This visit was conducted by Jennifer Davis, child care consultant, with Danielle Bush, lead teacher. Currently this program operates with a FOUR (4) star license re-issued 8/21/21. License restrictions include daytime care only; meets enhanced ratios and space, enhanced ratios minus one. This program is operated by Yadkin County Schools. There is one NCPK classroom on-site with enrollment. The license was posted, and restrictions were met. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 2/17/25. A sanitation inspection was completed on 10/3/2025 with a Superior classification. A fire inspection was conducted on 9/10/2025. All indoor and outdoor areas were observed and monitored during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed the children playing in centers during the visit. I observed positive interactions between staff and children. Storage of hazardous items was monitored. No medications were reported nor observed. The program does provide transportation for limited children at this time. DPI offsite verification forms for transportation (7/30/2025) was observed. Program records and required postings were monitored. A fire drill was documented on 1/23/26. A lockdown drill was documented on 12/18/25. A playground inspection was documented on 1/12/26. Staff records were monitored per DCDEE procedure. Children’s records were monitored per DCDEE procedures. DPI offsite verification forms for staff records and children’s records were observed. NCPK Requirements: Room 108A is designated NCPK. Fourteen (14) children are designated NCPK. Staff Education Administrator: Emily Goins - Adm. Level 3; MA/MS Reading w/ PL Lead Teacher: Danielle Bush - BS in Bus. Management w/ BK License Teacher: Emma Sorrell– Assoc Degree in Gen. Ed./Teacher Prep, Enrolled in 4 yr education program The program's instructional day is Monday-Friday, 7:30 a.m. to 2:30 p.m. Attendance is taken daily and if a child is absent for three consecutive days the teachers stated that a teacher or administration will contact the parent. Teacher reported examples of family engagement include open house, take home family activities, parent conferences when requested, various volunteer opportunities in the classroom, daily take home folders and DOJO app. Facility Requirements: The center currently holds a 4 Star License re-issued 8/21/21. An ECERS-R assessment was conducted in the NCPK class on 11/26/18 and the score was 6.16. You must maintain all requirements at all times. Child Health Assessment: Records of children enrolled in the NCPK classroom were reviewed today. One child needed a hearing screening. All other files were observed to contain health assessments which included physical examinations, updated immunizations, and vision, hearing and dental screenings. For any on-site screenings held at the program, the screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). The results are shared via verbal/written communication to the parents. Developmental Screening: All children’s files reviewed today contained developmental screenings (Dial 4 Assessment). Children were confirmed to have had a developmental screening administered within 90 days of the first day of attendance in the program or 6 months prior. The teachers stated that developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teacher stated Creative Curriculum and NC Foundations are used in the NCPK program. During the observation, children were engaged in free play in the activity centers throughout the room and a group activity. Throughout the classrooms, items were in activity areas were consistent with the lesson plan. Formative Assessments: The teacher stated that the formative assessment the center is using is Teaching Strategies Gold. The teachers stated that assessment results were shared with the parents and conferences were optional. Teachers reported that an additional assessment will be completed by the end of the school year and parent conference will be offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for the classroom. A lead teacher and teacher are assigned to the classroom. Instructional Staff Standards (Rule .3015): Instructional staff work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program provides adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. These related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. Teachers stated that a school-wide open house was conducted at the beginning of the school year and parents attended. Parents are encouraged to participate in classroom events. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on a training log and kept in each staff’s file. Staff training was monitored today. The following violation was cited during the visit: Violation Number Comment Rule 9995 15A NCAC 18A .2820 STORAGE(g) Purses and other personal effects belonging to child care center employees shall be kept inaccessible to children and shall be stored in accordance with this Rule, as applicable. Two staff purses were accessible to children. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation was corrected during the visit and staff purses were moved so they were inaccessible to children. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. License restrictions include daytime care only; meets enhanced ratios and space, enhanced ratios minus one. This program is operated by Yadkin County Schools. There is one NCPK classroom on-site with enrollment. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center free play, and routine care. NO new staff were reported. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. There were no medications reported. Storage hazardous items was monitored today. A playground inspection was recorded 8/12/2025. A fire drill was recorded 8/22/202. An emergency drill has not been conducted since February 2025. The most recent fire inspection was on 9/10/2025. I observed a sanitation inspection was last conducted on 9/6/2024 earning a Superior classification. The following violations were cited during today’s visit: Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. An annual sanitation has not been completed. The last inspection was dated 9/6/2024. 10A NCAC 09 .0304(b) 807 A safe indoor and outdoor environment was not provided for the children. There were three(3) bolts and two wheels on the bottom of the shelter that were sticking up and were a tripping hazard. 10A NCAC 09 .0601(a) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 2/22/24. A sanitation inspection was completed on 9/6/24 with a Superior classification. A fire inspection was conducted on 9/10/2024. All indoor and outdoor areas were observed and monitored during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed the children in group time and playing in centers during the visit. I observed positive interactions between staff and children. Storage of hazardous items was monitored. No medications were reported nor observed. The program does provide transportation for limited children at this time. DPI offsite verification forms for transportation (8/13/24) was observed. Program records and required postings were monitored. A fire drill was documented on 1/13/25. A lockdown drill was documented on 10/1/24. A playground inspection was documented on 1/13/25. All other files were observed to contain health assessments which included physical examinations, updated immunizations, and vision, hearing and dental screenings. For any on-site screenings held at the program, the screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). This visit was conducted by Jennifer Davis, child care consultant, with Taylor Medford, lead teacher. Currently this program operates with a FOUR (4) star license re-issued 8/21/21. License restrictions include daytime care only; meets enhanced ratios and space, enhanced ratios minus one. This program is operated by Yadkin County Schools. There is one NCPK classroom on-site with enrollment. The license was posted, and restrictions were met. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. The last annual compliance visit was conducted on 3/28/23. A sanitation inspection was completed on 12/5/23 with a Superior classification. A fire inspection was conducted on 9/11/23. All indoor and outdoor areas were observed and monitored during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed the children playing in centers during the visit. I observed positive interactions between staff and children. Storage of hazardous items was monitored. No medications were reported nor observed. The program does provide transportation for limited children at this time. DPI offsite verification forms for transportation (8/16/23) was observed. Program records and required postings were monitored. A fire drill was documented on 1/30/24. A shelter in place drill was documented on, 11/28/23. A playground inspection was documented on 2/2/24. Staff records were monitored per DCDEE procedure. Children’s records were monitored per DCDEE procedures. DPI offsite verification forms for staff records (8/16/23) and children’s records (8/16/23) were observed. NCPK Requirements: Room 108A is designated NCPK. Eleven (11) children are designated NCPK. Staff Education Administrator: Emily Goins - Adm. Level 3; MA/MS Reading w/ PL Lead Teacher: Taylor Medford - BA/BS Ch Dev w/ BK License Teacher: Belinda Pendry – NCECC w/ 2 years ECE experience (NCLB exception), enrolled AAS/ECE Qualifications and credentials were reviewed in the NCPK Plan prior to today's visit. The program's instructional day is Monday-Friday, 7:30 a.m. to 2:30 p.m. Attendance is taken daily and if a child is absent for three consecutive days the teachers stated that a teacher or administration will contact the parent. Teacher reported examples of family engagement include open house, take home family projects, parent conferences when requested, various volunteer opportunities in the classroom, daily take home folders and DOJO app. Facility Requirements: The center currently holds a 4 Star License re-issued 8/21/21. An ECERS-R assessment was conducted in the NCPK class on 11/26/18 and the score was 6.16. You must maintain all requirements at all times. Child Health Assessment: Records of children enrolled in the NCPK classroom were reviewed today. Files were observed to contain health assessments which included physical examinations, updated immunizations, and vision, hearing and dental screenings. For any on-site screenings held at the program, the screening agency and/or program representative reviewed all health assessment results and shared the results with families, as required by Rule .3005(b). The results are shared via verbal/written communication to the parents. Developmental Screening: All children’s files reviewed today contained developmental screenings (Dial 4 Assessment). Children were confirmed to have had a developmental screening administered within 90 days of the first day of attendance in the program or 6 months prior. The teachers stated that developmental screening results were shared with families by the child’s teacher as required (Rule .3006(b), via verbal/written communication with parents. Curricula: The teacher stated Creative Curriculum and NC Foundations are used in the NCPK program. During the observation, children were engaged in free play in the activity centers throughout the room and a group activity. Throughout the classrooms, items were in activity areas were consistent with the lesson plan. Formative Assessments: The teacher stated that the formative assessment the center is using is Teaching Strategies Gold. The teachers stated that assessment results were shared with the parents and conferences were optional. Teachers reported that an additional assessment will be completed by the end of the school year and parent conference will be offered. Staff-to-Child Ratio and Class Size: The class size is 18 children or less and the staff/child ratio is 1:9 or less for the classroom. A lead teacher and teacher are assigned to the classroom. Instructional Staff Standards (Rule .3015): Instructional staff work in direct contact with children in the Pre-K program for at least a 32.5-hour work week. In addition to these direct, day-to-day instructional experiences, the Pre-K program provides adequate additional time for the instructional staff for related instructional activities, including time for planning, scheduling and conducting home visits, meeting with children's families, or attending required professional development activities. These related activities shall take place outside of the six-and-a-half-hour day of direct teacher-child contact. Planning time occurs after the instructional day or during the day when opportunities allow. Teachers stated that a school-wide open house was conducted at the beginning of the school year and parents attended. Professional Development Requirements (Rule .3016) Professional Development opportunities such as training are logged on a training log and kept in each staff’s file. Staff training was monitored today. The following violations were cited during the visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The two front footings on the shade structure were exposed. The footings had two bolts and a hook sticking up. The back gate on the playground does not stay closed securely. 10A NCAC 09 .0601(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One substitute staff did not have acknowledgement on file that the policy was reviewed prior to providing care with children. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. COMPLIANCE DOCUMENTATION: You must correct the violation cited during today's visit immediately and send me documentation verifying compliance on or before 3/7/24. Technical assistance provided during today’s visit: • The prevention of shaken baby syndrome policy should be reviewed by staff prior to the first day of work. An acknowledgement that it was received should be on file for each staff member. • The footings on the shade structure should be covered so they are not exposed causing injury. The gate on the back of the playground needs to close and stay closed securely. Ms. Medford stated that are getting a new lock and maintenance will work on the gate so it will be secure. She stated she positions herself in front of it during playground time. • Your facility is in Cohort 1 for the rated license. This visit was conducted by Jennifer Davis, child care consultant, with Taylor Medford, lead teacher. Currently this program operates with a FOUR (4) star license re-issued 8/21/21. License restrictions include daytime care only; meets enhanced ratios and space, enhanced ratios minus one. This program is operated by Yadkin County Schools. There is one NCPK classroom on-site with enrollment. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children in center play and working directly with staff members. No new staff were reported. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. There were no medications reported. Storage hazardous items was monitored today. A playground inspection was recorded 9/1/2023. A fire drill was recorded 8/22/2023. A lockdown drill for the facility was recorded 9/5/2023. The most recent fire inspection was on 10/5/2022. I observed a sanitation inspection was last conducted on 4/11/2023 earning a Superior classification. The following violation was cited during today’s visit: Violation Number Comment Rule 9995 15A NCAC 18A .2832 OUTDOOR LEARNING ENVIRONMENT AND PREMISES (a) At child care centers, the premises, including the outdoor learning environment, shall be kept clean, drained to minimize standing water, free of litter and hazardous materials, and maintained in a manner which does not encourage the harborage of vermin.The outdoor play equipment was visibly dirty and needs to be cleaned. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violations were corrected during the visit today.
