Upon arrival I was greeted by Joanne Noel, the Director. There was a total of forty-nine (49) children present, aging in range from infants to four (4) years old. The children were observed playing in centers, playing outside, preparing for lunch and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued March 27, 2026 through NAEYC accreditation. INSPECTIONS *The last fire drill was conducted 5/29/26. *The last emergency drill was conducted 6/16/26. *The last playground inspection was completed 5/28/26. *The last sanitation was completed on 3/3/26 with an Approved’ classification. *The last fire inspection was conducted on 5/27/26. MONITORING During today’s visit, a partial assessment was conducted, monitoring all spaces occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled, therefore safe sleep requirements were monitored. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. It was observed in space #5 two bottles with a date of 6/16/26. Food brought from home for a child was observed not labeled. In space #6 four bottles were observed without a current labeled date. 15A NCAC 18A .2804(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Off Deep Woods insect repellant was observed without a parent authorization form and instructions in space #2. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Think Baby Sunscreen was observed with an expiration date of 5/2026 in space #1. .0803(12) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before July 1, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. NON-PRESCRIPTION/PRESCRIPTION I observed expired Think Baby Sunscreen in space #1 with an expiration date of 5/2026. In space #2 Off Deep Woods insect repellant was observed with no parent authorization form. I would recommend checking all creams weekly to ensure that all forms are in compliance. I would also recommend sending a memo out to the parents reminding them to complete parent authorization forms when needing to leave cream. INFANT BOTTLES I observed two bottles and food brought from home in space #5 without a current date. The two bottles had the date of 6/16/26. I in space #6 four bottles were observed without a current date. I would recommend reminding the staff to label bottles if they are not labeled when brought from home. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you completed the training in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Joanne Noel, the Director. There was a total of fifty-nine (59) children present, aging in range from infants to four (4) years old. The children were observed playing in centers, playing outside, preparing for lunch and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued April 5, 2024, to August 23, 2026 earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility Church, Yates Baptist listed as current/active. INSPECTIONS *The last fire drill was conducted 12/23/25. *The last emergency drill was conducted 12/23/25. *The last playground inspection was completed 12/23/25. *The last sanitation was completed on 12/2/25 with a ‘Superior’ classification. *The last fire inspection was conducted on 5/29/25. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters, a percentage of old staff and children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled, therefore safe sleep requirements were monitored. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Eucerin cream in space #103 did not have written authorization and instructions from the child's parent. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Calmoseptine ointment was not returned to the parents and it had an expiration date of 11/2025 stored in space #105. .0803(12) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A staff member in space #106 stored a coca cola soda on the desk in reach of children. .0901(i) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was not updated with the current administrators information. .0607(e) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. EMERGENCY PREPAREDNESS AND RESPONSE PLAN The EPR plan was observed not to be updated with current information. I would recommend updating when things change as well as annually. The administrator stated that the Assistant Director received access to the EPR plan portal today and will be updating the information. NON-PRESCRIPTION/PRESCRIPTION I observed expired Eucerin cream in space #103 without the parent authorization form. The Administrator spoke with the teacher, reminding the teacher to make sure a parent authorization form is collected before administration. In space #105 Calmoseptine ointment was observed with an expiration date of 11/25. I would recommend checking all creams weekly to ensure that all forms are in compliance. I would also recommend sending a memo out to the parents reminding them to complete parent authorization forms when needing to leave diaper cream. NUTRITION In space #106 a Coca Cola soda was observed on the desk in reach of the children. The teacher stated that she had just come from break and did not put the drink up. It is important to model appropriate eating behaviors by consuming food and beverages that meet the nutritional requirements. I would recommend that you review with all your staff the appropriate nutritional requirements to model. A good resource to use is Caring For Our Children regarding Nutrition and Food Service at http://nrckids.org/CFOC. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS Modernization The Administrator will be completing the Accreditation and Headstart Pathway. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. Upon arrival I was greeted by Joanne Noel, the Director. I completed a general walk-through of the center. At the time of the visit, there were a total of fifty (50) children present, ranging in age from infant to nine (9) years old. The children were eating lunch, transitioning to rest time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued April 5, 2024, to August 23, 2026, earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility Church, Yates Baptist listed as current/active. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. INSPECTIONS *The last fire drill was conducted on 5/30/25. *The last emergency drill was conducted 2/28/25. *The last playground inspection was completed on 5/30/25. *The last sanitation was completed on 4/28/25 with a “Superior" classification. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In space #105 was observed with an expiration date of 4/2025. .0803(12) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Safe sleep check documentation was not consistently completed. I observed a child having documentation last completed on 6/10/25. The child attended 6/18, 6/19, 6/20, 6/24, and 6/25. .0606(g) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space #106 a chick-fil-a cup, one bottle, and 2 cans of coca cola soda were observed in view and reach of the children. .0901(i) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place/ lockdown drill was conducted on 2/28/25. .0604(u);.0302(d)(8) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. DISCIPLINE Upon arrival I could hear the teacher in space #106 yelling at a child stating, take your feet off the table, I have told you for the last two days to take your feet off the table.’ While doing the walk through the teacher in space #106 yelled at a child stating, ‘get off of her.’ When completing the walk through the teacher in the same space told a child to ‘get up off the bed now.’ The child asked why, the teacher stated because you are not listening. I discussed with the Director that the teacher seemed to be very frustrated. The Director stated that she has some challenging behaviors in the classroom that may be causing frustration with the teacher. The Director stated that the teacher completed the conscious discipline training two weeks ago. It is important to use positive discipline methods to guide children and support the development of their emotional regulation, healthy social behavior, and problem-solving skills. A good resource for methods of positive behavior support is Caring for our Children (https://nrckids.org/CFOC). I would also recommend that you contact Early Years to contact a Healthy Social Behavioral specialist to assist with challenging behaviors and to receive training at 919.403.6950. It is also important to review with all staff the childcare rules concerning discipline (.1803, .1804, and .2102) with your staff, paying particular attention to the rule that states that discipline may not be related to food, rest, or toileting. NUTRITION In space #106 I observed a Chick-fila cup, two cans and one bottle of coca cola soda in reach and view of the children. It is important that staff model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements in the Meal patterns. I would recommend that you review with all staff the importance of modeling the appropriate meal patterns. A good resource that you can use is Caring For Our Children Chapter 4 regarding Nutrition and Food Service at https://nrckids.org/CFOC. SAFETY AND HEALTH In space #106 a purse was stored in an unlocked cabinet in reach of the children. The teacher was unaware that her purse needed to be locked. I would recommend that you discuss with all staff that purses and hazardous items needing to be locked. MEDICATION In space #105 an EpiPen was observed with an expiration date of 4/2025. The Director and teachers were unaware that the EpiPen was expired. I would recommend checking all medications monthly to ensure that medications are not expired along with the forms. EMERGENCY DRILLS The emergency drill was last conducted on 2/28/25. The Director thought the emergency drills were to be conducted three times a year. I discussed with her that the emergency drill should be conducted every three months. I would recommend setting a reminder on your outlook calendar to alert you when the emergency drill should be conducted. SAFE SLEEP The safe sleep chart was not consistently completed. One child safe sleep chart was last completed on 6/10/25. The child attended 6/18, 6/19, 6/20, 6/24, and 6/25 and did not have any documentation of sleeping. The teachers stated that when support teachers relieve them, they don’t document on the safe sleep chart. The director also stated that the support teachers are also available to hold the children when the child is sleeping. I would recommend that you retrain all teachers who assist in the infant room concerning safe sleep documentation every fifteen minutes. I would also have the teachers and support teachers to refresh on ITS-SIDS. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. Upon arrival I was greeted by Ms. Vanessa, the Office Manager. The Director, Joanne Noel, assisted me with the visit. There was a total of sixty-eight (68) children present, aging in range from infants to five (5) years old. The children were observed playing in centers, playing outside, participating in soccer shots, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued April 5, 2024, to August 23, 2026, earning five (5) points in staff education, seven (7) points in program standards and one (1) quality point. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. SAFETY AND HEALTH During today’s visit I observed the Pre-K classroom from space #105 in the gym participating in Soccer Shots. I observed two children not participating in soccer shots playing on the far-left side of the gym. The teachers were sitting on the bench watching the children participate in soccer shots. The other two children were observed flipping over a couch, playing on unsafe materials. One teacher went over to the children when I mentioned that the children were flipping over the couch. I discussed with the Director and the teacher’s options that can be used to assist in keeping them engaged and safe while the other children participate in soccer shots. The gym stored a structure that has nails sticking out, broken chairs, wooden boards with protrusions, old couches, and an assortment of other broken items. The Director discussed with me that at the church meeting they talked about the gym needing to be cleaned out removing all old furniture, and hazardous materials. I would recommend having all hazardous items removed out of the gym. I would also recommend discussing with all staff how to keep children engaged and properly supervised during extracurricular activities. SUPERVISION Upon entering into the gym, I observed two children playing on the far-left side of the gym. The children were flipping over a couch. The two teachers were sitting on the bleachers on the far-right side of the gym watching the children participate in soccer shots. One teacher was observed taking a video of the children participating. I mentioned that the two children were flipping over the couch to the director. One teacher then went over to the children. I would highly recommend going over your supervision policy with all staff. Adequate supervision for all children is important for the safety of each child daily. The director, June Shillito, assisted me in today’s visit. Upon arrival, you greeted me and welcomed me into your program. Six (6) classrooms were opened today. I conducted a general walk-through of the indoor and outdoor learning environment. I observed the children having free playing in their outdoor learning environment. Some of the other children were exploring different centers in their indoor learning environments and preparing for lunch time. The younger babies have some floor time with the teachers while getting hugs and exploring their indoor environment. RATED LICENSE STATUS The center holds a 5 star-rated license issued on 8/23/23. The program received 5 points in Staff Education, 7 points in Program Standards and 1 Quality point through the programmatic option of having a staff benefits package and an infrastructure for parent involvement. INSPECTIONS The last sanitation inspection was conducted on 6/27/23. The last fire inspection was conducted on 5/18/23. The last fire drill was conducted on 12/20/23. The last emergency drill was conducted on 12/8/23. The last playground inspection was conducted on 12/18/23. According to the NC Secretary of State website, the owner status of Yates Baptist Church, is current and active. During this visit, a full assessment of Child Care Requirements was conducted. The program does not provide transportation, so these requirements were not monitored. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. Cots in space #1, space #5, and space #6 were not stored properly to avoid contamination of the children's sheets when not in use. 15A NCAC 18A .2821(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space #5 a can of mosquito repellent was not stored in a locked room or cabinet. .2820(b) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. I would suggest doing a walk-through every morning to make sure all hazardous items are stored and locked in a cabinet out of the reach of children. I would suggest making sure you double check all medications brought in by parents. Paying close attention to prescription medications by making sure the medication is not only in the original container but also having all the correct forms filled out. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year January 24, 2023 -January 23, 2024. CONSULTATION Criminal background checks must be renewed every five (5) years. Criminal background checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge.
