A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Eleanor Luster. You assisted me with today’s visit. There was a total of forty-seven (47) children present, aging in range from infants to four years old. The children were observed playing in centers with materials, interacting with the teachers, eating lunch, preparing for rest time and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued November 22, 2017, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility ASKORP, INC listed as current/active. INSPECTIONS *The last fire drill was conducted 10/8/25. *The last emergency drill was conducted on 10/2/25. *The last playground inspection was completed 10/20/25. *The last sanitation was completed on 9/10/25, with a ‘Superior’ classification. *The last fire inspection was conducted on 10/29/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children including the outside learning environment. New staff CPR/FA, qualification letters and newly enrolled children’s files were reviewed. Ten percent of the old staff files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored during today’s visit. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last conducted fire inspection was conducted on 10/29/24. 10A NCAC 09 .0304(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #1 and #2 Children's Allergy medicine and Zyrtec was not stored in a locked cabinet or other locked container. 15A NCAC 18A .2820(d) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In space #3 an EpiPen was not in the original container with an Rx prescription. .0803(2)(a) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In space #1 a Monster Twister energy drink was observed on the shelf in view of the children. .0901(i) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member did not complete the health and training topics within five years of the previous trainings. .1103(b) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before November 10, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 94%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. NUTRITION In space #1 a Monster Twister energy drink was on the shelf in view of the children. Modeling good nutrition in child care centers is important because young children learn by observing adults. Practicing good nutritional habits helps establish healthy eating habits. I would recommend reviewing your nutritional policy with all staff members. I would also encourage having all outside drinks placed in an opaque bottle. MEDICATION In space #2 and space #3 Children’s Allergy medicine and Zyrtec were not kept in a locked storage. In space #2 and #3 and EpiPen was not kept five feet up out of the reach of children. The administrator stated that she will be mounting a medicine box holder on the wall five feet up. In space #3 the EpiPen was not stored in the original box. I would recommend sending a memo out to the parents reminding them that all prescription medications are to be kept in their original container or box FIRE INSPECTION The last fire inspection was conducted on 10/29/24. The administrator stated that the fire inspection is scheduled for 11/3/25. I would recommend using a reminder or an outlook calendar to remind you of all inspections. HEALTH AND SAFETY TRAINING One staff member health and training was not completed within five years. The staff member completed health and safety training in 2019. I would recommend using an excel spreadsheet or an outlook reminder to remind you of upcoming trainings. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS MODERNIZATION I spoke with the Administrator and she stated that the owner will be setting up a meeting with me to discuss the centers options. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. HEALTH CONSULTANT Ellie Morrise is the new Child Care Health Consultant. ONGOING TRAINING HOURS I observed training hours from 10/31/24 to 10/27/25 and all staff were found in compliance. The director, Ms. Eleanor, assisted us during this visit. The Deputy Director, Candace Witherspoon, accompanied me on today’s visit. Upon arrival, you greeted us and welcomed me into your program. Seven (7) classrooms were opened today. We conducted a general walk-through of the indoor and outdoor learning environment. The children were observing having circle time, having free play in their indoor learning environment, exploring their outdoor learning environment, eating lunch. RATED LICENSE STATUS The program holds a 5 star-rated license on 4/30/18. The program is in the “held harmless” status and points from the previous assessment are carried forward. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 Quality through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. INSPECTIONS The last fire drill was conducted on 4/12/24. The last emergency drill was conducted on 3/21/24. The last playground inspection was conducted on 4/17/24. The last sanitation inspection was conducted on 12/19/23. The fire inspection was conducted on 11/6/23. During this visit, a partial assessment of the child care requirements was conducted. The following items were monitored and found in compliance during this visit: nurturing interactions, daily attendance, license posted & permit restrictions followed, safe indoor environment, fire drills & inspections, CRC qualifications, CPR/First Aid certification, and storage of medications & hazardous materials. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Bleach bottles were not stored at least 5 feet high and out of the reach of children in space #1, space #2, and space #5. 10A NCAC 09 .0601(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A child in space #1 albuterol tube were not in it's original container. .0803(2)(a) 848 Questionable medication instructions from parents were followed without signed written dosage instructions received from physician or authorized health professional. A child in space #4 did not have a current permission slip to administer Benadryl and an epi-pen if need for a chronic emergency situation. 10A NCAC 09 .0803(5) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of the mulch was less than one inch around the slide fall zones on the older playground. .0605(k)(1-4) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by May 29, 2024. The letter should address each violation, and explain how and when it has been corrected. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest getting a shelf/basket that is put on the wall at least 5ft high to store all bleach bottle solution in so they are out of the reach of the children. I would suggest measure the depth of the mulch monthly when doing your playground inspection so you would know when you should order more mulch before being out of compliance. I would suggest making sure you communicate with the parents about what the expectations are when it comes to bringing in chronic medication and the forms that need to be completed. The action plans should be completed yearly, and the permission slips every six months. Also, we discussed having the teachers to double check the form when receiving the forms and medication to ensure no vital information was missed off the forms or expired. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, so they should begin to retake the training now.
