Belinda Goode, Owner/Operator, assisted me with the visit. CURRENT LICENSE STATUS Currently this fcch operates with a one-star license issued February 7, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the fcch was completed today, monitoring all areas used by the children indoors and outdoors. There were five preschool children present today. The three preschool children were observed building with legos initially then resting on cots for nap. Infants received care according to individual needs including diapering and bottle feeding. Sleep checks were completed. *The last annual compliance visit was conducted on July 30, 2025. *The last documented fire drill, outdoor inspection, shelter-in-place were all conducted on February 20, 2026. Two violations were observed today and must be corrected immediately. One was corrected during the visit. The violations are as follows: Violation Number Comment Rule 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Ziploc bags with bottle attachments were underneath the unlocked kitchen cabinet accessible to the children under the age of three. .1719(a)(18) 1986 Breast milk, formula, and other bottled beverages sent from home were not fully prepared, dated, and labeled with individual child's names. Bottles were not dated and labeled with the child's names. .1706(l) You are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was 97% as of 3/16/26. The violations documented must be corrected immediately. On or before April 6, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Email the letter to: Kia.Reid@dhhs.nc.gov and include the following information. -Facility name -Facility ID number -Your Signature TECHNICAL ASSISTANCE -A walk-through of the children’s space should occur prior to them arriving each day, removing any potential hazards and placing them at least five feet high or in locked storage. -All bottles must be labeled with the child’s name and date upon arrival. OPERATOR RECORDS Your file was monitored for criminal background qualifying letters, current CPR/First Aid, and other specialized training. On your next annual compliance visit, on-going training hours will be monitored. CONSULTATION CPR AND FIRST AID I verified today that the CPR and First Aid training was completed on 3/21/26 through CPR Certification Durham. Once the card has been received send me a copy. CLEAN WATER FOR CAROLINA KIDS The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. You have enrolled in the program, but no samples have been submitted for testing. Please contact RTI for the next steps 1-888-997-9290. Providers are required to test for lead in drinking water per Child Care Rule .1725. AUTOMATED BACKGROUND CHECK MONITORING SYSTEM North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: •See the real-time background check status of staff members or household members. •Run a printable report of the staff roster to assist with compliance visits. •See new background check applicants and add to staff roster. To get started, complete the ABCMS Child Care Provider Portal Training in Moodle—a video tutorial followed by a few questions. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. -Your health questionnaire will expire on 7/30/26. -Update your professional development plan and EPR plan before your next visit. The provider, Belinda Goode, was present and assisted me when needed. This documentation was reviewed with Ms. Goode, and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Goode. I completed a general walk through of the areas used by the children. There were five (5) preschool children present. The children were observed in center activities, dancing to music, eating lunch, and preparing for nap. CURRENT LICENSE STATUS Currently this family child care home operates with a one-star license issued February 7, 2025. INSPECTIONS *This program was issued a temporary license on August 6, 2024. *The last fire drill and outdoor inspections were both completed on June 30, 2025. *The shelter-in-place drill was completed on May 27, 2025. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. The feeding schedule was not on file for the infant currently enrolled. .1706(i) 919 Accurate records were not maintained for all staff and children. One child currently enrolled did not have the following paperwork on file: an application, receipt of discipline policy, receipt of shaken baby syndrome policy, receipt of summary of the law, receipt of written plan of care on file, pet acknowledgement and receipt of operational policies. G.S. 110-91(9) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. The caregiver did not document compliance with visually checking on the infant currently enrolled. .1724(a)(8)&(f) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by August 13, 2025. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number -Your signature COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the center was 100%. ANNUAL COMPLIANCE RECORDS I monitored the provider's file, five children’s files, and required program records. The required number of on-going training hours for the year have not been completed. You have until 8/6/25 to send me verification of completing 20 training hours. If the information is not received by the date listed a violation will be added to the visit for not completing on-going trainings for the year. TECHNICAL ASSISTANCE *You must observe and record sleep positions for all infants who are sleeping. You might find it helpful to set a timer for 15 minutes when infants are sleeping and reset the timer until all infants are awake. This gives an auditory reminder to check on the infants while they are sleeping. You can also make sure the safe sleep charts are kept near where the infant is sleeping to remind yourself to document sleep positions. A safe sleep environment is essential in reducing the risk of SIDS. I left a copy of the safe sleep chart. *One child present recently enrolled in the program and did not have any paperwork on file. I explained to Ms. Goode that all paperwork except for health assessments and immunizations is due on the 1st day the child is present. You should always have emergency information on file for the child. -I suggested Ms. Goode use the children’s file checklist to ensure all paperwork is in the file. Best practice would be to check the files twice per year to make sure all forms are present. Good record keeping is the key to a successful business. CONSULTATION If medications are administered to the children each medication must have an authorization form on file signed by the parent. The permission for over-the-counter medications is valid for 12 months. The medication authorization for chronic medical conditions is valid for six months. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *The pet vaccination for your dog will expire on 1/28/26. *Your CPR and First Aid training will expire on 3/23/26. *The professional development plan must be completed annually as per Child Care Rule .1703(i). *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you.
