Upon arrival, I was greeted by the director, Yvette Hawke, who walked me through the center. LICENSE STATUS The center currently operates with a Four Star License issued on November 1, 2019. The program received two points for Program Standards, seven points for Staff Education, and one Quality Point. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Victorious Community Development Corporation, is listed as current/active. INSPECTIONS/DRILLS STATUS -The last documented outdoor play area inspection was completed on 9/18/25. -The last documented fire drill was conducted on 9/16/25. -The last documented lockdown/shelter-in-place drill was conducted on 9/16/25. -The last annual compliance visit was conducted on 2/27/25. MONITORING I completed a walk-through of the center, visiting seven classrooms as well as the outdoor play area. The infants were drinking bottles and having diapers changed. The toddlers were observed sitting at a table singing songs with the teachers while waiting for lunch to be served. Preschoolers were observed playing in the block center with the teachers, transitioning from the playground, and eating lunch. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Staff records for newly hired staff members also were reviewed. The outdoor space is fenced and contains stationary equipment with a mulched surface. The center serves children under one year of age; therefore, safe sleep requirements were reviewed and found in compliance. I reviewed storage of medication as well as permission forms, and violations were cited. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A gate on the toddler playground could be pushed or pulled open when closed and latched. 10A NCAC 09 .0601(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. In the classroom for two-year-old children, diaper creams were not stored at least five feet high. .0604(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An expired epi-pen with an expired permission form was not returned to the parent for replacement. .0803(12) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The administrator's ITS-SIDS training expired on 6/21/25. .1102(f) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Several medication permission forms for diaper cream contained authorization dates that had expired. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. Two staff members working in the school-age room did not have documentation of BSAC training on file. .2510(j) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by October 15, 2025. The letter must address each violation and explain how it has been corrected. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Administrators and teachers who care for infants must renew their ITS-SIDS training every three years. Your certification has expired and needs to be renewed immediately. You can check the Early Years website to sign up for a renewal class. I also recommend that your floater teacher complete this training in case you ever need her to sub in the classroom for infants. - It is vital that all teachers are well-trained in handling emergency situations and providing needed care. CPR and First Aid must be renewed every two years, and one of your staff members working with school-age children (L.W.) does not have documentation of certification on file. You stated that you think she has completed this training but you can’t find the certificate. You can visit the DCDEE website for a list of approved training agencies: https://ncchildcare.ncdhhs.gov/Provider/Training-and-Professional-Development/Health-and-Safety-Trainings/Be-a-Smart-Consumer-of-First-Aid-and-CPR-Training -Basic School-Age Care training is required for all staff who work with school-age children. Neither teacher for your school-age group has this certification on file. You said you thought both had taken this class in the past, but you couldn’t locate their training certificates. In order to provide high-quality, age-appropriate programming, it is important that staff are well-trained to understand the development and care of school-aged children. You can find online BSAC training courses through the Southwestern Child Development Commission website: https://www.swcdcinc.org. -Playgrounds must be secure to prevent children from escaping the premises unnoticed and placing themselves in harm’s way. The gate on your toddler playground can be opened with a light push or pull, and it’s possible for a child to slip through the gate. I recommend securing the gate today with some zip ties until someone from a fence company can make a permanent repair. -It is important to keep medication administration forms current for all emergency medications and diaper creams. Today, one child with an epi-pen in the classroom for three-year-old children had an expired authorization form as well as expired medication. Child care rules state that all expired medication should be returned to the parent (or discarded after 72 hours if the parent fails to retrieve it), and that the medical authorization form must be renewed every six months. A few other children in classrooms for infants and two-year-olds had diaper cream permission forms that had expired. Please ask parents to complete new medication permission forms. I suggest checking medications and permission forms monthly for upcoming expiration dates. -Safety is a top priority when providing care for young children. Today I observed several tubes of diaper cream within reach of children under a changing table in a classroom for two-year-olds, which poses a hazard if they attempt to ingest it. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any potential hazards and placing them at least five feet high. CONSULTATION -We discussed the new licensure pathway options and how to access the QRIS information on the DCDEE website. You stated that you are still considering which pathway to choose. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. TRAINING REMINDERS -Health and safety training must be renewed every five years and can be accessed through Moodle. This includes “Recognizing and Responding to Suspicions of Child Maltreatment,” which can be found on the Positive Childhood Alliance NC website: https://preventchildabusenc.org/online-trainings/ New teachers must complete this training within one year of hire. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your staff members’ education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process. AGENCY UPDATES Please check the Division of Child Development and Early Education’s website frequently for any new policies or rules. Go to https://ncchildcare.ncdhhs.gov/, and click on the “What’s New” tab for all agency updates, including QRIS Modernization. I recommend exploring the new pathway options to obtain a rated license. New QRIS rules are in effect, and the Division is preparing for a gradual transition to the new requirements. Resources, forms, and training will be provided to support you in preparing for new options to earn your Two- through Five-Star Rated License. MONITORING I completed a walk-through of the center, visiting seven classrooms as well as the outdoor play area and kitchen. The infants were being read a story and napping. The toddlers were observed counting and engaging in an activity with their teachers, and children in the two year old classes were playing on the playground and engaging in center play in their classroom. Preschoolers were observed playing freely in centers. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Fourteen staff files and ten children’s files were reviewed. This program provides transportation. This requirement was monitored and one violation was cited. I reviewed the storage of medication and diaper creams as well as the corresponding medication administration forms. One medication was found to be expired and given to the director to follow up with the parents to obtain an up-to-date prescription. The center serves children under one year of age. Safe sleep requirements were monitored and a violation was cited. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. One staff member's qualifying letter expired on 10/29/24. G.S. 110-90.2(b) & .2703(n)&(o) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The provider stated that they had a list of children being transported but no accompanying photo or children's emergency information in the van used for transport. 10A NCAC 09 .1003(d) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. In the event of an accident, it is important to always have emergency information for the children readily accessible to provide appropriate care. Today, the vehicle used to transport children did not contain any of the children’s pictures or emergency medical information. I suggest making copies of the children’s enrollment forms to keep in a file in the van and stapling a photo of each child to their enrollment form. -When caring for infants, it’s important to follow safe sleep practices because the consequences for failing to follow procedures can be life-threatening. One required component of safe sleep policy is to post the policy in the classroom. It was not posted today and should be posted immediately. Another important rule to follow is that teachers who work with infants must be certified in ITS-SIDS training. Today, a teacher who had been employed for less than two weeks and did not have documentation of ITS-SIDS training was left alone in the room while caring for infants. Although she has two months to obtain her ITS-SIDS certification, child care rules state that another teacher who is certified in ITS-SIDS must be present in the classroom at all times. ITS-SIDS training is available through Early Years (formerly CCSA). Visit their website to sign up for a class: https://www.earlyyearsnc.org. You stated that the teacher said she is certified in ITS-SIDS but couldn't locate her certificate. -It is important to keep medication current for emergency situations. Today, one child’s medication for breathing emergencies had expired. Child care rules state that all expired medication should be returned to the parent (or discarded after 72 hours if the parent fails to retrieve it), and that the medical authorization form must be renewed every six months. Please return the medication to the parent today and ask them to send an up-to-date replacement immediately. -Regularly inspecting the building for potential fire hazards should be a priority when caring for young children. It has been more than a year since your last fire inspection. You stated that you will call the fire marshal today to schedule an inspection. I suggest adding a reminder note on your calendar a month before the next inspection is due so you don’t miss this deadline. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Upon arrival, I was greeted by the director, Yvette Hawke, who assisted me during today’s visit. LICENSE STATUS The center currently operates with a four-star license issued on November 1, 2019. The program received seven points for staff education, two points for program standards, and one quality point. It is licensed to serve children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, Victorious Community Development Corporation, is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 2/17/25, and the center scored a superior rating with three demerits. -The last fire inspection was completed on 2/6/24. -The last documented outdoor play area inspection was completed on 2/13/25. -The last documented fire drill was conducted on 2/13/25. -The last documented lockdown/shelter-in-place drill was conducted on 12/10/24. -The last annual compliance visit was conducted on 4/2/24. MONITORING I completed a walk-through of the center, visiting seven classrooms as well as the outdoor play area and kitchen. The infants were being read a story and napping. The toddlers were observed counting and engaging in an activity with their teachers, and children in the two year old classes were playing on the playground and engaging in center play in their classroom. Preschoolers were observed playing freely in centers. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Fourteen staff files and ten children’s files were reviewed. This program provides transportation. This requirement was monitored and one violation was cited. I reviewed the storage of medication and diaper creams as well as the corresponding medication administration forms. One medication was found to be expired and given to the director to follow up with the parents to obtain an up-to-date prescription. The center serves children under one year of age. Safe sleep requirements were monitored and a violation was cited. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued November 1, 2019, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 3/21/24. *The last emergency drill was conducted 3/15/24. *The last playground inspection was completed 3/28/24. *The last sanitation was completed on April 19, 2023 with a ‘Superior’ classification. *The last fire inspection was conducted on 2/6/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violations were observed and documented. The owners want to section off a portion of the right side (facing the stage) to be a preschool classroom for four- and five-year-old children. The facility will use room dividers and furniture to frame the preschool classroom space and the church chair storage section. The framing will be placed in line with the corner wall from the edge of the audio booth and proceed forward toward the stage. Then the framing will continue to the right toward an exterior wall which includes the direct exit to the outside. The School Age portion of the space will be located to the left of the stage. Access to a bathroom is down the hall where space three is located. The newly formed preschool section will accommodate 24 children at 35 square feet per child. Refer to the space measurement calculation worksheet for exact measurement details. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 2. I would recommend sending a memo to parents reminding them that prescription medication is to be kept in the original container. I would recommend a member of management check all prescription and non-prescription medicine for accuracy. INFANT BOTTLES I would recommend the teachers check all bottles for correct labeling of bottles with name and date before placing in the refrigerator. ACTIVITY PLANS Lesson plans should be posted with a current date. I would suggest posting your new lesson plan the Friday before the new week. CPR/FIRST AID CPR and First Aid for four staff members were expired. I would recommend that you use an outlook calendar to remind you of expiration dates. TRANSPORTATION All vehicles must meet all State and Federal laws and regulations before being used to transport children. I would recommend reviewing the North Carolina child Passenger Safety law at www.buckleupnc.org. STAFF FILES I would suggest using the staff checklist file to make sure that all trainings and/or paperwork is completed on or before its due date. FENCE The section of the fence along the walkway from the building leading to the toddler playground was not securely in place. The fence was leaning, with no bar at the top causing protrusions that are hazardous. I would suggest having the fence repaired. BROKEN TOYS Broken toys were on the playground that could potentially be hazardous. I would recommend removing those toys. HAZARDOUS CHEMICALS Sanitizer in the Preschool and preschool bathroom was not 5ft. high. I would recommend moving the Sanitizer to a higher shelf. You discussed with us that the maintenance man would be putting up shelving higher to meet the height requirements. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. Space #7 is the Sanctuary which gets converted weekly into the school age classroom during the school year and summer. The owners want to section off a portion of the right side (facing the stage) to be a preschool classroom for four- and five-year-old children. The facility will use room dividers and furniture to frame the preschool classroom space and the church chair storage section. The framing will be placed in line with the corner wall from the edge of the audio booth and proceed forward toward the stage. Then the framing will continue to the right toward an exterior wall which includes the direct exit to the outside. The School Age portion of the space will be located to the left of the stage. Access to a bathroom is down the hall where space three is located. The newly formed preschool section will accommodate 24 children at 35 square feet per child. Refer to the space measurement calculation worksheet for exact measurement details. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. Upon arrival I was greeted by an Administrator. I completed a general walk-through of the center. The Director Yvette Hawks arrived thirty (30) minutes later. At the time of the visit, there were a total of forty-four (44) children present, ranging in age from infant to five (5) years old and ten (11) staff. The children were observed participating in circle time, playing with materials, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued November 1, 2019, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. INSPECTIONS *The last fire drill was conducted 10/3/23. *The last emergency drill was conducted 9/28/23. *The last playground inspection was completed 10/4/23 *The last sanitation was completed on 4/19/23 with a ‘Superior’ classification. According to the NC Secretary of state website, the status of Victorious Community Development Corporation is “current and active.” MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violation was observed and documented. The school age playground fence measured 3ft. 22 inches with protrusions around the fence. The toddler playground did not measure 4ft and it also had a number of protrusions around the fence. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. FENCE The fence on the toddler playground did not measure 4 feet on the far-right side and the back of the playground. The fence measured 3ft 22 inches. The fence had a number of protrusions around the entire area of the fence. The fence on the school age playground did not measure 4 feet in multiple areas. The fence was also separating from the post at the front entrance of the fence. The fence had a number of protrusions around the fence. The director discussed with me that she has received quotes for the replacement of the fence in case they do not receive the Expansion Grant. I recommended that they purchase more pool noodles to temporarily fix the protrusions after the fence has been repaired to meet the 4 feet requirement.
