The visit summary was reviewed with Kathy Smith, Owner/Director, and a copy was given to her at the conclusion of the visit. Upon arrival, I was greeted by the lead teacher in space #3. She informed me that Ms. Smith was in the infant classroom. I completed a general walk through of the indoor and outdoor areas. The infants and toddlers were observed having their diapers changed, singing songs with their teachers, and outdoor play. The preschool children were observed in center activities, outdoor play and eating lunch. LICENSE STATUS Currently this center operates with a three-star license issued April 11, 2022, earning three (3) points in Staff Education, six (6) points in Program Standards and one (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted August 13, 2024. *The sanitation inspection was completed on May 1, 2025, with a "Superior" classification. *The fire inspection was completed on June 24, 2025, and the facility is approved for day time care only. *The last documented fire drill and lockdown drill were both conducted on June 5, 2025. *The last documented playground inspection was completed on July 3, 2025. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Little Engine Academy @ Bragtown, LLC is listed as current/active. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violation was observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The chain link fence on the left side of the playground is damaged and the music wall is broken leaning against the bent fence. 10A NCAC 09 .0601(a) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by August 7, 2025. The letter must address each violation and explain how it has been corrected. Please send this to Kia.Reid@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Item number COMPLIANCE HISTORY Prior to today's visit, the Compliance History Score for the center was 95%. According to NC General Statute 110-90 (4)(d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE ANNUAL COMPLIANCE RECORDS Ten percent of children’s records were monitored. The staff and training worksheets were received prior to the visit. Files for the new staff were monitored as well as ten percent of existing staff files. The on-going training requirements were monitored from August 2024-July 2025 and was in compliance. The director’s signature attests that staff files not monitored today contain all the required information. TECHNICAL ASSISTANCE FENCE DAMAGE Ms. Smith explained that a few trees from the neighbor’s yard next door fell onto the playground damaging the fence during Tropical Storm Chantel. The child care center did not receive any damage. Ms. Smith has contacted the insurance company and is waiting for someone to assess the damage. CONSULTATION While the QRIS modernization rules are not yet in effect, I encourage you to visit the DCDEE website in preparation for the new requirements. We are going away from a point-based system allowing you to have three different pathways to achieve stars. You will find the following resources in the QRIS section. -Webinars -Frequently asked questions -Updated Program Assessment tool (Environment Rating Scales-ERS) As of February 1, 2025, new Environment Rating Scales will be used. The ITERS-3, and ITERS-3, also known as the "3s” will be used for all assessments. Today’s unannounced visit was a follow-up of the 2/14/24 visit. During today’s visit I visited each classroom. Each classroom was maintaining adequate staff/child ratios during the visit. OBSERVATIONS: I observed the children playing in centers and putting on hats and coats to go outside. I observed the infants having tummy time and being fed their bottle of milk. STUBSTITUE STAFF: The co-owner of the business was currently the substitute cook. Today's lunch consisted of tuna salad, lima beans, mixed fruit, bread, and milk. During today’s visit I reviewed his file. The following violations were cited today: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The person, who was cooking, did not have Criminal Background Check clearance. G.S. 110-90.2(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. The cook did not have a negative TB test on file. .0701(d) All violations must be corrected immediately. Please send me a signed Corrective Action letter by 3/6/24, stating how the violation cited today has been corrected. TECHNICAL ASSISTANCE WAS PROVIDED TODAY ON THE FOLLOWING TOPICS: CRIMINAL RECORD CHECK: I explained to the owner/director that her husband (co-owner of the center) could not work at the center until he had obtained Criminal Background Check clearance. After obtaining Criminal Record Check clearance, the co-owner can resume cooking at the center. I explained that the need for Criminal Background Check clearance was not based on center ownership, but on his current role at the center. I discussed the situation with the licensing supervisor during the visit. Technical Assistance was providing on cooking options until the co-owner received the required Criminal Record Check clearance. RATED LICENSE: This center is in Cohort I for the Rated License reassessment. Based on the conversation during the 2/14/24 visit, I stated that I would plan to code the next Annual Compliance visit as Annual Compliance w/Rated License reassessment. I will send the owner/director the application prior to the Annual Compliance visit. Please contact me if you change your mind about the Environment Rating Scales, please contact. Please have everyone registered in the WORKS education database by the Annual Compliance visit, so I can access staff education and submit the paperwork for a new license.
