Upon arrival, I was greeted by the provider, Mary Hill, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a Three Star license issued on July 17, 2017. The program received four points for Staff Education, two points for Program Standards, and one Quality Point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 2/25/25. -The last documented monthly outdoor play area inspection was completed on 1/19/26. -The last documented monthly fire drill was conducted on 1/14/26. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 12/5/25. MONITORING I completed a walk-through of the home, including the children’s space, bathroom, and outdoor learning environment. At the time of the visit, there were five children present ranging in age from seven months to six years old. The children were observed playing freely with materials, getting diapers changed, being fed bottles, reading books with the provider, and playing on tablets. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does not currently transport children; therefore, this requirement was not monitored today. If you decide to provide transportation in the future, be sure to have a first aid kit and fire extinguisher in the vehicle, as well as children’s files containing their photos, permission to transport forms, emergency contact information, and emergency medical authorization. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and found in compliance. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Children are cared for in the lower level of a split-level home. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The bathroom is located within sight of the children’s play area. Portable cribs and mats are used for rest time. The outdoor play area is a fenced backyard that includes a several slides, climbers, and play kitchen centers. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 306 At least one 5 lb 2-A:10-B:C type fire extinguisher was not readily accessible for every 2500 square feet of floor space in the home. The fire extinguisher was empty. 10A NCAC 09. 1707(4) 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. The tv was playing children's programming while an infant and toddler were present. .1718(c) 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. A newly enrolled student's file was missing a signed safe sleep policy and signed summary of law. G.S. 110-91(9) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Attendance was documented inconsistently. .1721(e)(6) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by February 20, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact your overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Providers are required to keep daily attendance records for all children in their care, including drop-off and pick-up times. Accurate attendance records show that you remain in compliance with child care ratios as well as provide a list of children present in the event of an emergency. A review of your attendance records showed that you did not have attendance sheets for all children, and some were not documented consistently. I recommend placing the attendance log next to the front door for parents (or you) to sign children in and out each day. Please document when children are absent as well. -A newly enrolled student’s file was incomplete today. It was missing a signed safe sleep policy and a signed Summary of Law document. To ensure you have all required forms, I recommend collecting them from the parent before you begin caring for the child. You can also use the children’s file checklist available on the DCDEE website under the “Provider” tab. -For food safety, it is important that you keep refrigerator temperatures at 45 degrees or below. Today, a refrigerator used to store milk did not have a thermometer inside for checking the temperature. You said you thought it was in there, but you had an extra thermometer you placed in the refrigerator during my visit. Please be sure to check your refrigerator temperature daily before using it to store perishables. -Today upon my arrival, I observed a television showing children’s programming turned on in the room while toddlers were present. Screen time is not allowed for children under three years old, and screen time is limited to 30 minutes per day for a total of no more than 2.5 hours per week for children ages three and older. Children younger than three years old should be provided with alternate activities when older children have screen time. -It is important to have a working fire extinguisher within reach in case of fire. Today, you fire extinguisher was empty. You said you had recently attempted to have it recharged, but were told it would be less expensive to purchase a new one. You said you had not had a chance to buy a new one yet. Please make it a priority to obtain a new fire extinguisher this weekend before children return on Monday. CONSULTATION -We reviewed QRIS information and completed a form stating that you will pursue Pathway #2 (Classroom and Instructional Quality), with a planned license transition date of August 2026. You stated that you will reach out to Early Years for assistance in preparing. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. This is a requirement under Child Care Rule 10A NCAC 09 .1725 (a)(2). -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. This includes “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) which should be renewed every five years. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/ -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires 10/10/27. -ITS-SIDS training must be renewed every three years. RATED LICENSE STATUS New QRIS rules are in effect, and the Division is transitioning programs to their new rated license in 2026. Upon arrival, I was greeted by the provider, Mary Hill, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a three-star license issued on July 17, 2017. The program received four points for staff education, two points for program standards, and one quality point. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 4/9/24. -The last documented monthly outdoor play area inspection was completed on 1/6/25. -The last documented monthly fire drill was conducted on 1/6/25. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 11/4/24. MONITORING I completed a walk-through of the home, including the children’s space, kitchen, bathroom, and outdoor play area. At the time of the visit, there were five children present ranging in age from 14 months to five years old. There are eight children enrolled. The children were observed playing freely with materials in the play space, getting a diaper change, cleaning up, playing outside, and washing hands. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does transport children; therefore, this requirement was monitored and found in compliance. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are enrolled; therefore, safe sleep requirements were monitored and found in compliance. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in the lower level of a split-level home. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, and home living props. The bathroom and changing table are located within sight of the children’s play area. The outdoor play area is a fenced backyard, and it includes a variety of slides, climbers, and play kitchen centers. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. A pet dog's rabies vaccination expired on 1/8/25. .1719(b)(1) 915 The written schedule did not show blocks of time assigned to types of activities and/or include periods of time for both active play and quiet play or rest. Two pieces of equipment with broken/jagged plastic were in the outdoor play area and accessible to children. .1719(a)(17) 1964 Activity plan did not reflect that children have at least four different activities daily, at least one of which is outdoors. No activity plan was available for review. 10A NCAC 09 .1718(a)(8)(C)(i-v) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by March 11, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 92%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Safety is a top priority when providing care for young children. Today I observed broken/jagged pieces of plastic on two of your outdoor play items (climbing cube and play kitchen). You stated that you believed the damage happened yesterday and you would remove the items promptly. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any potential hazards. -It’s important to ensure that any pets in the home who might come into contact with children are up-to-date on their vaccines. Your dog’s rabies vaccine expired on 1/8/25 and needs to be updated immediately. You stated that you thought the dog’s vaccines were current, but you would contact your vet to update them. -No activity plan or schedule was available for review today. It is important to have an activity plan to share with parents so they are informed about what their children are learning. A schedule helps ensure that children have time during the day for group and individual play, personal routines, and outdoor time with gross motor activities. Be sure to use the Foundations document (on the DCDEE website) to guide your activity planning and help you address all learning domains. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly, and be sure to add me as your consultant. -Criminal Backgrounds Checks for the provider and all household members 16 years of age and older should be completed every five years. Your qualifying letter is valid until 2/25/26. -Remember to conduct a fire drill, lockdown/shelter-in-place drill, and outdoor inspection before the end of this month. TRAINING REMINDERS -Training from 4/9/24 to 2/24/25 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. All training hours have been completed for this year and 1.5 hours will carry over to apply to next year’s training requirement. -CPR and First Aid must be renewed every two years. Your CPR and First Aid certification expires on 5/10/25. -ITS-SIDS training must be renewed every three years. Your training expires on 9/19/27. -Health and safety training must be renewed every five years. Your training expires in 2027. Also, “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be renewed every five years. At the time of the visit, there were three (3) children present ranging in age from three (3) to four (4) years old. The children were observed having screen time, playing in centers, playing a matching game, interacting with the provider, and completing routine tasks. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Three (3) star license issued June 29, 2018, earning four (4) points in staff education, two (2) points in program standards and one (1) quality. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials and equipment. The provider does not have a child under 12 months enrolled, therefore safe sleep requirements were not monitored. No children enrolled have medications or medical care plans, so these requirements were not monitored. Transportation is provided; however, the van was not able to be monitored. The provider stated that the van was at the shop being repaired. I monitored the providers driver’s license and vehicle insurance. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 3/1/24. *The last documented shelter-in-place or lockdown drill was conducted 2/1/24. *The last documented playground inspection was conducted 3/1/24. I would recommend using the screen time log to document the actual day and time the screen time started. Once screen time is over, I would recommend that you then document the time. I also recommend you go to the Caring for Kids website (nrckids.org) and read about Screen time. STORAGE OF HAZARDOUS MATERIALS The provider stated that she is having her home painted after the children leave in the afternoon. The paint was not removed while children were in attendance. I also observed gain laundry detergents pods, and baking soda that stated ‘keep out of reach of children’ in reach of the children. I would recommend checking after the painters to make sure the paint is put away once they are complete each night. CHILDREN FILES I would recommend you using the children file checklist to make sure all paperwork is completed and filed on or before the child’s first day of attendance. ATTENDANCE I would recommend that you as the provider check daily to make sure attendance has been recorded for all children daily. I would recommend using the state provided attendance form. I provided a copy of the state attendance form. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 1. Your reassessment year will be between July 1, 2024, and June 30, 2025. I discussed with the provider to consider going through FCCERS-R assessment during June 2024. I also discussed that if the FCCERS-R assessment is completed during the prep year and the scores are liked they can be used immediately upon the administrators request or be held until the actual assessment year.
