Upon arrival, I was greeted by the afterschool site manager, Dominique Taylor, who assisted me during today’s visit. LICENSE STATUS The facility currently operates with a Five Star license issued on September 7, 2017. The program received seven points for Staff Education, seven points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last documented fire drill was conducted on 1/5/26. -The last documented lockdown/shelter-in-place drill was conducted on 1/9/26. -The last annual compliance visit was conducted on 2/12/25. MONITORING I completed a walk-through of the center, visiting the cafeteria, which is the primary space for the afterschool program, as well as the restrooms, gym, and outdoor play area. The afterschool program serves 12 children from RN Harris Elementary, 18 students from Burton Elementary School, and one child from Fayetteville St. Elementary School. Children were observed arriving and participating in activity centers in the cafeteria, including art, math, science, board games, blocks, manipulatives, and a reading center. After all children arrived, the older group of children went to the gym for active play (due to ice on the playground). The younger children used the restroom, washed their hands, and ate a snack of white cheddar popcorn and juice. The older children had their snack about 20 minutes later. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Three staff and five children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. One child has an epi-pen; therefore, I reviewed the storage of the epi-pen and appropriate medication forms and cited a violation. This program’s license currently has a restriction stating that the playground does not meet childcare safety standards. This restriction is being removed due to a change in statute (House Bill 412), and a new license will be issued. VIOLATIONS The following violation was observed and documented during today’s visit: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. One child's emergency epi-pen had expired in November 2025. 10A NCAC 09 .0803(1)(d) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by February 13, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 99%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -It is important to keep emergency medications current since expired medication may have reduced effectiveness. Today, one child’s epi-pen had expired. Child care rules state that all expired medication should be returned to the parent (or discarded after 72 hours if the parent fails to retrieve it), and that the medical authorization form must be renewed every six months. Please return the medication to the parent and ask them to send an up-to-date replacement immediately. CONSULTATION -You plan to pursue Pathway 1 (Program Assessment) with a planned rated license assessment in April 2026. In the meantime, you can start to prepare by completing the self-study for the SACERS-U Environment Rating Scale. You can also create a QRIS binder/folder with the following information: *Examples of Continuous Quality Improvement *Examples of Family and Community Engagement practices *A Status Letter from WORKS for each staff member in order to determine their individual star rating *Your chosen approved curriculum -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. Also, child care providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. The Division no longer recognizes or accepts Change of Information forms. One new staff member needs to complete this training within one year of hire. RATED LICENSE STATUS New QRIS rules are in effect, and the Division is transitioning programs to their new rated license in 2026. Resources, forms, and training will be provided to support you in preparing for new pathway options to earn your Two- through Five-Star Rated License. All the newest information will be posted at https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization and will also be provided through emails and Raise NC newsletters. A transition plan has been created for your facility with a target month for your rated license assessment. I was accompanied by Mandy Annunziata, Child Care Consultant. Upon arrival, we were greeted by the afterschool site manager, Dominique Taylor, who assisted us during today’s visit. LICENSE STATUS The facility currently operates with a five-star license issued on September 7, 20217. The program received seven points for staff education, seven points for program standards, and one quality point. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 5/1/24, and the center scored a superior rating with zero demerits. -The last fire inspection was completed 1/19/24. -The last documented outdoor play area inspection was completed on 9/3/24. -The last documented fire drill was conducted on 8/27/24. -The last documented lockdown/shelter-in-place drill was conducted on 8/27/24. -The last annual compliance visit was conducted on 9/12/23. MONITORING We completed a walk-through of the center, visiting the cafeteria, which is the primary space for the afterschool program, as well as the outdoor play area. The children were observed arriving, transitioning to the playground, and eating snack. After snack, they were able to choose between a number of activity centers set up in the cafeteria, including arts and crafts, math manipulatives, science activities, board games, and a homework table. They could also play in the gym. During our visit, children arrived by bus from C.C. Spaulding, Fayetteville St., and Burton Elementary Schools to participate in the afterschool program. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. This program does not provide transportation; therefore, this requirement was not monitored. One child has a epi-pen; therefore, we reviewed the storage of the epi-pen and appropriate medication forms and found them to be in compliance. The following violations was observed and documented during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A large railroad tie was placed on a picnic table on the playground for older children, posing a falling and tripping hazard. 10A NCAC 09 .0601(a) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. TECHNICAL ASSISTANCE (samples) It is important to maintain a safe environment on the playground. Today, we observed a large railroad tie set on a picnic table on the playground for the older children. This is dangerous for children who could accidentally run into it or knock it off the table, where it could hit someone and potentially break a bone. Please speak to the school principal and maintenance staff about having it removed from the table and playground immediately. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll by November 1, 2024. Lead testing should be conducted every three years. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. TRAINING REMINDERS -Training from 9/12/23 to 9/4/24 was reviewed today and results can be found on the attached Staff/Training Worksheet. -CPR and First Aid must be renewed every two years. One new staff member needs to complete this training within 90 days of hire, and another staff member’s certifications expire on 10/29/24. -Health and safety training must be renewed every five years. One new staff member needs to complete this training within one year of hire. Also, “Recognizing and Responding to Suspicions of Child Maltreatment” should be completed within two weeks of hire. This training can be found on the Prevent Child Abuse NC website: https://preventchildabusenc.org/online-trainings/ --Criminal Backgrounds Checks for all staff members should be completed every five years. All three staff members caring for children have qualifying letters that will expire within the next month. Please have them initiate new background checks immediately. In addition, the principal of the school needs to have a background check and qualifying letter on file. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your staff members’ education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process.
