Upon arrival, I was greeted by the lead teacher, Gina Allen, who assisted me during today’s visit. The director, Jennifer Lacewell, arrived approximately 1.5 hours later and helped me from that point on. LICENSE STATUS The center currently operates with a Five Star License issued on May 2, 2025. The program achieved its star rating through NAEYC accreditation. The center is licensed to serve 34 children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, White Rock Baptist Church, is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 12/10/25, and the center scored a superior rating with four demerits. -The last fire inspection was completed on 4/28/25. -The last documented outdoor play area inspection was completed on 1/14/26. -The last documented fire drill was conducted on 1/7/26. -The last documented lockdown/shelter-in-place drill was conducted on 12/17/25. -The last annual compliance visit was conducted on 3/6/25. MONITORING I completed a walk-through of the center, visiting two NC Pre-K/Durham Pre-K classrooms as well as the outdoor learning environment, kitchen, and bathrooms. Preschoolers were observed participating in circle time, learning about the Olympics, dancing, and freely playing in learning centers. The themes of the two classrooms’ activity plans this week are “Clothes” and “Community Helpers.” The classrooms follow the Durham Pre-K required ratio of 1:9. The outdoor learning environment used by students is a fenced playground with climbing equipment, swings, a sandbox with sand toys, a music wall, and outdoor centers such as blocks and books. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Three staff and four children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. I reviewed the storage of medications as well as corresponding medication permission forms and found them in compliance. The center does not serve children under one year of age, thus, safe sleep requirements were not monitored. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One teacher's qualification letter expired on 12/3/25, and another teacher's qualification letter expired on 1/7/26. G.S. 110-90.2(b) & .2703(n)&(o) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Several staff members' First Aid certification expired on 11/13/25. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Several staff members' CPR certification expired on 11/13/25. .1102(d) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two teachers last completed health and safety training in October 2017. .1103(b) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. No reviewed children's files contained this signed policy. .0608(b)(1-6) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by March 3, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 95%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for all providers should be completed every five years. Two staff members have qualifying letters that have expired: G.A. (12/3/25) and K.M. (1/7/26). Both teachers stated that they have appointments tomorrow to obtain their background checks. They have 15 days to obtain new qualifying letters; after that, they will not be able to work in the center until they receive new qualifying letters. You may also contact the CBC Unit via telephone (919) 814-6401 or by email DHHS.CBC.Unit@dhhs.nc.gov if you have questions or need assistance. Providers may request new background checks up to six months in advance of their qualifying letter’s expiration date. -In order to be prepared in case of an emergency medical event, CPR and First Aid certification must be renewed every two years. Several staff members’ certifications have expired. You stated that you will call a trainer today to set up training as soon as possible. Two teachers’ (G.A. and K.M.) training has expired and must be completed now. They can find this training in Moodle on the DCDEE website. Also, “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be completed within 90 days of hire and renewed every five years. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -Parents must review and sign the “Shaken Baby Head Trauma Policy” when enrolling a child in your center. It is important for program staff too share with parents the vulnerabilities of children to help prevent injuries and explain how to properly handle infants and young children. No children's files that were reviewed today contained this signed document. Moving forward, please have parents review and sign this document when enrolling children. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit cleanwaterforuskids.org/carolina for more information. Your water was last tested on 4/24/24 and no mitigation efforts were required. Per sanitation rules, centers are required to test for lead in water every three years. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Child care providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. The program received seven points for Program Standards, seven points for Staff Education, and one quality point. The center is also NAEYC accredited, and their accreditation renewal visit is due this month. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, White Rock Baptist Church, is listed as current/active.
