The family childcare provider, Sherronda Hopkins, assisted me with today’s visit. Upon arrival, you greeted me and welcomed me into your program. Three children were present today. I conducted a general walk-through of the childcare spaces. I observed the children exploring their Indoor learning environment and eating lunch. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 7/15/19. The program is in the “held harmless” status and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 7 points in Staff Education, and 1 quality point by Infant capacity restricted to no more than two children under one year of age. INSPECTIONS The last fire drill was conducted on 1/6/25. The last emergency drill was conducted on 1/7/25. The last playground inspection was conducted on 1/8/25. During this visit, a full assessment of Child Care Requirements was conducted with the following exceptions. You reported that you do not transport children and do not administer medication. Therefore, these requirements were not monitored. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. One child did not have a signed and dated discipline policy on file for review during today visit. G.S.110-91(10); .1727(a)&(b) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. One child did not have a signed copy of the Summary of NC Child Care Law on file for review during today's visit. GS 110-102 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. One child did not have a written plan on file for review during today's visit. .1712(e )(6) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. There were plastic Ziplock bags in the bathroom and in the children learning environment which enrolls children under 3 years of age. .1719(a)(18) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. One child did not have a signed and dated copy of the shaken baby syndrome and abusive head trauma policy on file for review during today visit. .1726(b)&(c) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by January 29, 2025. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest making sure that parents return all paperwork prior to enrollment or on the first day. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 93%. It will be updated after today's visit, and you will be informed of the new score. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year January 16, 2024 – January 15, 2025. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Anyone who completed the trainings in 2017, should begin to retake the training now. Sherronda Hopkins, the provider, was present and assisted me during the visit today. The visit summary was reviewed with Ms. Hopkins and a copy was given to her at the conclusion of the visit. I completed a general walk through of the indoor environment. The outdoor environment was not monitored due to the rain. There were three (3) preschool children present. The infants were observed in tummy and story time activities. The preschool child was observed in center activities and story time with the provider. CURRENT LICENSE STATUS Currently this family child care home operates with a five-star license issued July 15, 2019, earning seven points in Staff Education, seven points in Program Standards and one Quality Point. INSPECTIONS *The last annual compliance visit was conducted on January 23, 2023. *The last documented fire drill was conducted on December 4, 2023. *The last documented lockdown drill was completed on December 4, 2023. *The last documented outdoor inspection was completed on January 8, 2024. INDOOR ENVIRONMENT In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is located in the children’s space and is accessible to those that are potty trained. All activities take place in the child care space. *There are two pets in the home. *The outdoor play area is on the side of the home and is fenced. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1818 Operator did not implement and follow the written safe sleep policy adopted to maintain a safe sleep environment. The wall thermometer in the child care space was broken, therefore the temperature in the child care space could not be monitored. G.S. 110-91(15);.1724(a)(1-12) 1986 Breast milk, formula, and other bottled beverages sent from home were not fully prepared, dated, and labeled with individual child's names. The bottles for the infants were not dated. .1706(l) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by January 30, 2024. The letter must address each violation and explain how it has been corrected. If the letter is not received by the date listed a RETURN visit will be made to the fcch. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the family child care home was 96%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE -All bottles must be labeled and dated with the child’s name. You should ask the parents to complete this task before bringing the bottles in. -Your wall thermometer needs to be replaced to monitor the temperature for the infants. CONSULTATION ON-GOING TRAINING HOURS You have until 1/24/24 to send confirmation of 4 completed training hours. If the trainings are not received by the date listed a violation of not completing on-going training hours for the year will be added to the visit. RATED LICENSE INFORMATION Rated License assessments have resumed, and DCDEE has decided that each program will complete the process based on Cohort assignments (1-3). Your program is in Cohort 2. The planning period (preparation year) will start on 7/1/24 and end on 6/30/25. The assessment period begins 7/1/25 and ends 6/30/26. We discussed the planning (preparation) period and the assessment period for the fcch. -Consider requesting an (Environment Rating Scale) ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways: -ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality if your scores are not what you expected. -If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment. -Use this time to create a WORKS account. You currently have education information that was located in our previous system and has to be migrated to WORKS. To create your account, go to the DCDEE website, click the provider tab, then click DCDEE WORKS.
