Name of Operation: FORBUSH OUT-OF-SCHOOL-TIME PROGRAM Facility ID: 99000060 Consultant: JENNIFER DAVIS Operation Type: Center Case Number: Visit Date: 2/24/2026 Number Present: 15 Completed Date: 2/24/2026 Age: From 5 To 11 Total Minutes: 100 Time In: 02:45 PM Time Out: 04:25 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced An unannounced annual compliance visit was conducted at this child care center to monitor compliance with applicable child care requirements and assess for a rated license. This visit was conducted by Jennifer Davis, child care consultant, with Malaya Caudill, on-site director. Currently this program operates with FIVE (5) star license issued 5/10/24. Restrictions include daytime care only; school age children only; meets enhance space; meets enhanced ratios; reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. Upon arrival staff/child ratio was observed and was in compliance as indicated on the attached worksheet. The license was posted and restrictions were met. All indoor and outdoor areas were observed during today's visit. I observed developmentally appropriate materials and activities available to the children in each classroom. I observed children eating snack and having free center play. I observed adequate supervision and staff assisting children as needed. This program is operated by Yadkin County Schools. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 3/6/2025. The last fire inspection was conducted on 9/29/2025. Program records and required postings were monitored. A fire drill was conducted on 2/11/2026. A shelter in place drill was documented on 2/13/2025. Outdoor inspections were current. An inspection was not conducted in January due to two snow events and school was closed. Staff files and children’s records were monitored per DCDEE procedures. Storage of hazardous items was monitored today. I observed one emergency medication. Hazardous supplies are kept in locked storage. I did observed a first aid kit that had 15 packets with at least two warnings that need to be locked. This was corrected during the visit. The outdoor learning environment is not regulated by DCDEE and permission to play on the playground was observed in the children’s records. The program coordinator stated that the center does transport children on field trips during the summer. I observed emergency information. The program uses an activity bus and bus driver provided by YCS. No violations were cited during today's visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A first aid with fifteen(15) packets that had two or more warnings including antiseptic wipes, antibiotic, non-aspirin and cold packets were not locked. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Additional information provided during today’s visit: - The three pathway options for the QRIS are on the DCDEE website under the “What’s New” Tab. Your rated license will be due in May 2027. This visit was conducted by Jennifer Davis, child care consultant with Malaya Caudill, on-site director. Currently this program operates with FIVE (5) star license issued 5/10/24. Restrictions include daytime care only; school age children only; meets enhance space; meets enhanced ratios; reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The license was observed, and the restrictions were met. This program is operated by Yadkin County Schools. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children playing in activity centers. I observed one staff file. Ms. Caudill worked with the out-of-school-time program previously. No medications were reported. Storage hazardous items was monitored today. Program records and required postings were monitored. A playground inspection was recorded 9/10/25. A fire drill was recorded 9/12/25. A shelter in place drill was recorded 8/21/25. The program does not provide routine transportation at this time. They do transport for field trips in the summer. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain 75% compliance. The following violation was cited during the visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not have current First Aid from an approved organization. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member did not have current CPR from an approved training organization. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Ms. Caudill stated that she is signed up to take CPR/First Aid on September 24th. The approved training list is located on our website. You must correct the violation cited during today's visit immediately and send me documentation verifying compliance on or before 10/2/2025. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Additional information provided during today’s visit: - • A new Pathways to the Stars Rules Roll Out module is now available in the DCDEE Moodle digital learning platform. Beginning In October, child care consultants will discuss the new rules during licensing visits. Consultants will review the pathway options, identify facility needs, answer questions, and work with the facility operator to establish an individualized timeline for transition to a new rated license within 12 months based on the pathway chosen. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
