The purpose of today’s unannounced visit was to conduct a full assessment visit and monitor applicable child care requirements to complete the star-rated license assessment requirements. The last Annual Compliance visit was conducted on 9/8/25. This center operates with a five-star license, which was issued 2/1/21 with 7 points in education, 7 points in programming, and 1 quality point. CORPORATION STATUS: According to the NC Secretary of State web site, the corporation status of Families & Communities Rising, Inc. is “current-active”. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 94%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: During today’s visit there were 11 children (ages 4 and 5) present with two teachers. I observed the children engaged in free play activities, having circle time, and enjoying the outdoor environment. Food allergies and medication were monitored today. STAFF RECORDS: During today’s visit staff files requirements were verified. The staff had current Criminal Record Check clearance, CPR/First Aid training, Staff had updated Heath Quesitonnaire and Emergency information. CHILDREN’S RECORDS: Four children’s records were randomly selected and reviewed today. CURRICULUM: This center implements the Creative Curriculum. Nutrition: Lunches are catered through JMS Catering. Lunches are plated, using a small section of the kitchen and a rolling cart. TRANSPORTATION: This program does not offer transportation services. INCIDENT LOG: The incident log was reviewed during the visit. INSPECTIONS: The last documented annual fire inspection was completed on 5/1/25. I reminded the director that an annual fire inspection was due prior to 5/1/26. The last documented sanitation inspection was conducted on 12/18/25. The last documented fire drill was conducted on 2/11/26. The last documented lockdown drill was conducted on 2/20/26. The last documented playground inspection was completed on 3/5/26. The last documented water testing for Clean Water for NC Kids was completed on 7/4/24. The following violations were cited today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The posted activity plans were for the week of 3/16/26. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. During today's visit there was a opened box of Corn Flakes in the cabinet. All violations must be corrected immediately. TECHNICAL ASSISTANCE was provided today in the following areas: HEALTH AND SAFETY TRAINING: During today’s visit I reminded the director that one lead teacher needed to complete the Health and Safety trainings again in 2026, in accordance with NC Child Care Rule 10NCAC 09 .1103 (b). RATED LICENSE: I received an application on 3/24/26 for Star-rated license reassessment for this facility using Pathway III. Today’s visit was conducted as part of the Rated License assessment process, since it had been longer than six months since the previous full assessment visit. As soon as I receive a Corrective Action letter for today’s visit, paperwork will be submitted to issue this center a Pathway to Stars five-star license. SANITATION: During today’s visit I observed that the kitchen, which is shared with Durham Housing Authority, was messy. The director stated that the Durham Environmental Health inspector, who conducted the 12/18/25 inspection, made the same observation. She stated that the EH inspector wanted to talk with me about adding wording of “shared kitchen space” to the child care license. The purpose of today’s unannounced visit was to monitor applicable child care requirements during a Routine Unannounced visit. The director was not present during today’s visit. Today’s visit was facilitated by the lead teacher. The last Annual Compliance visit was conducted on 9/8/25. This center operates with a five-star license, which was issued 2/1/21. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 97%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: This facility operates as one group of children. I observed the children enjoying the outdoor environment. After outdoor play, the children sat as a group on the rug, while the lead teacher talked with them in English and in Spanish. Then the children became engaged in a variety of indoor free play activities. A timer was used to transition children to the different activity areas. All the required information was posted on the parent information board, to include current activity plans, menus, staff/child ratio chart, and food allergies. The thermometer in the refrigerator was checked. Food for this program is catered through JMC Caterers. Lunch today will consist of turkey/cheese sandwich, lettuce/tomato, carrots, mandarin oranges, and milk. Medication forms were checked for two children. CHILDREN’S RECORDS: children’s records were not reviewed today. STAFF RECORDS: During today’s visit I reviewed the file for one new staff person. During today’s visit I verified that staff had current Criminal Record Check clearance, along with CPR and First Aid. The lead teacher was aware that she needed to complete the Criminal Record Check requalification process before 3/10/26. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 5/1/25. The last documented sanitation inspection was conducted on 12/18/25. TECHNICAL ASSISTANCE was provided today in the following areas: MEDICATION PERMISSION: I reviewed with the lead teacher the information that was needed on the Medical Action Plan and the medication permission form for authorization to administer the medication. ABCMS PROVIDER PORTAL North Carolina child care administrators can now view and edit their facility staff roster in ABCMS, the DCDEE’s criminal background check system. The provider will have access to see background check status for all employees and add/delete applicants on their roster. Access to the ABCMS Provider Portal is granted by the DCDEE Criminal Record Check Unit after completing the required Moodle training and submitting the signed form available at the conclusion of the training. NC child care providers must obtain access to the ABCMS Provider Portal to meet the requirement to record new employees within five days of hire, in accordance with NC Child Care Rules. The Division no longer recognizes or accepts the Change of Information form. RATED LICENSE: This facility plans to complete the star-rated license reassessment, using Pathway III. This center operates with a five-star license, which was issued on 2/1/2021. The last Annual Compliance visit was conducted on 9/17/24. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 97%. During today’s visit indoor and outdoor licensed space was monitored, along with the kitchen. OBSERVATIONS: There were 12 children present today with three teachers. I observed the children engaged in free play activities, having circle time, and eating lunch. Lunches are catered to this facility. Today’s lunch consisted of meat balls, mashed potatoes, milk, and diced pineapple. STAFF FILES: During today’s visit I reviewed the file for the staff person, who began employment on 1/8/25. I verified that staff had current Criminal Record Check clearance and CPR/First Aid training. I reminded one teacher that her CPR/First Aid training would expire on 5/17/25. CURRICULUM: This facility implements Teaching Strategies Gold. Current activity plans were posted. INSPECTIONS: The last documented annual fire inspection was completed on 5/13/24. The last documented sanitation inspection was conducted on 11/8/25. The last documented fire drill was conducted on 2/28/25. The last documented shelter-in-place drill was conducted on 3/3/25. One of the two garden planters on the fenced-in space was completed destroyed. TECHNICAL ASSISTANCE was provided today in the following areas: MEDICATION: During today’s visit I reviewed the medication for three children. The permission to administer a prescribed topical medication to one child indicated that the medication should be administered twice daily. At another place the parent recorded that the medication should be administered as needed. The teacher stated that the child had been absent for an extended period of time. I requested that the administrator talk with the parent to obtain clarifying information when the child returned and before the medication was administered. NUTRITION: Please change the posted menu to reflect the substitutions that were made in today’s lunch. The children were served meatballs instead of beefaroni. RATED LICENSE: Please know that the “hold harmless” status for Rated License reassessment has been extended until the new QRIS guidelines are ready for implementation. I was greeted by Ms. Chatiba Bullock as she assisted me with the visit today. I conducted a general walk-through of one (1) classroom and the outdoor play area. The children were observed engaged in circle time, center activities, and outdoor play. CURRENT LICENSE STATUS Currently this center operates with a five-star license re-issued February 1, 2021, earning seven (7) points in Staff Education, seven (7) points in Program Standards and one (1) Quality Point. ANNUAL INSPECTIONS: *The last annual compliance visit was conducted on 11/16/22. *The sanitation inspection was conducted on 9/6/2023. *The fire inspection was conducted on 5/25/23. *The last fire drill was conducted on 7/27/23. *The last outdoor inspection was conducted on 8/28/23. *The last shelter-in-place or lock down drill was conducted on 11/4/22. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Families & Communities Rising, Inc. is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored. The program does not transport children; therefore, these requirements were not assessed today. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last documented fire drill was conducted on 7/25/23. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A monthly playground inspection shall be conducted by an individual trained in playground safety requirements. I was greeted by Ms. Chatiba Bullock as she assisted me with the visit today. I conducted a general walk-through of one (1) classroom and the outdoor play area. The children were observed engaged in circle time, center activities, and outdoor play. CURRENT LICENSE STATUS Currently this center operates with a five-star license re-issued February 1, 2021, earning seven (7) points in Staff Education, seven (7) points in Program Standards and one (1) Quality Point. ANNUAL INSPECTIONS: *The last annual compliance visit was conducted on 11/16/22. *The sanitation inspection was conducted on 9/6/2023. *The fire inspection was conducted on 5/25/23. *The last fire drill was conducted on 7/27/23. *The last outdoor inspection was conducted on 8/28/23. *The last shelter-in-place or lock down drill was conducted on 11/4/22. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Families & Communities Rising, Inc. is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored. The program does not transport children; therefore, these requirements were not assessed today. The following violations were observed and recorded during today’s visit.
