Denriqua Thomas, Owner/director, was present and assisted me during the visit. CURRENT LICENSE STATUS Your program currently operates with a three-star license, issued December 20, 2024, earning 4 points in the education component, 2 points in the program standards component (meeting enhanced space) and 1 quality point for having staff benefits package and infrastructure of parent involvement. The license was posted, and the restrictions were not in compliance. *The last annual compliance visit was conducted 12/4/25. *The sanitation inspection was completed 2/16/26 with a “Superior” classification. *The last fire inspection was conducted on 10/28/25, and your facility was approved for daytime and night care only. *The last documented fire drill was conducted on 5/1/26. *The last documented playground inspection and lockdown drill were both completed on 6/3/26. I completed a general walk through of the indoor and outdoor environments. Children throughout the facility were participating in group time, free play in activity areas, personal care routines, and eating lunch. Six violations were observed today and must be corrected immediately. The violations are as follows: Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older.In space #2 there was a child one year of age grouped with children three years of age. 10A NCAC 09 .0713(a)(6) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The mulch has deteriorated on the preschool playground. .0605(j) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. The closet located in space #2 was unlocked with cans of paint inside. .2820(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The First Aid training for one staff expired on 2/18/26. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The CPR training for one staff expired on 2/18/26. .1102(d) 1756 Enhanced staff/child ratios and group sizes were not met. In space #1 there were eleven children ages 3-5 years old with one staff. 10A NCAC 09 .2818 In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by July 8, 2026. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the facility was 96%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. STAFF RECORDS I monitored new staff and existing staff files for criminal background qualifying letters, current CPR/First Aid, and other specialized training. On your next annual compliance visit training hours will be monitored. TECHNICAL ASSISTANCE -Upon my arrival in space #1 the classroom was not maintaining the correct staff child ratios. We discussed. The importance of maintaining staff-child ratios when caring for children is to ensure the health, safety and well-being of every child in your care. When the number of children exceeds a caregiver’s approved ratios, it can lead to: -Reduced supervision and increased risk of accidents or injuries. -Delayed response to children’s needs. -Difficulty providing appropriate care, guidance and interaction. -Increased stress and fatigue for staff which may affect decision making and the quality of care. -Children one year of age cannot be grouped with children three years of age. You should review the enrollment in each classroom and adjust accordingly. A return visit will be made in the near future due to the ratio violations cited. -All staff who provide direct care to children must complete certification in First Aid and CPR appropriate to the ages of children in care. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *You must complete a fire drill before 6/30/26. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you. The Owner/Director, Denriqua Thomas, was present and assisted me during the visit. The visit summary was reviewed with Ms. Thomas and a copy was emailed due to the printer not functioning. Upon arrival, I was greeted by Ms. Thomas. I completed a general walk through of the indoor and outdoor environments. The children were observed in circle time, center activities, and eating lunch. CURRENT LICENSE STATUS Currently this center operates with a three-star license issued June 16, 2024, earning four (4) points in Staff Education, two (2) points in Program Standards and one (1) Quality Point. INSPECTIONS *A temporary license was issued on December 15, 2023. *The sanitation inspection was completed on January 29, 2024, with a "Superior" classification. *The fire inspection was completed on October 28, 2024, and the facility is approved for day time and night care. *The last documented fire drill was conducted on November 11, 2024. *The last documented playground inspection was completed on December 2, 2024. *The last documented lockdown drill was completed on December 2, 2024. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #1 the diazepam medication authorization form expired on 7/11/24. In space #2 the epi-pen medication authorization form expired on 7/11/24 and the epi-pen expired on 11/30/24. 10A NCAC 09 .0803(1)(a & b) 1898 Staff did not complete the health and safety training within one year of employment. Four staff did not complete the health and safety training within one year of employment. .1102(a) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by December 23, 2024. The letter must address each violation and explain how it has been corrected. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE -The medication authorization form for chronic medical conditions is valid for six months and the medical action plan is valid for one year. Best practice is to check medications and forms monthly or keep a chart of medications with expiration dates. -Health and Safety trainings are required to be completed within one year of employment. ANNUAL COMPLIANCE RECORDS Six children’s records, and all staff files were monitored. The on-going training requirements were monitored from December 2023-2024 and was in compliance. -Ms. Thomas stated that there are no infants currently enrolled and no accident/incident reports have been written. You can find more information about each item by accessing www cpsc.gov. Please share with the parents in your program. Ms. Thomas is interested in providing care for 2nd shift. We discussed what is required to have this added to the permit. I have received the fire inspection for approval of daytime and night care and a letter requesting the change with the hours of operation. I will need a copy of the daily schedule that includes 2nd shift and a menu.
