Name of Operation: FALL CREEK OUT-OF-SCHOOL-TIME PROGRAM Facility ID: 99000062 Consultant: JENNIFER DAVIS Operation Type: Center Case Number: Visit Date: 12/6/2023 Number Present: 28 Completed Date: 12/6/2023 Age: From 5 To 11 Total Minutes: 90 Time In: 02:30 PM Time Out: 04:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable childcare requirements for a routine unannounced visit. Amanda Sizemore, on-site director assisted me with the visit. Casey Brown also assisted with the visit. Currently the program operates with a FIVE (5) Star License issued 8/14/17. License restrictions include daytime care only; school age children only; meets enhanced ratios and space, reduced group size by one in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The program is operated by Yadkin County Schools. The program’s compliance history was 97% prior to today’s visit. Licensed programs are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least 75%. Any violation(s) documented may impact the compliance history score. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were observed in compliance. A walk-through of the facility was completed today. Indoor areas were observed. The outdoor area was not observed due to rain. The outdoor play area is not monitored by DCDEE. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having snack and outside for gross motor play. No new staff were reported. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. There are currently no children on any medications. Storage hazardous items was monitored today. A playground inspection was recorded 11/30/23. A fire drill was recorded 11/23/23. A shelter-in-place drill for the facility was recorded 11/23/23. The most recent fire inspection was on 9/13/23. I observed a sanitation inspection was last conducted on 5/17/23 earning a superior classification. The program does not currently provide transportation. The following violation was cited during the visit today: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. There was a red and green pillow in Rm.13 that were torn and need to be replaced. In Room 16, there was a broken handle on a cabinet in the kitchen center. G.S. 110-91(6); .0601(b) Child Care programs are expected to achieve and maintain compliance at all times. The violations were corrected during the visit. The pillows were removed and the broken handle was removed during the visit today. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements.
