Upon arrival I was greeted by Vanessa Hernandez, Director. I completed a general walk-through of the center. At the time of the visit, there were a total of thirty (30) children present, ranging in age from one (1) to four (4) years old. The children were observed playing outside, interacting with the teachers, eating lunch, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued September 17, 2019, earning seven (7) points in education, seven (7) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility Lakewood Children’s School LLC listed as current/active. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements for one newly hired staff and one newly enrolled child. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children. INSPECTIONS *The last fire drill was conducted on 4/30/25. *The last emergency drill was conducted 3/10/25. *The last playground inspection was completed 4/11/25. *The last sanitation was completed on 4/23/25 with a “Superior ‘’ classification. *The fire inspection was conducted on 3/19/24. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was conducted on 3/19/24. 10A NCAC 09 .0304(a) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Insect repellent and Neutrogena sunscreen in space #1 did not have written authorization from a child's parent. 10A NCAC 09 .0803(1)(a & b) 843 A drug or medicine was administered after its expiration date. Aquaphor and Think Baby sunscreen was observed in space #1 with an expiration date of 3/2025. 10A NCAC 09 .0803(1)(d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before May 15, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 93%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. MEDICATION Insect repellent, and Neutrogena sunscreen did not have an authorization form completed on file from the parents observed in space #1. Aquaphor and Think Baby sunscreen was observed with an expiration date of 3/2025. I would recommend checking all medications and forms monthly to make sure all forms and medications are up to date. ANNUAL INSPECTIONS The last fire inspection was conducted on 3/19/24. I would recommend setting up an outlook reminder to remind you of the upcoming fire inspection. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. The director, Sarah, assisted us during the visit. Child Care Consultant, Mandy Annunziata accompanied me on today’s visit. Upon arrival, you greeted me and welcomed me into your program. Three (3) classrooms were open during today's visit. The children were observed exploring their indoor learning environment, eating morning snack, and having circle time. Some of the children were observed exploring their outdoor learning environment. RATED LICENSE STATUS The program holds a 5 star-rated license on 9/17/19. The program is in the “held harmless” status and points from the previous assessment are carried forward. The program received 7 points in Program Standards, 7 points in Staff Education, and 1 Quality point through the educational option of at least 75% of lead teachers having a BA, BS, or higher degree in Early Childhood Education. INSPECTIONS The last sanitation inspection was conducted on 2/2/24. The last fire inspection was conducted on 3/19/24. The last fire drill was conducted on 5/13/24. The last emergency drill was conducted on 5/25/24. The last playground inspection was conducted on 5/13/24. The following items were monitored during today's routine unannounced visit: indoor and outdoor area, supervision, capacity, CPR & 1st Aid certifications, CBC’s completed, storage of hazardous substances, storage of medication, adequate and approved space, provider records, license posted and permit restrictions. The following violations were observed and recorded during today's visit. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The refrigerator thermometer did not maintain a temperature of 45 degrees or below. The thermometer in kitchen refrigerator temperature is reading 55 degrees. 15A NCAC 18A .2806(j)(2) 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. Cots were not stored properly with their sheets on them in space #3, space #5 and space #6. 15A NCAC 18A .2821(a) 807 A safe indoor and outdoor environment was not provided for the children. A broken dump truck was on the younger children playground. 10A NCAC 09 .0601(a) In order to comply with the NC Laws & Rules, any violations cited today must be corrected immediately. A compliance letter should be sent to me by June 13, 2024. The letter should address each violation, and explain how and when it has been corrected. It will be updated after today's visit, and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest coming up with a plan on ways to make sure cot sheets are not touching when they are stored. I would suggest making sure teachers are doing a daily toy check to remove any toys that are broken on the playground or in the classrooms. I would suggest checking the thermometer weekly to ensure that it reads 45 degrees or below. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must renew at least every 5 years.
