Upon arrival, I was greeted by the provider, Angela McDonald, who assisted me during today’s visit. LICENSE STATUS The family child care home currently operates with a three-star license issued on April 21, 2020. The program received five points for staff education, two points for program standards, and one quality point for serving no more than two infants under the age of one. INSPECTIONS/DRILLS STATUS -The last annual compliance visit was conducted on 11/15/23. -The last documented monthly outdoor play area inspection was completed on 9/16/24. -The last documented monthly fire drill was conducted on 9/16/24. -The last documented quarterly lockdown/shelter-in-place drill was conducted on 8/12/24. MONITORING I completed a walk-through of the home, including the children’s space, downstairs kitchen, bathroom, and outdoor play area. At the time of the visit, there were five children present ranging in age from two years old to four years old. There are nine children enrolled, including four who attend after school or second shift. The children were observed in free play with center materials, listening to a story in circle time, dancing to music, coloring, playing with play-doh, engaging in an art activity, playing outside, and eating lunch. Lunch consisted of lasagna, peas, pineapple, and milk. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, capacity, health and safety requirements, equipment and furnishings, and program records. The provider does transport children; therefore, this requirement was monitored and found in compliance. No enrolled children have medications or medical care plans, so these requirements were not monitored. Children 12 months or younger are not enrolled; therefore, safe sleep requirements were not monitored. The provider verified that all the information on the FCCH Verification of Required Information for Operator and Additional Caregivers form was correct. INDOOR/OUTDOOR ENVIRONMENT Inside the home, children are cared for in a space on the lower level of the home with a separate entrance. The space is divided into three areas and is set up like a preschool room with centers. A variety of items for learning through play are available, including manipulatives, blocks, books, art materials, musical instruments and home living props. The bathroom is located in a hallway just outside the children’s space, and a changing table is inside the children’s space as well as two cribs and cots for napping. There is a child-sized table for eating and for play. The outdoor play area is a fenced backyard. There is an above-ground pool which is fenced separately with a locked gate so it is inaccessible to children. I was unable to monitor the outdoor space today due the rain and mud. Instead, children played briefly in the driveway. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The provider's qualification letter expired on 9/11/24. G.S. 110-90.2 & .2703(n) & (o) 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. The provider's husband's background check expired in September 2024. The provider's son recently moved back home and has not completed a background check. G.S. 110-90.2 & .2703(q) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The EPR plan was not available for review. .1714(e ) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by October 15, 2024. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the facility’s compliance history score was 97%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The facility was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your program. TECHNICAL ASSISTANCE -Criminal Backgrounds Checks for providers and all household members ages 16 and older should be completed every five years. Your last qualifying letter expired 9/11/24 and must be renewed immediately. You and your household family members have 15 days to obtain new qualifying letter/background check; after that, you will not be able to keep children in child care until you receive your new qualifying letter. -You shared today that you are having trouble logging in and accessing your EPR plan. You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. CONSULTATION -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Visit www.cleanwaterforuskids.org/carolina to enroll by November 1, 2024. -To better organize your children’s files, you should consider using the children’s file checklist available on the DCDEE website in “Provider Documents and Forms” under the “Provider” tab. -Although you take your children's files with you when you transport them, it is best practice to keep the children's emergency information and photos in the van so you don't have to remember to take them back and forth. I recommend you make a copy of only the children's health assessments and emergency medical care plans to keep in the vehicle, and leave the complete files at home. TRAINING REMINDERS -Training from 11/15/23-9/30/24 was reviewed today and documented on the Verification of Required Information for Operator and Additional Caregivers form. Please remember to send me your certificate for the “Business Side of Childcare” class when you receive it. -ITS-SIDS training must be renewed every three years. Your certification is valid through 5/22/26. -Health and safety training must be renewed every five years. Your training will need to be renewed in 2027. -CPR and First Aid must be renewed every two years. Your certification is valid through 5/7/26. -“Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be renewed every five years. You are due to renew by the end of 2024. This training can be found on the Prevent Child Abuse NC website: https://preventchildabusenc.org/online-trainings/ RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Please use this time to update your WORKS account with your education information. You can visit the North Carolina Rated License Project website (https://www.ncrlap.org) for resources regarding the rated license assessment process and the FCCH self-study. A checklist was used to note the requirements monitored today. The Family Child Care Home Provider, Angela was present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were five (5) children present aging in range from one (1) to four (4) years old. The children were observed, coloring, playing in centers, participating in circle time, and transitioning to outside play. LICENSE STATUS The Family Child Care Home (FCCH) operates with a Three (3) star license issued April 21, 2020. The facility is approved for first, second and third shift with a maximum of five preschool children at any time and serves no more than two (2) infants under the age of one (1). MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children, including the outdoor play environment. The provider’s and children files were reviewed. All spaces were monitored for supervision, child capacity, materials, and equipment. The provider does not provide transportation; therefore, this requirement was not monitored. The provider does not have children under 12 months enrolled, therefore safe sleep requirements were not monitored. No children enrolled have medications or medical care plans, so these requirements were not monitored. Outdoor Environment I observed an above-ground pool in the backyard. The provider had fencing and a lockable gate installed around the pool. The pool was also observed covered. The provider stated that the fire pit and all motorized equipment was removed. I did not observe the motorized fire pit during today’s visit. The FCCH provider verified the information on the Verification of required information for operator and additional caregiver’s form to be accurate. --PROGRAM RECORDS *The last documented fire drill was conducted 11/8/23. *The last documented shelter-in-place or lockdown drill was conducted 11/8/23. *The last documented playground inspection was conducted 11/2/23. DESIGNATED CHILDREN’S SPACE The child care space is located in the downstairs of the home. There is a separate entrance to the children’s space located on the left side of the home. The children's space has a variety of manipulatives, pretend play toys, and books for the children's use. The bathroom is just outside of the children’s space on the left and is accessible to the children that are being potty trained. A changing table is in the infant area for those in diapers. The children eat their meals in this area. Mats, cribs and a bassinet are used for nap and rest time. The outdoor play area is located in the backyard of the home, and it is fenced. EMERGENCY MEDICAL CARE PLAN I discussed and suggested to the provider to post the Emergency Medical Care Plan in an area that it would be easily accessible in case of an emergency once the form has been completed. I also suggested that she update it annually when she changes the batteries to the smoke detector. CHILDREN FILES I would recommend that the Children’s Checklist be utilized to ensure that all forms are collected on or before the start date. EMERGENCY PREPAREDNESS AND RESPONSE PLAN The Emergency Preparedness Response Plan (EPR) will need to be updated annually. I discussed with you that you will need to use your NCID to log in and update. You will need to go to rmp.nc.gov/portal to update your plan. I would recommend that you utilize your outlook calendar to remind you when to update. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. RATED LICENSE STATUS On Monday June 12, 2023, the hold harmless legislation was extended and signed into law. All facilities are allowed to remain at their current star level without reassessment until June 30, 2024. Your facility has been determined to be in Cohort 2. Your reassessment year will be between July 1, 2025, and June 30, 2026. I will reach out to you prior to the beginning of your preparation year. Please use this time to make sure all your staff have a WORKS account with all education information updated. Also review updated resources on the North Carolina Rated License Project (NCRLAP) website (www.ncrlap.org). You may also want to consider the FCCERS-R self-study process and timeline for completion since it must occur over a 3-month time period prior to your rated license reassessment. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2017, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years.
