The off-site administrator was present for today’s visit. The purpose of today’s unannounced visit was to monitor child care requirements during your Annual Compliance visit. The last Annual Compliance visit was conducted on 11/9/22. The compliance history for this center for the previous eighteen-month timeframe was 95%. All indoor licensed space and the outdoor environment were monitored. The lead teacher stated that all fifteen children enrolled in classroom space #1 were NC Pre-K. NC Pre-K requirements were monitored during the visit. All required information was posted. Both classrooms had current activity plans. Meals for this center are catered from Chicken Hut in Durham. Lunch today would consist of chicken nuggets, roll, corn, pineapple, and milk. OBSERVATIONS I observed the children enjoying the outdoor environment and engaged in teacher-directed activities. STAFF FILES: The staff training worksheet was received prior to today’s visit. Staff files were reviewed during the visit and yearly in-service training was tallied. CHILDREN’S FILES: Nine children’s files were reviewed today (six NC Pre-K files and three non-NC Pre-K files). INSPECTIONS: The last documented annual fire inspection was conducted on 5/25/23. The last documented sanitation inspection was conducted on 2/2/23. The last documented fire drill was conducted on 9/18/23. I reminded the staff to conduct a fire drill today or tomorrow for the month of October. The last documented lockdown drill was conducted on 10/11/23. The last playground inspection was conducted on 10/2/23. The following violations were cited today: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Upon arrival the file for the Family Support employee was not on the premises. G.S. 110-91( 9) 1767 The health assessment did not include a vision screening. Of the six NC Pre-K files reviewed today, one child needed a vision screening on file. .3005 (a)(3) 1768 The health assessment did not include a hearing screening. Of the six NC Pre-K files reviewed today, one child needed a hearing screening on file. .3005 (a)(4) 1769 The health assessment did not include a dental screening. Of the six NC Pre-K files reviewed today, five children needed a dental screening on file. .3005 (a)(5) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 11/13/23, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY ON THE FOLLOWING TOPICS: FAMILY SUPPORT STAFF: The Family Support staff person stated that she had requested more medical information from Lincoln Community Hospital for the children’s files. I explained why the Family Support staff person needed a file at both her assigned locations. RATED LICENSE: The Rated License three-year reassessment process has resumed. This center is in Cohort 1 for reassessment. The preparation year for the assessment is from 7/1/23 through 6/30/24. The assessment year will begin on 7/1/24. After reviewing the options, the site administrator stated that she wanted to request the Environment Rating Scales at this time. I stated that I would email her an application and request form. Prior to today's visit the administrator had sent me the Rated License Staff information sheet. I verified through WORKS that the center had achieved 7 points in the education component of the Rated License assessment process (7 points for administrator, 7 points for lead teachers, and 7 points for teachers).
