The director, Sandra Tate, was present and assisted me during today’s visit. Upon arrival, you greeted me and welcomed me into your program. Five (5) classroom was opened today. I conducted a general walk-through of the indoor and outdoor learning environment. The children were observed having floor time, reading books, eating lunch and resting their bodies. RATED LICENSE STATUS The program holds a 4-star-rated license issued on 1/9/18. The program is in the "held harmless" status and points from the previous assessment are carried forward. The program received 4 points in Program Standards, 6 points in Staff Education, and 1 quality point through the programmatic option of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last fire drill was conducted on 11/6/24. The last playground inspection was conducted on 11/6/24. The last emergency drill was conducted on 10/25/24. The last sanitation inspection was conducted on 10/3/24. The fire inspection was conducted on 10/31/24. During this visit, a full assessment of Child Care Requirements was conducted. I did not assess transportation requirements because the program does not transport children. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. A child's diaper cream ointment had expired on 10/24 in space #3. 10A NCAC 09 .0803(1)(d) 1245 For each child, there was not at least 30 sq. ft. inside space per the total licensed capacity and 100 sq. ft. outside space for each child using the outdoor learning environment at any one time. There were 11 one year old children with 2 staff in space #3, which exceeded the total inside space allowed per child. 10A NCAC 09 .2809(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Two children in space #2 did not have a medication authorization form for signed by a parent administer the child's EPI-Pin if needed. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. I would suggest making sure all pertinent information is filled out for all emergency medication before allowing parents to leave them at the center. I would suggest having teacher to check all diaper cream monthly to ensure that they are not expired. I would suggest reviewing the space capacity with all teachers to ensure that they are aware of the number of children allowed in their space at all times. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year November 29, 2023- November 25, 2024. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, they should begin to retake the training. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website: nchildcare.ncdhhs.gov frequently to stay updated on rule requirements. LICENSING FEES Your facility’s annual licensing fee Invoices will be emailed to the facility addresses on file by November 30, 2024. The visit summary was reviewed with Ms. Tate, and a copy was given to her at the conclusion of the visit today. Upon arrival I was greeted by church personnel. I completed a general walk through of the facility observing the indoor and outdoor environments. Ms. Tate was providing coverage in space# 3 today. The infants were engaged in tummy time and free play with their teacher. The toddlers and preschool children were observed getting diapers changed, having center play and outdoor play. CURRENT LICENSE STATUS Currently this center operates with a four-star license issued January 9, 2018, earning six (6) points in Staff Education, four (4) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on November 29, 2024. *The sanitation inspection was completed on October 24, 2023, with a "Superior" classification. *The fire inspection was completed on December 11, 2023, and the facility is approved for day time care only. *The last documented fire drill, playground inspection, and lockdown drill were all conducted on April 3, 2024. During this visit, a partial assessment of Child Care Requirements was conducted. The items monitored were Supervision, Staff/Child Ratio, Discipline, License Posted, Permit Restrictions, Adequate & Approved space, Program Records, Staff Records, CPR, First Aid, ITS-SIDS, Criminal Background Checks, Storage of Hazardous Substances, and Storage of Medication. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 201 A room was occupied by more children than the space allowed at 35-sq. ft. of floor space per child. Two classrooms exceeded space capacity. In space #2 there were 12 children present, however the space capacity is 11. In space #3 there were 12 children present, however the space capacity is 10. GS 110-91(6); .1401(f) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The substitute present who started employment in August 2023 did not have First Aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The substitute present who started employment in August 2023 did not have CPR training. .1102(d) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The substitute present who started employment in August 2023 and works in the infant classroom did not have ITS-SIDS training. .1102(f) 1898 Staff did not complete the health and safety training within one year of employment. There were 12 children two-years of age present. I reviewed the space capacity for each classroom with Ms. Tate. I explained that the group size for the two classrooms is correct for the age groups present today. However, the space capacity for the classrooms allows less children to accommodate the space. Space #1-14 children Space #2-11 children Space #3-10 children Space #4-13 children Space #5-13 children CONSULTATION RATED LICENSE INFORMATION This facility is in Co-Hort 1 and will complete a prep year assessment. Their window for completing the Environment Rating Scales is April 8th-May 3rd. I reminded the director of the following: -If you do a prep year ERS and are satisfied with your scores, the staff education evaluation could be completed in the assessment year. -If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment. -If you are not satisfied with your scores ERS assessments can be used as guidance to enhance program quality for the next assessment. RULE CHANGES EFFECTIVE 1/1/24 We discussed child care rule changes effective 1/1/24 for child care center. For specific details and training regarding these changes, please ensure you have the January 2024 Child Care Rulebook and enroll to view information in the DCDEE Moodle. The director was covering in a classroom therefore I completed a walk through of the facility alone. I monitored the indoor and outdoor environments. The infants were enjoying free play with their teachers and music time. The toddlers and preschool children were observed in center activities and gross motor play. CURRENT LICENSE STATUS Currently this center operates with a four-star license issued January 9, 2018, earning six (6) points in Staff Education, four (4) points in Program Standards and one (1) Quality Point. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, New Life Christian Center is listed as current/active. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 504 Perishable foods were not stored to protect against spoilage. The milk observed today had expired. One gallon of milk expired on 11/24/23 and two gallons of milk expired on 11/25/23. 15A NCAC 18A .2806 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. The director was covering in a classroom therefore I completed a walk through of the facility alone. I monitored the indoor and outdoor environments. The infants were enjoying free play with their teachers and music time. The toddlers and preschool children were observed in center activities and gross motor play. CURRENT LICENSE STATUS Currently this center operates with a four-star license issued January 9, 2018, earning six (6) points in Staff Education, four (4) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on December 7, 2022. *The sanitation inspection was completed on October 24, 2023, with a "Superior" classification. *The fire inspection was completed on December 1, 2022, and the facility is approved for day time care only. *The last fire drill and playground inspection were both conducted on November 6, 2023. *The last shelter-in-place/lockdown drill was completed on August 11, 2023. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, New Life Christian Center is listed as current/active. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered.
