This documentation was reviewed with the site manager, Theresa Wilson, and a copy of the visit summary documentation was given to her at the conclusion of the visit. Upon arrival, I checked into the main office then proceeded to the gym to meet the site manager. I was greeted by James Carr, Group Leader who informed me that the Site Manager, Theresa Wilson was covering at another program. I completed a general walk through of the spaces used by the children and outdoor play areas. Although this DPI program does not want their playground to be monitored for child care requirements, I did check for safety concerns. I observed the children arriving to the program, transitioning to activities, and eating afternoon snack. Ms. Wilson arrived to the program at 2:45pm. CURRENT LICENSE STATUS Currently this program operates with a five-star license issued April 8, 2019, earning 7 points in the Staff Education Component, 2 points in the Program Standards Component and 1 Quality Point. The facility operates after school care from 2:15pm-6:00pm. The primary space is in the cafeteria. The bathrooms are in the hallway outside of the cafeteria. The auxiliary space used by the children is the media center. INSPECTIONS *The last annual compliance visit was conducted on April 27, 2022. *The last documented shelter-in-place drill was completed on March 9, 2023. *The last documented fire drill was conducted on March 6, 2023. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Staff/Child Ratio, CPR/FA, Special training, Criminal Background Checks, Emergency Medical Care Plan, Storage of Hazardous Substances, Storage of Medication, General Safety, Discipline, Adequate/Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions. The following violation was observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. One staff was responsible for twenty-five children ranging in age from five years old to eight years old. 10A NCAC 09 .2818 In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by October 26, 2023. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number TECHNICAL ASSISTANCE STAFF CHILD RATIO The elementary school dismissal is at 2:15pm and the primary space for the afterschool children is in the cafeteria. Children not enrolled in the afterschool program come to the cafeteria for bus/van dismissal. Parkwood Elementary staff are responsible for those children until 2:30pm. At 2:30pm the group leaders of the afterschool program take the children enrolled in their program. There were two group leaders present today from 2:30-2:45pm. Mr. Carr informed me that the site manager was at another school providing coverage, the group leader responsible for group #2 called out today, and the substitute who is an employee at the school doesn’t arrive until 3:00pm. Mr. Carr was responsible for twenty-six children (groups 1 and 2) until 2:45pm. I asked Ms. Wilson if the substitute could arrive at 2:30pm and she stated the principal wouldn’t allow that. I explained that she would need to provide coverage at her program if all staff are not present or talk with the principal about later dismissal for the afterschool children. If those options don't work contact the program specialist to cover in order to maintain compliance. A follow-up visit will be made in the near future due to the staff-child ratio violation that was cited. If a violation of staff-child ratio is cited on the follow-up visit your program will be at risk of being issued an administrative action. STAFF RECORDS Staff records were monitored for all new staff since the last annual compliance visit in April 2023. On your next annual compliance visit training hours will be monitored. All staff must complete on-going training hours by 3/31/23. RATED LICENSE INFORMATION DCDEE has decided that Rated License assessments will resume, and each program will complete the process based on Cohort assignments (1-3). Your program is in Cohort 2. The planning period will start on 7/1/24 and the assessment period begins 7/1/25. -Use this time to make sure all your staff have a WORKS account with updated education information. -Review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). -Participate in trainings related to the Environment Rating Scale SANITATION RULES There are new Child Care Sanitation Rules, effective July 1, 2023. You can access https://ehs.dph.ncdhhs.gov/hhccehb/cehu/ccs/children to get the latest Rules as well as Q and A documents from the past webinars. CPSC RECALLS The U.S. Consumer Product Safety Commission has recalled a few items. Please share this information with the parents in your program.
