The purpose of today’s visit was to monitor applicable child care requirements during the Annual Compliance visit. This Family Child Care Home operates with a three-star license, which was issued on 5/10/19. The last Annual Compliance visit was conducted on 3/13/25. The compliance history for the previous eighteen-month timeframe was 100%. OBSERVATIONS: There were 5 infant/toddler age children present. I observed the children playing in child care space, drinking their bottle of milk, having their diapers changed, and napping. The operator had a daily schedule posted, along with current activity plans. CHILDREN’S RECORDS: The children’s records were reviewed today. OPERATOR’S FILE: Upon arrival the operator gave me the completed enrollment worksheet, signed statements of understanding, and operator verification form for herself and the additional caregiver. The three documents were reviewed with the operator. TRANSPORTATION: This Family Child Care Home is not currently offering transportation services. SMOKE DETECTORS: The electrically operated smoke detector with battery back-up was tested during the visit. INCIDENT REPORTS: The operator stated that no incidents had occurred to record in the incident log. INSPECTIONS: The last documented fire drill was conducted on 1/27/26. The last documented shelter-in-place drill was conducted on 1/29/26. The last outdoor inspection was conducted on 1/27/26. The operator completed the Clean Water for NC Kids on 2/14/23. The operator will request bottles to submit new water samples for an updated clearance. The operator completed Clean Classrooms for Carolina Kids on 4/26/24. The following violations were cited today: Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. A medication permission form needed to be completed by the parent to administer an Epi-Pen to a child with a food allergy. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator and the additional caregiver needed to complete additional training hours to meet their yearly requirement. GS 110-91(11); 10A NCAC 09 .1705(b)(5) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 3/12/26, stating how the violation has been corrected. TECHINCAL ASSISTANCE WAS PROVIDED DURING THE VISIT ON THE FOLLOWING TOPIC AREAS RATED LICENSE: On 2/20/26 I received an email from the operator, stating that she wanted to operate with a one-star license and not complete the QRIS process for a license with high stars. During today’s visit the operator stated again that she would operate as a one-star Family Child Care Home. The operator signed her email to confirm her decision.
