The purpose of today’s visit was to monitor applicable child care requirements during a Routine Unannounced visit. This Family Child Care Home operates with a three-star license, which was issued on 11/18/23. The last Annual Compliance visit was conducted on 11/13/24. The compliance history for the previous eighteen-month timeframe was 98%. This Family Child Care Home has children enrolled on first and second shift. The operator stated that first shift hours were from 6:00 am to 4:00 pm, and second shift hours were from 4:00 pm to 9:30 pm. During today’s visit, the indoor child care space and the designated outdoor environment were monitored. The operator stated that currently the designated outdoor environment was the front yard until repairs could be made in the backyard. OBSERVATIONS: There were two children present today, ages one and four. I observed the children playing in child care space and having a morning snack of pretzels, pineapple, and milk. The required information was posted in child care space. CHILDREN’S RECORDS: I reviewed the record for the child, who began attending on 3/13/25. OPERATOR’S FILE: During today’s visit I verified that the operator had current Criminal Record Check clearance, along with current CPR and First Aid training. The operator’s adult resident daughter was present during today’s visit. The daughter had current Criminal Record Check clearance. TRANSPORTATION: Prior to today’s visit the operator had indicated that she wanted to be approved for transportation. Prior to today’s visit and during today’s visit I reviewed the transportation requirements with the operator. Please review the transportation requirements located in the NC Child Care rules 10NCAC 09 .1700. During today’s visit I observed that the 30-day tag for the operator’s new vehicle expired on 4/16/25. The operator stated that the Department of Motor Vehicles was sending her another temporary license for 60 days until an error made by DMV could be corrected. The operator was aware that she could not begin transportation services until she had permanent tags on the vehicle. I verified that the operator had current insurance on the vehicle. The operator stated that she was purchasing items needed for the vehicle to provide transportation services. SMOKE DETECTORS: The smoke detector in child care space was tested during today’s visit. INSPECTIONS: The last documented fire drill was conducted on 5/10/25. The last documented shelter-in-place drill was conducted on 4/16/25. The last outdoor inspection was conducted on 5/8/25. During today’s visit the water testing bottles arrived from the Clean Water for Carolina Kids. The operator stated that she had watched the video and submitted her responses to Clean Classrooms for Carolina Kids regarding lead paint and asbestos. The following violations were cited today: Violation Number Comment Rule 708 Indoor stairs with more than two steps were not made inaccessible to children two years of age or younger. The steps leading to the upstairs level of the home were accessible to the toddler present today. 10 A NCAC 09 .1719(a)(29) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The operator's IT'S SIDS training expired on 2/16/25. .1703(a)(4) 1964 Activity plan did not reflect that children have at least four different activities daily, at least one of which is outdoors. There were no activity plans for the children who attended on second shift. 10A NCAC 09 .1718(a)(8)(C)(i-v) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 6/17/25, explaining how each violation has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY IN THE FOLLOWING AREAS: SMOKE DETECTOR: It appeared that the smoke detector in child care space was battery-operated only. The operator did not know if it was also electrically operated. I stated that the smoke detector in child care space needed to be electrically operated with a battery back-up, or the operator could have an electrically operated smoke detector located beside a battery back-up detector, in accordance with NC Child Care Rule 10NCAC 09 .1728 © (3). The operator stated that she would have an electrician make necessary changes or additions, so the smoke detector requirement was being met. Please include this matter in the Corrective Action letter for today’s visit. ADDITIONAL CAREGIVER: During today’s visit the operator stated that her daughter wanted to be an additional caregiver. She stated that her daughter had completed NCECC and CPR/First Aid. During today’s visit I stated that the daughter needed to create a file, which contained all the requirements in 10NCAC 09 .1729. The requirements were discussed during today’s visit. I stated that I would review the daughter’s file during the next visit. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation. The operator stated that she had had the FCCERS conducted at her Family Child Care Home in the past. I recommended that the operator participate in the free online webinar on the FCCERS-3 assessment tool on the NC Rated License Assessment Project website to familiarize herself with the differences in FCCERS-R and FCCERS-3spell. www.ncrlap.org CONTACT INFORMATION: Please contact me if I can be of any assistance to you. The purpose of today’s unannounced visit was to monitor applicable child care requirements during your Annual Compliance visit. The last Annual Compliance visit was conducted on 11/17/23. The compliance history for the previous eighteen-month timeframe was 100%. This Family Child Care Home operates with a three-star license, which was issued on 11/18/23. During today’s visit the indoor child care space and the currently designated outdoor environment (front yard) were monitored. OBSERVATIONS: There were two preschool age children present today. I observed both children preparing for naptime and resting during naptime. During arrival I received the enrollment worksheet, the operator’s verification form, and the signed statements of understanding. The information was reviewed and discussed with the operator. RESIDENTS IN THE HOME: The operator stated that she and one daughter were the only current residents in the home older than age 16. INSPECTIONS: The last documented fire drill was conducted on 11/6/24. The last documented outdoor inspection was completed on 11/5/24 (front yard). The last documented lockdown drill was conducted on 10/6/24. SMOKE DETECTOR: The electrically operated smoke detector with battery back-up was not tested during the visit. TRANSPORTATION: This Family Child Care Home does not offer transportation services. IN-SERVICE TRAINING: In-service training hours were tallied during today's visit. I remember the operator that she needed to complete an updated IT'S SIDS training by 2/16/25. The following violations were cited today: Violation Number Comment Rule 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. The children needed the signed statement on file. GS 110-102 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. The preschool age children needed the verification on file that parents had been given a copy of the shaken baby syndrome and abusive head trauma policy. .1726(b)&(c) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 11/27/24, explaining how each violation has been corrected. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation. AGENCY UPDATES: Child Care facilities are required to test their drinking and cooking water for lead every three years. For information you should contact Clean Water for Carolina Kids www.cleanwaterforcarolinakids.org. The operator stated that she was waiting for a response regarding the water testing process. Child care facilities are now required to test for lead and asbestos hazards in their buildings. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch the pre-enrollment webinar, which are available at www.cleanwaterforUSkids.org/carolina. The operator stated that she had watched the video. The operator stated that she would answer the questions and submit the information for clearance.
