The director, Tammy Sneed, was present and assisted me during the visit. The visit summary was reviewed with the director, assistant director and the owner and copy given at the conclusion of the visit. Upon arrival, you greeted me and welcomed me into your program. I completed a general walk through of nine (9) classrooms and their outdoor learning environment. The children were observed having free play, center activities, exploring their indoor learning environment, eating lunch, and preparing for nap. RATED LICENSE STATUS The program holds a 5-star-rated license that was issued on 5/22/17. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 quality point through the programmatic option of having a staff benefit package and an infrastructure for parent involvement. INSPECTIONS The last fire drill was conducted on 1/5/25. The last emergency drill was conducted on 10/2/25. The last playground inspection was conducted on 12/12/25. The last fire inspection was conducted on 1/13/25. The last sanitation inspection was conducted on 9/25/25. The status of the center's owner, Learning & Beyond, LLC, is current and active on the NC Secretary of State website. During today’s visit, a full assessment of Child Care Requirements was conducted. I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A child in space #8 and space #9 did not have a current date on their bottles. 15A NCAC 18A .2804(d) 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. A child feeding schedule in space #9 did not have a parent signature and date when received. .0902(a) 847 Parent's medication authorization did not include required information. A child in space #9 did not have the proper instructions on the dose that should be administered. 10A NCAC 09 .0803(4)(6-9) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by January 20, 2026. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 96%. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest that you double all medication forms and feeding schedules to ensure all pertinent information is filled out before allowing parents to leave the paperwork and medication. I would suggest making sure teachers are double-checking bottles that are brought in to ensure they are label with the current date on a daily basis. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year January 7, 2025 -January 6, 2026. QRIS INFORMATION During the meeting, we reviewed the three Licensure Pathways and discussed resources available to assist with the transition to the new Rated License Rules. You obtained information on any support that the facility needs during their transition and the option that would be most beneficial for your facility. The facility has decided to complete the COGNIA Accreditation process and has begun the candidacy phrase. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. If you completed the trainings in 2017, you should begin to retake the training. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website: nchildcare.ncdhhs.gov frequently to stay updated on rule requirements. The children were observed having free play, center activities, exploring their indoor learning environment, eating lunch, and preparing for nap. RATED LICENSE STATUS The program holds a 5-star-rated license that was issued on 5/22/17. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 quality point through the programmatic option of having a staff benefit package and an infrastructure for parent involvement. Two children in space #8 and one child in space #9 did not have a parent signature on the child's written feeding plan that was posted in the classroom. .0902(a) 847 Parent's medication authorization did not include required information. The visit summary was reviewed with the director, Tammy Sneed, and a copy was given to her at the conclusion of the visit. Upon arrival I was greeted by Ms. Sneed. We completed a general walk through of nine (9) classrooms and the outdoor play areas. The infants were observed having tummy time, eating lunch, and being fed bottles. The toddlers were observed in indoor and outdoor activities with their teachers. The preschool children were observed in center play and eating lunch. CURRENT LICENSE STATUS Currently this center operates with a five-star license issued May 22, 2017, earning seven (7) points in Staff Education, six (6) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on January 25, 2023. *The sanitation inspection was completed on November 21, 2023, with a "Superior" classification. *The fire inspection was completed on January 18. 2023, and the facility is approved for day time care only. *The last documented fire drill was conducted on January 5, 2024. *The last documented lockdown drill was conducted on December 18, 2023. *The last documented playground inspection was completed on December 7, 2023. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Learning and Beyond, LLC is listed as current/active. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. One child's application did not include this information. .0801(a)(7) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child's medical exam only included the child's weight. GS110-91(1) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. I must receive a letter of compliance by January 25, 2024. The letter must address each violation and explain how it has been corrected. If the letter is not received by the date listed a RETURN visit will be made to the center. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS Ten children’s records were reviewed. The staff and training worksheets were received prior to the visit, and I monitored new staff files and two (2) existing staff files. The on-going training requirements were monitored from January 2023-2024 and was in compliance. The directors signature attests that staff files not monitored today contain all the required information. TECHNICAL ASSISTANCE -When allowing parents or staff to submit paperwork that is not on a DCDEE form, please remember that all information from the DCDEE form must be included. Example: a printout from a child's pediatrician must include all the same information from the DCDEE medical report form. -The children’s application must include at least one person authorized to pick up other than the parents/guardian, in the event of an emergency. -CPR and First Aid training must be completed in person. RATED LICENSE INFORMATION Rated License assessments have resumed, and this program is in Cohort 1. Your planning period started on 7/1/23 and the assessment period begins 7/1/24. The director informed me that the facility is receiving technical assistance from CCSA and would like to complete the Environment Rating Scales (ERS) during the prep year. The director completed the Rated License Request Review form. I reminded the director of the following: -If you do a prep year ERS and are satisfied with your scores, the staff education evaluation could be completed in the assessment year since there will not be the pressure of scheduling or waiting on scores for the ERS. -If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment. Access www.ncrlap.org to review videos and resources in preparation for the scales. Refer to the All About the ITERS and ECERS books also. These books serve as a resource guide which defines what the requirements in the scale mean, and also has definitions of words used in the scale, with descriptions of what to look for and the rules that are used to score. The additional notes have been updated effective 10/25/23. The NCRLAP assessors conduct assessments using the most current notes. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. .1102(d) 1308 Application did not include the names of individuals to whom the center may release the child, as authorized by the person who signs the application. One child's application did not include this information. .0801(a)(7) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child's medical exam only included the child's weight. GS110-91(1) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. I must receive a letter of compliance by January 25, 2024. The letter must address each violation and explain how it has been corrected. If the letter is not received by the date listed a RETURN visit will be made to the center. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS Ten children’s records were reviewed. The staff and training worksheets were received prior to the visit, and I monitored new staff files and two (2) existing staff files. The on-going training requirements were monitored from January 2023-2024 and was in compliance. The directors signature attests that staff files not monitored today contain all the required information. TECHNICAL ASSISTANCE -When allowing parents or staff to submit paperwork that is not on a DCDEE form, please remember that all information from the DCDEE form must be included. Example: a printout from a child's pediatrician must include all the same information from the DCDEE medical report form. -The children’s application must include at least one person authorized to pick up other than the parents/guardian, in the event of an emergency. -CPR and First Aid training must be completed in person. RATED LICENSE INFORMATION Rated License assessments have resumed, and this program is in Cohort 1. Your planning period started on 7/1/23 and the assessment period begins 7/1/24. The director informed me that the facility is receiving technical assistance from CCSA and would like to complete the Environment Rating Scales (ERS) during the prep year. The director completed the Rated License Request Review form. I reminded the director of the following: -If you do a prep year ERS and are satisfied with your scores, the staff education evaluation could be completed in the assessment year since there will not be the pressure of scheduling or waiting on scores for the ERS. -If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment. Access www.ncrlap.org to review videos and resources in preparation for the scales. Refer to the All About the ITERS and ECERS books also. These books serve as a resource guide which defines what the requirements in the scale mean, and also has definitions of words used in the scale, with descriptions of what to look for and the rules that are used to score. The additional notes have been updated effective 10/25/23. The NCRLAP assessors conduct assessments using the most current notes. ADDITIONAL NOTES -Review your ITERS-R and ECERS-R assessments completed in April 2017 with the current staff. -Non-mobile infants must have the materials from all activity areas of the room brought to them or be moved around to each activity area. -A teacher must always be within arm’s reach of the children while they are eating for children under two years of age. -Mouthed toys must be picked up and disposed of before another child touches it. -Check for books that are in poor condition, and do not contain frightening content. -Waiting more than 3 minutes without an activity is too long. -All children should be served water throughout the day. -The children must be away from the areas where staff will spray the sanitizing or disinfecting solution. -The tables and chairs must be the proper height for the children. Their arms can rest comfortably on the tables and their feet are flat on the floor. -All materials in the classroom must be age appropriate for the children in care.
