The visit summary was reviewed with the Principal, Jeri Jackson, and a copy was given to her at the conclusion of the visit. After signing in, I walked to the licensed classroom (classroom #131). The children were observed in center activities, circle time, and going to the cafeteria to eat lunch. The lead teacher and teacher were both out sick today. Two substitutes provided care for the ten (10) children present today. CURRENT LICENSE STATUS Currently this program operates with a five-star license issued April 21, 2017, earning 7 points in the Staff Education Component, 7 points in the Program Standards Component and 1 Quality Point. INSPECTIONS *The last Annual Compliance Visit was completed on September 24, 2024. *The sanitation inspection was completed on April 16, 2025, with a “Superior” classification. *The last documented fire drill was conducted on April 11, 2025. *The last documented shelter-in-place drill was conducted on March 7, 2025. *The last fire inspection was completed on February 6, 2025. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The two substitutes providing care today did not have emergency information on file. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The two substitutes caring for the children today did not have First Aid training on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The two substitutes caring for the children today did not have CPR training on file. .1102(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two children currently enrolled did not have a medical exam on file. GS110-91(1) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. According to NC General Statute 110-90 (4)(d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. ANNUAL COMPLIANCE RECORDS I reviewed children's records, staff files and program records. I informed Ms. Jackson of the paperwork substitutes needed to have on file when caring for the children. I left a substitute file checklist for reference. Although the substitutes have gone through the criminal background check process with DPS, they are required to complete the process through DCDEE while working in the licensed child care space. Please return the pre-service for administrator form to me by 6/12/25. REMINDERS The following requirements will come due in the near future; please make arrangements to complete the items before the deadline. *You must complete a fire drill by 5/31/25. *You must complete a shelter-in-place or lockdown drill by 6/9/25.
