You, Shirley Leake, Owner/Operator, assisted me with the visit. Your program currently operates with a four-star license, issued September 20, 2019, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for serving no more than two infants under age one. The license was posted, and the restrictions were in compliance. A walk-through of the fcch was completed today; monitoring all areas used by the children. I observed children in both the indoor learning environments and found supervision and capacity to be in compliance. The two preschool children present were participating in activity areas. The last annual compliance visit was conducted on 5/13/25. The last documented fire drill, outdoor inspection and shelter-in-place drill were all completed on 1/6/26. One violation was observed, and must be corrected immediately. The violation is as follows: Violation Number Comment Rule 1739 All records required shall be available during operating hours, for review by a representative of the Division. The outdoor inspections from June-November 2025 were not available for review. .1721(f)(7) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The program’s compliance history was Ninety-Nine percent as of 1/5/26. The violation documented must be corrected immediately. On or before January 26, 2026. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how the violations were corrected. Email the letter to: Kia.Reid@dhhs.nc.gov The compliance letter must include: -Facility name -Facility ID -Your name and signature TECHNICAL ASSISTANCE Keep your inspections for 2025 on file for one year. OPERATOR RECORDS Your file was monitored for criminal background qualifying letters, current CPR/First Aid, and other specialized training. On your next annual compliance visit, on-going training hours will be monitored. RATED LICENSE INFORMATION During today’s visit we discussed the new QRIS pathways to the Stars, and you were given an opportunity to ask questions. You stated you will visit the DCDEE website to review the new Rated License requirements. The following resources are in the QRIS section. -Pathways to the Stars Family Child Care Home Session Slide Deck -Pathways to the Stars Questions and Answers -Frequently asked questions As of February 1, 2025, the FCCERS-3 will be used for the assessments. Access www.ncrlap.org to participate in webinar sessions that will focus on the changes. I stated that the webinars could be used to meet yearly in-service training hour requirements. A technical assistance visit was scheduled for 3/2/25 to discuss which pathway will be chosen and complete the QRIS conversation template. CHILD CARE RULES Child Care Rules have been updated as of 7/1/25. Please visit the DCDEE website to review and download a copy. The following free trainings are available on the Moodle portal. -Child Development module -Summary of Rule Changes Effective July 1, 2025 (Pathways to the Stars Rule Rollout) CRIMINAL BACKGROUND CHECK *Your qualification letter will expire on 7/1/26. The provider, Shirley Leake, was present and assisted me when needed. This documentation was reviewed with Ms. Leake, and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by Ms. Leake. I completed a general walk through of the areas used by the children inside. The outdoor play area was not monitored due to the rain. The three (3) preschool children present were observed in story time with the Durham County librarian then in free play activities with the provider. CURRENT LICENSE STATUS Currently this family child care home operates with a four-star license issued September 20, 2019, earning seven (7) points in Staff Education, two (2) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on May 15, 2024. *The last fire drill, outdoor inspections were both completed on May 5, 2025. *The shelter-in-place drill was completed on April 4, 2025. DESIGNATED CHILDREN’S SPACE In the children’s space, there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to those that are potty trained. Activities take place in the child care space and dining area. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 908 Health questionnaire was not completed annually. The last health questionnaire was completed on 5/1/24. .1703(a)(1) 920 Records were not made available for review. The child receiving drop-in care did not have paperwork on file. G.S. 110-91(9) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The last documented day of attendance was 5/5/25. .1721(e)(6) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by May 27, 2025. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the fcch was 96%. According to NC General Statute 110-90 (4)(d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS I monitored the provider's file, three children’s files, and required program records. The on-going training requirements were monitored from May 2024-2025 and was found to be in compliance. TECHNICAL ASSISTANCE *Drop-in children must have paperwork on file while in care. It is very important to ensure that the application is on file in case of an emergency. *Attendance records must be completed daily. When completing your meal count sheets check the attendance records to make sure the information has been completed for the day. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *Your ITS-SIDS training will expire on 10/24/25. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you.
