The purpose of today’s visit was to monitor applicable child care requirements during a Routine Unannounced visit. The last Annual Compliance visit was conducted on 11/13/25. This center operates with a five-star license, which was issued on 3/27/26. CORPORATION STATUS: According to the NC Secretary of State website, the status of Families and Communities Rising, Inc. is “current-active”. COMPLIANCE HISTORY: The compliance history for this center for the previous eighteen-month timeframe was 96%. During today’s visit all indoor and outdoor licensed space was monitored. OBSERVATIONS: Today I observed two classrooms of children enjoying the outdoor environment. I also observed teacher-directed activities, circle time, and free play in other classrooms. CHILDREN’S RECORDS: Children’s records were reviewed today. STAFF RECORDS: During today’s visit I reviewed the files for three staff, who began employment at this center after the 11/13/25 Annual Compliance visit. I verified that all staff have current Criminal Record Check clearance, along with current CPR/First Aid training. I verified that staff, who work with infant age children, have IT’S SIDS training. Each classroom had current activity plans, current menu, and food allergy lists posted. TRANSPORTATION: This program does not offer transportation services. INSPECTIONS: The last documented annual fire inspection was completed on 5/4/26. The last documented sanitation inspection was conducted on 4/14/26. The last documented fire drill was conducted on 4/25/26. The director stated that a fire drill for the month of May would be conducted later today or tomorrow. The last documented lockdown drill was conducted on 3/11/26. The last documented playground inspection was completed on 5/4/26. The last documented water testing for Clean Water for NC Kids was completed on 4/24/24. The center had received an exemption on 8/14/25 from Clean Classrooms for Carolina Kids requirements. The following violation was cited today: Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. One child, under the age of 15 months, did not have a feeding schedule on file. 10A NCAC 09 .0902(a) Since the one violation cited during today’s visit was corrected during the visit, a Corrective Action letter was not requested. FLOORPLAN: During today’s visit the consultant was given an updated floor plan for the center. The use of each room was labeled, using letters for classroom identification. TECHNICAL ASSISTANCE was provided today in the following areas: SAFE INDOOR ENVIRONMENT: During today’s visit objects fell from the top of a tall cabinet in classroom #F when the teacher retrieved and returned the medicine bag for review by the consultant. The children were seated at the table at the cabinet when the items fell. The items did not strike the children. I observed that multiple materials were stored on the top of the cabinets in classroom space #F and other classrooms. As Technical Assistance, I discussed this safety situation with the director. Before the end of the visit, the director discussed her plan for changing storage practices to include installing a hook, high on the wall, for the medical bag, which is carried outside on the playground with the children every day. HEALTH AND SAFETY TRAINING: I reminded the director that one staff needed to complete the Health and Safety trainings again in 2026, in accordance with NC Child Care Rule 10 NCAC 09 .1103(b). The program manager, LaDrenna Jackson, and I conducted a walkthrough of the indoors and outdoors spaces. All spaces used for children were monitored. There were 109 children, aged 0 years through 5 years of age present today. Children were observed eating breakfast, free play, and outdoor play. CURRENT LICENSE STATUS: Your center currently operates with a Five Star License, issued on February 14, 2018, earning 7 points in Education, 7 points in Program Standards and 1 Quality point for benefit package. The program Families and Communities Rising, Inc. was current and active with the North Carolina Secretary of State. INSPECTION/SAFTEY DRILL STATUS: The last sanitation inspection was completed on 11/8/23. The last fire inspection was completed on 5/25/23. The last outdoor play area inspection was completed on 12/1/23. The last fire drill was conducted on 11/30/23. The last Shelter in Place/Lockdown was conducted on 12/5/23. The last update EPR plan was on 9/7/23. The last Annual Compliance was conducted on 1/25/23. PROGRAM RECORDS: Staff on-going training requirements were assessed from January 2023 – January 2024. Twenty children’s records were viewed and were also in compliance. ITEMS MONITORED: During the visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The Staff Training Worksheet and Fire Inspection were emailed to me prior to today’s visit. Your signature on these forms will serve as verification that the information provided is complete and accurate. THE FOLLOWING VIOLATIONS WERE OBSERVED DURING TODAY’S VISIT. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhale, or handled were not stored in a locked room or cabinet. Lysol and other hazardous chemical were store accessible to children in space H & I. .2820(b) A Corrective Action Letter is not required because the violation was corrected during the visit. COMPLIANCE HISTORY: Prior to today’s visit your compliance history was 93%. RATED LICENSE You are currently in your preparation year; it will end on June 30, 2024. Your assessment year will start on July 1, 2024. To take advantage of the benefits now available, you must decide when you want to have the assessment done. Remember you can decide to count your score toward your assessment year if you are pleased with the result. I am leaving a copy of the Rated License Assessment Request Review with you today. Please email the form when completed. TECHNICAL ASSISTANCE: The information below was shared with you to maintain quality within your program and to prevent future violations. TRAINING P. Morgan training hours is due on January 31, 2024. You can email the training hours on or before the due date. Employees that have only high school education may want to use this pathway to meet lead teacher status. The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. WORKS Now is the time to make sure all staff are enrolled in NC WORKS and their information is current. To enroll in training and view information in the DCDEE Moodle tool, you must use your NCID - the same NCID that is used for WORKS registration, and/or the criminal background check (CBC) portal. The summary of the North Carolina Child Care Law and Rules was revised in September 2023. Please make sure you have updated your posting on the parent board. ANNUAL LICENSING FEE The Division of Child Development and Early Education is required by law [North Carolina General Statute 110-90(1a)] to establish an annual licensing fee for child care facilities. Payment must be made on the DCDEE website using a Mastercard/Visa debit or credit card, or by e-check. You will need your Facility ID number and invoice number to complete the payment process. The Annual Licensing Fee is due by December 31, 2023. Failure to pay the license fee is a violation of child care law (N.C.G.S. 110-90(1) (a) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action, up to and including revocation of the child care license. Ms. LaDrenna Jackson, center director was present and assisted me during the visit. CURRENT LICENSE STATUS: Your center currently operates with a Five Star License, issued on February 1, 2021, earning 7 points in Education, 7 points in Program Standards and 1 Quality point for enhanced ratios. There were 94 children, aged 0 year through 5 years of age present today. There were also 16 staff members present during the visit. Children were observed during naptime, eating snack, and preparing for center activities. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 4/26/23 with a “Superior” classification. The last Fire Inspection was completed on 5/25/23. The last Fire Drill was conducted on 8/30/23. The last emergency drill/lockdown, conducted on 9/8/23. The last outdoor inspection was conducted on 9/1/23. The last annual compliance visit was conducted 1/25/23. A partial monitoring of applicable child care requirements was conducted on today’s visit. I did not assess transportation requirements because the program does not transport children. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Albuterol Sulfate authorization form did not have dates to administer. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In a pre-k classroom an Epinephrine Injection expired on 4/21. .0803(12) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff hired on 2/28/23 didn't have verification for completing First Aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff hired on 2/28/23 didn't have verification of completing CPR training. .1102(d) While the Division allows you time to explain how you have corrected violations when violations are documented, it is expected that you correct all violations immediately. When you send your compliance to me, it verifies that you have already corrected or addressed the violations from your visit summary. If I do not receive your letter by September 26, 2023, and/or if you do not address all violations, a return visit may be made to your program to monitor for compliance of violations and to obtain your compliance letter. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. VERIFICATION OF RECORDS During today’s visit the following items were monitored: CBC qualification, CPR/FA, and Recognizing and Responding to Suspicious of Child Maltreatment. COMPLIANCE HISTORY: Prior to today’s visit your compliance history was 98%. TECHNICAL ASSISTANCE HEALTH AND SAFETY TRAININGS: Several staff completed the Health and Safety Training in 2018. The training must be completed every five (5) years. Visit the DCDEE Moodle website (https:// www.dcdee.moodle.nc.gov/). to begin taking the health and safety training. All staff must complete the “Recognizing and Responding to Suspicions of Child maltreatment” training. Please go to www.preventchildabusenc.org to access the training. RATED LICENSE TIMELINE: Your planning period begins 7/1/23 and your Assessment period starts 7/1/24. Make sure to update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS). The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. SANITATION RULE REVIEW Children’s Environmental Health has set up virtual trainings for child care center operators on the newly readopted child care sanitation rules. The dates for the trainings are September 20, 2023, and September 26, 2023; both scheduled from 9:00 am – 11:30 am. These trainings are identical, so only one will need to be attended.
