Upon arrival, I was greeted by the co-owner, Tiffany Dunn, who assisted me during today’s visit. The other co-owner, Thalia McQueen also was present. LICENSE STATUS The center currently operates with a four star license issued on August 28, 2018. The program received five points for staff education, five points for program standards, and one quality point for having a staff benefits package and infrastructure for parental involvement. It is licensed to serve children on first and second shift. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 1/21/25, and the center scored a superior rating with four demerits. -The last fire inspection was completed on 3/14/24. The fire inspector came by during my visit and said he would return for an inspection on 3/14/25. -The last documented outdoor play area inspection was completed on 3/3/25. -The last documented fire drill was conducted on 3/7/25. -The last documented lockdown/shelter-in-place drill was conducted on 3/7/25. -The last annual compliance visit was conducted on 4/30/24. MONITORING I completed a walk-through of the center, visiting two classrooms as well as the outdoor play area, kitchen area, and bathroom. When I arrived, all children were playing in the fenced backyard. The infant was being held by a caregiver while the older children played with balls and gross motor equipment. I observed the children using the restroom and washing their hands when they returned inside for lunch. Lunch consisted of Salisbury steak, peas and carrots, peaches, and milk. The center has two classrooms, one for infants and toddlers, and the other for preschoolers. Both rooms are set up with a variety of age-appropriate materials for learning through play. The provider stated that they recently painted both classrooms. Children’s artwork today was child-directed; the preschool children used potatoes cut in half and dipped in paint to make prints on paper. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Two staff and four children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. No children require medications; therefore, this requirement was not monitored. The center serves children under one year of age, thus, safe sleep requirements were monitored and found in compliance. VIOLATIONS The following violation was observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. No activity plan for the older preschool group was available for review. GS 110-91(12); .0508(a) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by March 26, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 97%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -No activity plan for the older class was available for review today. It is important to have a lesson plan posted to guide your teaching so you cover all the required domains of early learning, and it’s also important to share the activity plan with parents so they are informed about what their children are learning. I suggest posting the activity plan on Friday afternoon for the upcoming week. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. TRAINING REMINDERS -Training from 4/30/24 to 3/11/25 was reviewed today and results can be found on the attached Staff/Training Worksheet. All required training hours for the current year should be completed by 4/30/25. Please send me documentation of training hours to avoid violations. -CPR and First Aid must be renewed every two years. Your certification expires 7/17/25. -Health and safety training must be renewed every five years. -ITS-SIDS training must be renewed every three years. Your training is valid until 11/3/26. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your staff members’ education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process.
