Upon arrival I was greeted by Latoya McCrimmon. You assisted me with the visit. I completed a general walk-through of the center. At the time of the visit, there were a total of Eighty-one (81) children present, ranging in age from three (3) to five (5) years old. The children were observed interacting with the teachers, dancing, eating lunch, preparing for rest time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued November 29, 2018, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility, Durham Public Schools listed as current/active. INSPECTIONS *The last fire drill was conducted on 3/23/2026. *The last emergency drill was conducted 3/5/2026. *The last playground inspection was completed 2/26/2026. *The last sanitation was completed on 8/5/2026 with a “Superior ‘’ classification. *The fire inspection was completed on 9/24/2025. The inspection report stated that the inspector will return on or after 10/24/2025 to inspect the violations. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. 10% of old staff, all new staff CPR/FA, qualification letters, and 10% of enrolled children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. In space #1 it was observed a fan in use not mounted and without a mesh covering guard. 10A NCAC 09 .0604(d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before April 9, 2026, the letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. SAFETY HAZARD It was observed in space #1 that with an electric fan not mounted or with a mesh covering. Space #1 cares for children ages three to five years of age. The fan was in reach of the children and was on. I discussed with the administrator and the teacher the importance of having the fan mounted and/or with a mesh covering. Having a mesh covering can prevent any injuries occurring pertaining to the fan. I would encourage you to send out a memo reminding all teachers of making sure the fans are mounted with a mesh covering out of the reach of children before using. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 95%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. The Principal, Latoya McCrimmon, assisted me with today’s visit. There was a total of sixty-six (66) children present, aging in range from three (3) to five (5) years old. The children were observed resting during nap time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued November 29, 2018, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 11/25/24. *The last emergency drill was conducted 11/19/24. *The last playground inspection was completed 11/25/24. *The last sanitation was completed on with a ‘Superior’ classification. *The last fire inspection was conducted on 9/16/24. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violations were observed and documented during today’s visit Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. During today's visit a personal bag/purse was not stored in a locked cabinet in space #4 and space #5. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #4 Lysol wipes were stored in an unlocked cabinet in reach of children. .2820(b) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. PERSONAL BAGS/PURSES During today’s visit I observed in space #4 and #5 personal bags/purses not in a locked cabinet. In space 4 the cabinet would not lock, and a personal bag/purse was stored in the cabinet. In space #5 a personal bag was hanging on a chair. I would recommend always storing all personal bags/purses in a locked cabinet. HAZARDOUS ITEMS During today’s visit I observed Lysol wipes being stored in an unlocked cabinet in space #4. The teacher stated that a work order has been placed to have the cabinet fixed. I would suggest using an alternate type of lock to ensure that you maintain compliance. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. The director, Dr. LaToya McCrimmon, assisted me with today's visit. Upon arrival, you greeted me and welcomed me into your program. I observed eight (8) classrooms of children during today's visit. The children were observed eating lunch, bathroom breaks, and napping. Also, I observed the children preparing to depart the school during dismissal. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 11/29/18. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 6 points in Staff Education, and 1 quality point through the programmatic options of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last fire drill was conducted on 3/22/24. The last emergency drill was conducted on 2/8/24. The last playground inspection was conducted on 4/16/24. The last sanitation inspection was conducted on 9/14/23. The fire inspection was conducted on 2/16/23. This program is owned by Durham Public Schools. During this visit, a full assessment of Child Care Requirements was conducted. The center does not enroll children under 3 years of age, so I did not assess safe sleep records or feeding schedules. Also, I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A current fire inspection was not available for review during today's visit. The last fire inspection was February 16, 2023. 10A NCAC 09 .0304(a) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. A child in space #8 did not wash hands between finishing lunch and standing in line for the restroom. 15A NCAC 18A .2803(c) 807 A safe indoor and outdoor environment was not provided for the children. In space #1 bleach solution and hand sanitizer was not stored 5ft high out and out of the children's reach at all times. 10A NCAC 09 .0601(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One employee did not have a current first aid certification on file for review during today's visit. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. An employee did not have a current CPR certification on file for review during today's visit. .1102(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Four employees did not complete the required on-going training hours with their annual training year. .1103(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. An employee in space #1 did not complete recognizing and responding to Suspicions of Child Maltreatment training within 90 days of hire. .1102(g) 9999 Water bottles were not labeled with the name of the child to whom the water bottle belongs, individually stored in the child's cubby, and sent home with the child at the end of the day. In space #1 four children’s water bottles were not labeled with the child’s name. This is a violation of a rule in NC Sanitation Requirement 15A NCAC 18A .2804(i). During this visit, a full assessment of Child Care Requirements was conducted. The center does not enroll children under 3 years of age, so I did not assess safe sleep records or feeding schedules. I would suggest reviewing hand washing with all staff and expressing the importance of when and why it must be done at those times. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year April 25, 2023 -April 18, 2024.
