I was accompanied by Angela Alger-Walker, Lead Child Care Consultant. After checking in at the front office, we were directed to Rm. 206, the Pre-K room. We were greeted by Rolanda Smith, the NC Pre-K teacher, who assisted us during today’s visit. LICENSE STATUS The center currently operates with a five-star license issued on December 3, 2018. The program received seven points for staff education, seven points for program standards, and one quality point for having a staff benefits package and infrastructure for parental involvement. They are licensed to serve 18 children on first shift. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 2/21/24, and the center scored a superior rating with zero demerits. -The last fire inspection was completed on 4/1/24. -The last documented outdoor play area inspection was completed on 8/29/24. -The last documented fire drill was conducted on 8/29/24. -The last documented lockdown/shelter-in-place drill date was not available for review in the main office. -The last annual compliance visit was conducted on 9/13/23. MONITORING We completed a walk-through of Pre-K classroom and outdoor play area. Students were observed listening to a story in circle time, transitioning to lunch in the cafeteria, using the restroom and washing hands, playing on the playground, and transitioning to rest time. The playground space used by Pre-K students is a paved courtyard and adjoining grassy area just outside the classroom door. Children can ride tricycles or play with portable gross motor equipment. The activity plan listed this week’s theme as “Classroom Rules and Routines,” Children have a variety of learning centers with materials to choose from, including blocks, dramatic play, books, puppets, manipulatives, art, and science. There are 18 children enrolled in the Pre-K classroom, including some who are NC Pre-K students. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, children’s files, and program records. Staff files were not available for review today and will be reviewed at the next visit. VIOLATIONS The following violation was observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. No activity plan was posted or available for review. GS 110-91(12); .0508(a) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by 9/24/24. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 97%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. (The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score.) An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNINCAL ASSISTANCE -No activity plan was available for review today. You stated that you had started working on it but had not finished it. It is important to have a lesson plan posted in case a substitute teacher is in the room, and it's also important to share the activity plan with parents so they are informed about what their children are learning. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll by November 1, 2024. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -We reviewed handwashing procedures and discussed the appropriate use of hand sanitizer since a student in the classroom was recently diagnosed with Hand, Foot, and Mouth Disease. The teachers have been diligent about cleaning the toys to help prevent the spread of illness. -While attendance is kept online, it would be helpful to have a printed roster to use when transitioning from one space to another. I recommend doing both a head count and a name/face match before leaving a space. TRAINING REMINDERS -CPR and First Aid must be renewed every two years. -Health and safety training must be renewed every five years. Staff members who completed training in 2019 are due to renew in 2024. --Criminal Backgrounds Checks for all staff members should be completed every five years. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your staff members’ education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process. The school administrative staff directed me to classroom space #206. I observed 14 children transitioning from lunch to the playground. I observed them playing on the stationary equipment. The children transitioned to classroom #206 from lunch to complete routine tasks and have outside play time. I observed the children transition from outside play to prepare for rest time. LICENSE STATUS The facility was issued a five (5) star license on 12/3/18 earning seven (7) points in Staff Education, seven (7) points in Program Standards and one (1) Quality Point. MONITORING During today’s visit, a full assessment was conducted monitoring all spaced occupied by the children, including the outdoor play environment. Two (2) staff and five (5) children’s files were reviewed. All spaces were monitored for supervision, child capacity, materials, and equipment. According to the posted activity plan, the theme was, “All About Me.” There were fourteen (14) children enrolled in the NC Pre-K Program. The teacher stated that none of the children were taking medication. The designated outdoor play area was fenced in with stationary equipment. --PROGRAM RECORDS *The last playground inspection was conducted 8/31/23. *The last documented fire drill was conducted 9/1/23. *The last documented shelter-in-place or lockdown drill has not been conducted yet. *The last Sanitation Inspection was conducted on 5/2/23 with ‘3’ demerits and a ‘Superior’ classification. *The last Fire Inspection was conducted on 5/2/23. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The right side of the fence has three areas of sharp edges and protrusions on the NC-Prek playground. GS 110-91(6); .0605((i) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be received on or before September 27, 2023, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. FENCE It was discussed with me that a work order for the fence was placed last week to repair the fence. I would recommend sending a work order with ‘high importance’ to get the protrusion areas of the fence repaired. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. Please use this time to make sure all your staff have a WORKS account with all education information updated. Also review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2017, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date.
