as indicated on the attached enrollment worksheet. Children were adequately supervised during today’s visit. All indoor and outdoor areas were observed during today's visit. Eight (8) children were present during today’s visit. The children were observed playing indoors, completing routine tasks and eating lunch. Fire drill reports and playground inspections were monitored during today's visit. The last fire drill was completed April 18, 2024. It took the children three (3) minutes and thirty-two (32) seconds to evacuate. The last emergency drill, a lockdown drill, was completed on April 15, 2024. Children’s and staff files were reviewed according to the policy set by the Division of Child Development and Early Education. Hazardous supplies were kept in locked storage. Ms. Simpson stated that no children enrolled have emergency medication. Ms. Simpson stated that they do provide transportation. The NC Pre-K requirements were monitored today. The facility currently has ten (10) NC Pre-K children. The following violation was observed during today's visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. There is a play airplane that has areas of chipping paint. G.S. 110-91(6); .0601(b) Consultation: 1-1-The health and safety trainings are required to be completed within the first year of employment, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) trainings must be completed within ninety (90) days of employment. Following the initial completion of the health and safety trainings they must be completed within the next five years of employment or prior to expiration (CPR and First Aid). There is a training log available on the Division’s website to document the completion of the trainings. All topics must be covered. The Recognizing and Responding to Suspicions of Child Maltreatment training can be found at https://www.preventchildabusenc.org/. 2.
