Upon arrival, I was greeted by the director, Tawanna Beasley, who assisted me during today’s visit. After Ms. Beasley left for the day, the assistant director, Tonya Williams, was present and signed the visit paperwork. LICENSE STATUS The center currently operates with a Five Star License issued on March 24, 2020. The program received six points in Staff Education, six points in Program Standards, and one Quality Point. The center is licensed to serve 141 children on first shift. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, La Petite Academy, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 1/28/26, and the center scored a superior rating with four demerits. -The last fire inspection was completed on 3/24/26. The facility will be reinspected after violations are corrected. -The last documented outdoor play area inspection was completed on 3/13/26 -The last documented fire drill was conducted on 3/13/26. -The last documented lockdown/shelter-in-place drill was conducted on 3/11/26 -The last annual compliance visit was conducted on 4/24/25. MONITORING I completed a walk-through of the center, visiting eight classrooms as well as the outdoor learning environments, kitchen, and bathrooms. The infants were being fed bottles, napping, and playing on the floor. The toddlers were observed playing freely with toys and eating lunch. Preschoolers were observed playing freely in centers and participating in circle time. School-aged students were playing on the playground. The outdoor learning environment used by students is fenced and contains a variety of permanently installed climbers with slides, as well as riding toys, hula hoops, and balls. There are two outdoor spaces: one for infants/toddlers/2 year olds, and one for preschoolers and school-aged children. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Ten staff and nine children’s files were reviewed. This program does provide transportation; therefore, this requirement was monitored and a violation was cited. I reviewed the storage of medications/diaper creams as well as corresponding medication permission forms and found them in compliance. The center serves children under one year of age, thus, safe sleep requirements were monitored and found in compliance. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Several children's photos were missing from their files in the vehicle used for transportation. 10A NCAC 09 .1003(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child who enrolled on 7/1/25 was missing a health assessment. GS110-91(1) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. A child enrolled on 3/27/25 was missing a signed discipline policy. .1804(c) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. A child enrolled on 9/9/25 was missing a signed Prevention of Shaken Baby Syndrome policy. .0608(b)(1-6) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by April 15, 2026. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 88%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -It is important to keep children’s medical records on file to ensure children are healthy and able to participate in activities. Today, one child was missing a health assessment. I recommend using the children’s file checklist under “Provider Documents” on the DCDEE website and checking files regularly to help you keep track of required records. Health assessments should be on file within 30 days of a child’s enrollment. -It is important that parents and caregivers are aware that young children’s heads and necks are especially vulnerable to injury from rough handling or harsh discipline. Today, one child’s file was missing the signed Prevention of Shaken Baby and Abusive Head Trauma policy. It is required to be on file for all children ages birth to five years. I recommend collecting all required forms from parents before their child’s first day of school. -Keeping parents informed of your center’s discipline policy ensures that parents and teachers are on the same page and may prevent misunderstandings in the future. Today, a signed copy of the discipline policy was missing in one child’s file. Documentation of the parent’s receipt of the center’s discipline policy should be on file by a child’s first day of school. -Transporting children is both a big responsibility and a great liability. In the event of an accident, it is important to always have emergency information for the children readily accessible to provide appropriate care. Today, the vehicle used to transport children did not contain a photo in each child’s file. Please be sure to include emergency information, including children’s photos, in each file for any child being transported by the center. CONSULTATION -You are pursuing Pathway #3 (using NAEYC accreditation) with a rated license assessment planned for December 2026. You stated that your center is on track to transition to your new license in December. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Your water was last tested on 9/14/23 and is due to be tested again by September 2026. -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. New staff members must complete training within one year of hire. -“Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) should be completed within 90 days of hire and renewed every five years. Upon arrival, I was greeted by the assistant director, Tonya Williams, who walked me through the center. The director, Tawanna Beasley, is at an off-site meeting today. LICENSE STATUS The center currently operates with a Five Star License issued on March 24, 2020. The program received six points for Program Standards, six points for Staff Education, and one Quality Point. MONITORING I completed a walk-through of the center, visiting eight classrooms as well as the outdoor learning environments. Infants were observed sleeping, being fed by staff, and playing on the floor. Toddlers were playing freely in centers. Preschoolers were observed playing outside, participating in Soccer Shots, using the restroom/washing hands, and playing freely in centers. The outdoor learning environment is fenced and contains a variety of gross motor equipment, riding toys, hula hoops, and balls. There are two outdoor spaces: one for infants/toddlers/2 year olds, and one for preschoolers. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Staff records for four newly hired staff members also were reviewed. Children under 12 months of age are enrolled; therefore, safe sleep requirements were monitored. Storage of medication/diaper creams and corresponding permission forms were monitored and found in compliance. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. Incident reports completed after 8/12/25 were not recorded on an incident log. .0802(g)(1-6) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Disposable gloves in space #4 were stored less than five feet high. .0604(q) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff member's (Y.G.) qualification letter expired on 9/20/25. G.S. 110-90.2(b) & .2703(n)&(o) 1534 Personnel records were not maintained on file in the timeframes given by Rule .2318(7). No staff orientation was documented in the file for L.R., who was hired on 5/27/25. .2318 (7) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A signed shaken baby policy was not in a new staff member's file (L.R.). .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member (L.R.) hired on 5/27/25 did not have documentation of this training on file. .1102(g) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. Criminal backgrounds checks for all staff members should be completed every five years. One staff member’s (Y.G.) qualifying letter expired on 9/20/25 and must be renewed immediately. They have 15 days to obtain a new qualifying letter; after that, they will not be able to work in your center until they have received their new qualifying letter. -Generation of incident reports is required by child care rule .0802(e) and maintenance of incident logs are required by Child Care Rule .0802(g)(1-6); both documents are an important measure in ensuring the health and safety of the children in your care. Injury patterns can be discerned from such records and can be used to prevent future problems. All incident reports should be documented on the Incident Log. Today, you had incident reports available for my review, but they had not been documented on the incident log since 8/12/25. Please document all incidents on the log moving forward. Remember, any time the child’s injury results in medical attention, a signed copy of the incident report must be sent to me within seven days after the incident occurred. -Safety is a top priority when providing care for young children. Today I observed disposable plastic gloves within reach of children in the diaper changing area in Room 4, which has two year olds enrolled. This poses a suffocation and choking hazard for the young children in your care. I suggest conducting a walk-through of the children’s space prior to children’s arrival each morning, removing any potential choking hazards and placing them at least five feet high. -It is important that all adults working with young children are trained to spot potential signs of abuse. New staff members are required to complete the class “Recognizing and Responding to Suspicions of Child Maltreatment” (part of the Health and Safety Training) within 90 days of hire. A staff member hired on 5/27/25 (L.R.) does not have documentation of this training on file. Moving forward, please be sure new staff complete the training within the required time. This training can be found on the Positive Child Alliance NC website: https://preventchildabusenc.org/online-trainings/. -Basic School-Age Care training is required for all staff who work with school-age children. Today, the lead teacher of your school-age group (L.R.) did not have this certification on file. In order to provide high-quality, age-appropriate programming, it is important that staff are well-trained to understand the development and care of school-aged children. You can find online BSAC training courses through the Southwestern Child Development Commission website: https://www.swcdcinc.org. -Staff members must review and sign the “Shaken Baby Head Trauma Policy” before beginning work with young children. LICENSE STATUS The center currently operates with a Five Star License issued on March 24, 2020. The program received six points for Program Standards, six points for Staff Education, and one Quality Point. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, La Petite Academy, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 8/5/25, and the center scored a superior rating with eight demerits. -The last fire inspection was completed on 3/3/25. -The last documented outdoor play area inspection was completed on 11/4/25. -The last documented fire drill was conducted on 11/6/25. -The last documented lockdown/shelter-in-place drill was conducted on 9/15/25. -The last annual compliance visit was conducted on 4/24/25. MONITORING I completed a walk-through of the center, visiting eight classrooms as well as the outdoor learning environments. Infants were observed sleeping, being fed by staff, and playing on the floor. Toddlers were playing freely in centers. Preschoolers were observed playing outside, participating in Soccer Shots, using the restroom/washing hands, and playing freely in centers. The outdoor learning environment is fenced and contains a variety of gross motor equipment, riding toys, hula hoops, and balls. There are two outdoor spaces: one for infants/toddlers/2 year olds, and one for preschoolers. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Staff records for four newly hired staff members also were reviewed. Children under 12 months of age are enrolled; therefore, safe sleep requirements were monitored. Storage of medication/diaper creams and corresponding permission forms were monitored and found in compliance. Criminal backgrounds checks for all staff members should be completed every five years. One staff member’s (Y.G.) qualifying letter expired on 9/20/25 and must be renewed immediately. They have 15 days to obtain a new qualifying letter; after that, they will not be able to work in your center until they have received their new qualifying letter. -Generation of incident reports is required by child care rule .0802(e) and maintenance of incident logs are required by Child Care Rule .0802(g)(1-6); both documents are an important measure in ensuring the health and safety of the children in your care. Injury patterns can be discerned from such records and can be used to prevent future problems. All incident reports should be documented on the Incident Log. Please remove the tricycle from the play area until the new part arrives, and be sure to inspect the playground each morning for hazards before children arrive. -Medication permission forms for diaper creams must indicate the amount of cream to apply (for example, “dime-sized”) as well as specific application instructions for how often to apply it (for example, “after every diaper change”). MONITORING I completed a walk-through of the center, visiting seven classrooms as well as the outdoor play areas. The infants were getting their diapers changed, being rocked, and playing on a mat with toys. The toddlers were observed in free play and transitioning to the playground. Preschoolers were observed playing on the playground, eating lunch, and playing in centers in the classroom. A full assessment of child care requirements was conducted. I used a checklist to monitor all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Five staff and nine children’s files were reviewed. This program does not provide transportation; therefore, this requirement was not monitored. Some children in the center require medication or diaper creams; therefore, medication storage and permission forms were reviewed. The center serves children under one year of age, thus, safe sleep requirements were monitored and found in compliance. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. No First Aid poster was available for reference. .0802(h) 847 Parent's medication authorization did not include required information. In the infant room, two children's diaper cream medication authorization forms did not specify the amount or frequency of the application of the cream. 10A NCAC 09 .0803(4)(6-9) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. A tricycle with a missing pedal was being used by the children on the playground. 10A NCAC 09 .0604(p) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Two children's files were missing this signed policy. .0608(b)(1-6) In order to comply with the NC Laws and Rules, any violations documented today must be corrected immediately. I need to receive your compliance letter by May 8, 2025. The letter must address each violation and explain specifically when and how it was corrected, as well as how you will prevent the same violations from happening in the future. Please send this to mandy.annunziata@dhhs.nc.gov and include the following information: -Facility name -Facility ID number -Each item number COMPLIANCE HISTORY Prior to today’s visit, the center’s compliance history score was 88%. According to NC General Statute 110-90(4)(d), all facilities must maintain a compliance history of at least 75% for the past 18 months. Failure to maintain compliance may result in an administrative action. The center was not in compliance with all applicable minimum licensing requirements during today’s visit and violations were documented that could impact the center’s overall compliance history score. An updated compliance history score will be generated after today’s visit, and you will be informed of the results during the next visit to your center. TECHNICAL ASSISTANCE -Two newly enrolled students’ files were missing a signed Prevention of Shaken Baby Syndrome policy. To ensure you have all required forms, I recommend collecting them from the parent before you begin caring for the child. You can also use the children’s file checklist available on the DCDEE website under the “Provider” tab. -It’s best practice to have easily accessible materials, including a first aid kit and instructions, that you can use in the event of an emergency. Your first aid poster was not available for reference. You can order a free copy of this poster from healthychildcare.unc.edu. Click on “Resources” and then “Posters.” Be sure to post it in your child care area. -A tricycle on the playground was missing a pedal, exposing a metal piece that could potentially scratch a child. You said you would order a new pedal. Please remove the tricycle from the play area until the new part arrives, and be sure to inspect the playground each morning for hazards before children arrive. -Medication permission forms for diaper creams must indicate the amount of cream to apply (for example, “dime-sized”) as well as specific application instructions for how often to apply it (for example, “after every diaper change”). The instruction to apply “as needed” is not acceptable. There were two children in the infant room whose medication permission forms did not supply complete directions as specified in the child care rules. Please have parents correct the forms. CONSULTATION -You can access and update your EPR plan by using your NCID to log into the North Carolina Risk Management Portal at https://rmp.nc.gov/portal/portal.aspx. Please review and update your EPR plan yearly. -Criminal Backgrounds Checks for all staff members should be completed every five years. Several staff member’s qualifying letters will expire in 2025. I recommend starting the renewal process soon. Background checks may be completed up to six months prior to the qualifying letter’s expiration date. -One of the biggest challenges that early childhood educators and administrators face is children’s behavior. Often these behaviors are developmentally appropriate, however, some behaviors can be difficult for a teacher to manage. Providing technical assistance, coaching and professional development for staff to prevent and address these challenging behaviors, as well as promote children’s social competence can be beneficial. If you would like assistance with dealing with challenging behaviors of children in your care, you can contact a Healthy Social Behavior Specialist at Early Years; they can be reached at (919) 403-6950. I will also email you some resources for addressing biting in the classroom. TRAINING REMINDERS -Training from 5/6/24 to 4/23/25 could not be fully reviewed today because the Staff and Training Worksheets were incomplete. Please complete all fields and email them for my review by 4/29/25. All required staff ongoing training hours for the current year should be completed by 5/6/25. A good resource for training hours is the North Carolina Rated Assessment Project website. They have several webinars explaining the new rating scales. -Health and safety training must be renewed every five years. Staff members who completed training in 2020 are due to renew in 2025. -ITS-SIDS training must be renewed every three years. Staff members who completed training in 2022 are due to renew in 2025. RATED LICENSE STATUS The NC State Legislature has extended “hold harmless” status until new rated licensing guidelines are in place, and you may opt to keep your current license and delay your rated license assessment until the new guidelines are adopted. Upon arrival, I was greeted by the assistant director, Amani Moxley, who introduced me to the director, Tawanna Beasley. Ms. Beasley walked me through the center. Both the director and assistant director began their new positions within the last month. LICENSE STATUS The center currently operates with a Five Star License issued on March 24, 2020. The program received six points for Program Standards, six points for Staff Education, and one quality point. Prior to today’s visit, I reviewed the NC Secretary of State’s website and noted that the owner of this facility, La Petite Academy, Inc., is listed as current/active. INSPECTIONS/DRILLS STATUS -The last sanitation inspection was completed on 6/10/24. -The last fire inspection was completed on 12/15/23. -The last documented outdoor play area inspection was completed on 11/13/24. -The last documented fire drill was conducted on 11/13/24. -The last documented lockdown/shelter-in-place drill was conducted on 9/24/24. -The last annual compliance visit was conducted on 5/6/24. MONITORING I completed a walk-through of the center, visiting seven classrooms. The infants were playing with toys on the floor with a caregiver, getting their diapers changed, and napping. The toddlers were observed playing freely with toys and eating lunch. Lunch today was a chicken sandwich, carrots, peaches, and milk. Preschoolers were observed in center playing with vehicles, building with k’nex and bristle blocks, and completing a turkey craft. A partial assessment of child care requirements was conducted. I monitored all applicable requirements, including but not limited to supervision, staff-child ratios, health and safety requirements, equipment and furnishings, and program records. Staff records for newly hired staff members also were reviewed. VIOLATIONS The following violations were observed and documented during today’s visit: Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. According to child care rules, an incident log should be completed any time an incident report is completed. This log shall include the name of the child, the date of the incident, the date the incident report was submitted to the Division (if applicable), the name of the staff member who complete the incident report, and be cumulative and maintained in a separate file. This log should be completed on a form provided by the Division. A copy of the log may be found on the Division's website. CONSULTATION -Your yearly fire inspection is due by 12/15/24. If you haven’t contacted the fire marshal to request a visit, please do so now. -The Clean Classrooms for Carolina Kids Program provides free facility-wide testing for lead in water and inspections for asbestos and lead-based paint hazards without any out-of-pocket costs. Tonia Cain, the director, was present and assisted me during the visit. I reviewed the visit summary with Ms. Cain and a copy was given to her at the conclusion of the visit. I completed a general walk through of the facility observing the indoor and outdoor environments. The Infants were being changed, fed, and having free play. The toddlers and preschool children were observed in center activities and eating lunch. CURRENT LICENSE STATUS Currently this center operates with a five-star license issued March 24, 2020, earning six (6) points in Staff Education, six (6) points in Program Standards and One (1) Quality Point. INSPECTIONS *The last annual compliance visit was conducted on May 17, 2023. *The sanitation inspection was completed on October 3, 2023, with a "Superior" classification. *The fire inspection was completed on December 15, 2023, and the facility is approved for day time care only. *The last documented fire drill was conducted on April 9, 2024. *The last documented playground inspection was completed on April 25, 2024. *The last documented lockdown drill was completed on March 7, 2024. Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, La Petite Academy, Inc. is listed as current/active. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. One feeding schedule in space #1 was not signed by the parent. .0902(a) 847 Parent's medication authorization did not include required information. In space #2 the medication authorization form to administer Baby healing ointment had "anytime" in the dates to be given and when to apply sections. The medication authorization form to administer Auqaphor ointment did not have dates to be given and when to apply. 10A NCAC 09 .0803(4)(6-9) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Three staff had expired First Aid training on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Three staff had expired CPR training on file. .1102(d) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The ITS-SIDS training for one infant teacher and assistant director expired in April 2024. .1102(f) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. TECHNICAL ASSISTANCE *To ensure each file is complete for staff and children you should use the checklist available on the DCDEE website. This will ensure all the required paperwork is on file. You should also check files at least twice per year to make sure there are no missing documents. I explained to the provider that good record keeping is important to the success of your child care business. *Medication forms should be checked monthly to ensure all information has been completed by the parent. ON-GOING TRAINING HOURS The on-going training requirements were monitored from May 2023- 2024 and was not in compliance. One staff has until 5/23/24 to submit 8 training hours. If the trainings are not received by that date a violation of not completing on-going training hours will be added to the visit. New staff files and two existing staff files were monitored. Lloyd must complete Recognizing and Responding to Suspicions of Child Maltreatment *C. Tyler and S. Clayton must complete CPR and First Aid training by 1/16/24. *T. Camara must complete Recognizing and Responding to Suspicions of Child Maltreatment and CPR/First Aid training by 3/5/24. *The Division of Child Development and Early Education is required by law [North Carolina General Statute 110-90(1a)] to establish an annual licensing fee for child care facilities. You will receive your invoice via email. Payment must be made online on the DCDEE website by December 31, 2023. Failure to pay the license fee is a violation of child care law (N.C.G.S. 110-90(1a)) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action, up to and including revocation of the child care license. *Please check the Division of Child Development and Early Education website: ncchildcare.ncdhhs.gov frequently and click the “what’s new” tab to stay informed of all updates available to you.
