The family childcare provider, Faith Wells, assisted me with today’s visit. This documentation was reviewed with Ms. Wells, and a copy of the visit summary documentation was given to her at the conclusion of the visit. Upon arrival, you greeted me and welcomed me into your program. Two children were present today. I conducted a general walk-through of the childcare spaces. I observed the children exploring their indoor learning environment and eating lunch. RATED LICENSE STATUS The program holds a 4-star-rated license issued on 1/24/20. The program received 2 points in Program Standards, 7 points in Staff Education, and 1 quality point by having a BS/BA or higher in Early Childhood/Child Development. During today’s visit we discussed the new QRIS pathways, and you were given an opportunity to ask questions. I advised you to visit the DCDEE website to review the QRIS Modernization requirements and to visit the North Carolina Rated License Assessment Project at www.ncrlap.org for more information about the FCCERS 3. The child care provider is not sure about completing the scales but agreed to look at the different pathway. INSPECTIONS The last fire drill was conducted on 10/1/25. The last emergency drill was conducted on 8/1/25. The last playground inspection was conducted on 10/1/25. During this visit, a full assessment of Child Care Requirements was conducted with the following exceptions. You reported that you do not administer medication. Therefore, these requirements were not monitored. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 908 Health questionnaire was not completed annually. The provider did not have a current questionnaire on file to review during today's visit. .1703(a)(1) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance).The child care provider did not have a current criminal background letter on file for review during today's visit. The last criminal background letter expired on 2/3/25. G.S. 110-90.2 & .2703(n) & (o) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by November 18, 2025. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest putting important dates on the calendar to ensure that dates are not overlooked and out of compliance at any given time. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 98%. It will be updated after today's visit, and you will be informed of the new score. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year November 11, 2024 – November 4, 2025. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Anyone who completed the trainings in 2017, should begin to retake the training now. The provider, Faith Wells was present and assisted me during the visit today. The visit summary was reviewed with the provider and copy was given to her at the conclusion of the visit. Upon arrival I was greeted by the provider and additional caregiver. I completed a general walk through of the areas used by the children and outdoor play area. I observed the two children present down for nap. CURRENT LICENSE STATUS Currently this family child care home (fcch) operates with a four-star license issued January 24, 2020 earning seven (7) points in Staff Education, two (2) points in Program Standards and one (1) Quality point. INSPECTIONS *The last annual compliance visit was conducted on December 8, 2022. *The last fire drill, playground inspection and lockdown drill were all completed on November 1, 2023. INDOOR ENVIRONMENT The bedrooms in the fcch are made inaccessible by closed doors. In the children’s space, there are a variety of manipulatives, pretend play toys and books for the children to explore throughout the day. All activities take place in the childcare space, including meals. The bathroom is located in the childcare space and is equipped with a diaper changing table for those in diapers. *The outdoor play area is in the front yard of the home and is not fenced. There are no sheds on the property. *The provider informed me of the following: -There have not been any incident/accident reports written since my last visit. -There is one pet in the home. -There are no children with allergies. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. The last documented health questionnaire was completed on 6/23/23. .1703(a)(1) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Plastic bags were accessible to the children under the age of three. .1719(a)(18) The two violations cited were corrected during the visit therefore no letter of compliance is needed. COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the family child care home was 92%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE Plastic bags can not be accessible to children under the age of three. The bags must be kept at least five feet out of reach of the children. CONSULTATION ANNUAL COMPLIANCE RECORDS The provider’s file, additional caregiver file, children’s records and program records were reviewed. You have completed the required number of on-going training hours for the year. HEALTH AND SAFETY TRAININGS You have met the requirement of renewing the health and safety trainings every five years as per Child Care Rule .1703(d)(2) as of 8/29/23. RATED LICENSE INFORMATION DCDEE has decided that Rated License assessments will resume, and each program will complete the process based on Co-Hort assignments (1-3). Based on the information I have received you are in Co-Hort 2. Your planning period will start on 7/1/24 and the assessment period begins 7/1/25. Once the process begins remember that you must complete a “self-study” to maintain two points in Program Standards. The self-study can include ways you have made improvements or enhancements to your program over a three-month period. I will also need an updated copy of your operational policies. -You must create a WORKS account. The WORKS portal holds education information. The WORKS portal shows you have existing education in the previous portal however this information must be migrated to WORKS. Using your NCID to create the account, go to the DCDEE website, click the provider tab, then click DCDEE WORKS. -Review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). -Request technical assistance from CCSA and the Child Care Health Consultant if you are interested in the Family Child Care Environment Rating Scales (FCCERS) -Participate in trainings related to the FCCERS-R ANNUAL LICENSING FEE *The Division of Child Development and Early Education is required by law [North Carolina General Statute 110-90(1a)] to establish an annual licensing fee for child care facilities. Your invoice will be emailed, and payment must be made on the DCDEE website by December 31, 2023. Failure to pay the license fee is a violation of child care law (N.C.G.S. 110-90(1a)) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action, up to and including revocation of the child care license.
