The family childcare provider, Ms. Mary Garrett, assisted me with today’s visit. The visit summary was reviewed with Ms. Garrett, and a copy was given to her at the conclusion of the visit. Upon arrival, you greeted me and welcomed me into your program. Four children were present during today’s visit. I conducted a general walk-through of the childcare spaces. I observed the children finishing up lunch, getting diapers and taking naps. RATED LICENSE STATUS The program holds a 1-star-rated license issued on 9/25/25. INSPECTIONS The last fire drill was conducted on 11/15/25. The last emergency drill was conducted on 12/1/25. The last playground inspection was conducted on 2/16.26. During this visit, a full assessment of Child Care Requirements was conducted with the following exceptions. You reported that you do not administer medication, care of any infants, and do not transport children. Therefore, these requirements were not monitored. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 803 Operator did not wash his/her hands before, as well as after, diapering each child. The operator did not wash her hands between diapers changes nor after she finished changing diapers. 10 A NCAC 09 .1720(d)(3)(A) 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. The refrigerator did not have a working thermometer in it to help maintain the temperature. 10A NCAC 09 .1725(a)(10) 815 Children's hands were not washed after diapering. Two children one year of age and one child two years age did not wash their hands after getting their diapers changed. .1725(a)(6)(C) 816 Each child's hands were not washed before and after eating. Two children one year of age and two children two years of age did not wash their hands after eating lunch. They got their hands wiped with baby wipes. .1725(a)(7)(B) 908 Health questionnaire was not completed annually. The operators did not have a current health questionnaire available for review during today's visit. .1703(a)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two children did not have a completed medical assessment on file within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1853 The operator did not conduct a monthly fire drill. A current fire drill was not document and available for review. The last fire drill was conducted on 11/15/25. .1719(a)(15) & .1721( e)(2) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. There were Lysol sitting on the counter in the kitchen. .1719(a)(6) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by March 25, 2026. The letter should address each violation, and explain how and when it has been corrected. Please send the letter to shannel.hall@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 98%. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest putting all monthly inspections on the calendar. I would suggest reviewing the hand-washing policy and looking for trainings that would aid in teaching the steps for washing adults and children’s hands and when to wash them. I would suggest doing a walk through to ensure all hazardous items are locked up and out of the reach of children at all times. I would suggest double checking all forms that is giving to parents when they return to ensure that all required information is provided. A. Ortis did not have a medical on file from the doctor and Y. Everette parents did not complete the parent portion of the medical assessment. ADDITIONAL ITEMS DUE to Additional Violations to Be Added! *Copy SIDs Training for M. Garrett & T. Hunt by 3/16/26. *Copy of First Aid and CPR for M. Garrett & T. Hunt by 3/16/26. *Health and Safety trainings are due to me by 3/24/26. *20 Training hours by 3/24/26 for M. Garrett & T. Hunt. *Emergency Drill needs to be conduct by the end of 3/31/26. *EPR plan needs to be completed by 3/24/26. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. ANNUAL IN-SERVICE TRAINING The in-service hours were monitored for the annual year March 24, 2025 -March 11, 2026. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. You completed the trainings in 2018, should have already began to retake the training now. The temporary time period license was issued on 3/24/25 and will expire on 9/24/25. Mary Garrett, additional caregiver, greeted me upon my arrival. Ms. Hunt was not at the facility as she was caring for her aunt who is currently under hospice care. You were present with six preschool-aged children enrolled. The children were observed finishing their lunch, being diapered, and resting. The license was posted along with the NC Summary of the Law. A first aid reference sheet was available. The daily schedule and activity plan were posted. The center does not provide transportation nor aquatic activities; thus, those requirements were not monitored during this visit. The most recent fire drill was completed on 6/19/25. The outdoor inspection was completed on 6/19/25. The first emergency drill was conducted on 6/19/25. The children’s files were reviewed during this visit and found to be in compliance. The following violation was cited during today's visit: Violation Number Comment Rule 102 Number of preschool children exceeded five children, including the provider's own preschool children. Six children ranging in age from 5 months to four years were present during the entire visit. One of the preschool-aged children was the additional caregiver's grandchild and the youngest infant was the operator's grandchild. GS 110-91(7)(b) The violation documented during today's assessment was reviewed with the additional caregiver and must be corrected IMMEDIATELY. Evidence of correction must be sent to me no later than August 1, 2025. Compliance verification must be a dated and signed letter that includes statements addressing how and when the violations were corrected. Please include the facility's name and ID number on the letter of compliance. The letter may be emailed to angela.algerwalker@dhhs.nc.gov. If you email the compliance letter, you must include a signature block, which contains your name, position, facility name, and facility ID number. An example is: Jane Doe, Operator AAA Child Care ID # 12345678 RATED LICENSE INFORMATION During today's visit we reviewed the information in the WORKS database. The operator is still in the process of setting up her WORKS account. She has enrolled in the EDU-119 class that begins on 5/21/25. The operator will need to choose a quality point before 8/1/25. The provider began a three-month self-improvement study to qualify for two points in program standards on 5/1/25. She will contact me when she has completed the three-month self-study. The rated license will be issued at the end of the temporary time period. REMINDERS: 1. We reviewed the NC Foundations of Early Learning Guidelines and how to reference that in the weekly lesson plans. 2. You must complete the Health and Safety trainings before 3/24/26. 3. The operator must complete the Emergency Preparedness and Response training before 3/24/26. You must complete and submit an emergency plan online within four months of completing the class. 4. 20 In-Service training hours are due by 3/24/26.
